The complication was, they operated to remove the undescended tooth and pack it with I forget what. After all that healed, I went ahead with the usual procedure, not problem for 15 years now.

Hello Doc, thank you for this video. I can share my experience with an upper front tooth. As well the dental jaw surgeon as my dentist advised go for the implant because teeth at left and right were not to good enough for a bridge. The surgeon I had was painful at only the seducian tranculizing injections, it were for my memory around 15 needles. It really hurts. But after that no hurting at all, the opening skin, drilling, surface equalization with artificial bone, implant screwinging in and sowing skin over it. Really amazing that this is possible. I am just a week farther now and there was no pain and no problems, just a bit of soft food needed. In a few month I go for the next step and that is opening skin and get ready for the crown on it. It costs a lot of money, but I think best choice. Thanks.

@@drhoulikThank you for asking about the next step. My dentist put a crown on the implant and also renewed a plate/cover on the other upper front tooth. No seduction/tranquilizing needed, just a little pain when pushed/screwed into the flesh. Before screwing crown on it, the hole was preventive cleaned. No pain and no infections afterwards. Details: Titanium implant with porcelain crown. It almost took a year since front tooth was mandatory pulled out. Total costs almost 2.500 euro.

There are some studies that show ~95% survival rate after 15 years. This doesn't mean there is an expiration date on them but that when the patient is a good candidate, OHI is maintained, they are done well and bite forces and controlled properly they should last a very long time @@yusuphasanneh4833

Hello! Really appreciate your work and effort to explain the procedure. I had 2 implants done, next to each other, about 8 months ago. Everything checked out good on the X-Ray, but when I went back for the abutment install, the surgeon said it has to come out (both of it) because there is, quote: "not enough bone mass". Now I am in a very confusing position, my problem is not solved, I paid a lot of money and I lost my trust in my surgeon to finish what she started. I will really appreciate if you can point me in the right direction. With regards, a citizen "in pain".

Sorry to hear that. That’s tough and I understand your position. If it’s the last two teeth missing ( no other tooth behind) the options seem limited. Either leave the space, make something that is removable or try implant surgery again. There could be a number of reasons why the implants failed. If there isn’t enough bone then there simply isn’t enough unless you rebuild the area with grafting. After things heal I think it’d be worth taking another X-ray and evaluating the site. If there is enough bone you could explore doing it again and would ask about a guided surgery. As providers the last thing we want is to have the patient pay a lot, things not work and end up in the same position but with the patient out the money. It seems it’d be worth asking if there is anything they’d be open to doing or could do to help. I cant and don’t want to speak for other offices as each have their own policies and implants/treatment could have problems for different reasons. We have a policy in our office that if we have problems we will apply the majority portion of what was paid toward alternative treatments or do it again and see if we have success. I hope that helps. @@habana7638 the films taken in advance should help confirm there is enough bone quantity and quality, then place the implant. Afterwords the X-rays shouldn’t be for determining if there is sufficient bone but rather confirm everything looks good and the bone is being maintained. I hope that helps

I just had 2 implants placed, but my dentist inserted bone into my lower jaw when he extracted the 2 teeth (bone was placed in both) then 3 months later I had the screw driven in and the metal cap placed, 3 more months I had the actual tooth installed. I guess why does your procedure not require a bone graft? Just for the record I have bad teeth, my gums are awesome, I brush, I floss I get my teeth cleaned 3 times a year, just unlucky I guess, my brother has great teeth but he’s bald…..I can get implants and no one can tell, if he puts a rug on his head everyone will point.

Dr. Houlik, great video. I had my back tooth on bottom extracted and bone was inserted as I was considering an implant. Since then I think am okay without a tooth there, but the gum is still very sensitive (a year later). Is there anything I can have done?

You can try using desensitizing toothpaste or gels. I have also checked with kriss ai and it said that the sensitivity might be due to the bone graft, the healing process, or another underlying issue such as infection or inflammation so it's best to visit your dentist and have it checked 😄

I had implant year ago (molar) and less than a week after I got a pretty bad infection. Was told it was due to foreign object and my body had to get use to it. Within the last two month got another infection. Both times had to take anti-biotics. I’m hoping with time this will stop. My experience was painful for both the implant and infections.

@Tiff: I got an implant a week ago. The surgeon/implant specialist put me standard on anti-biotics. No fun, diareha, but it works out no infection. Although I did not like the seducian tranqualizer injections before the surgery, all went fine, no pain. I haven't got the bill but as many described here there are huge costs. Although I will not be fined for $ 5000 as one reaction I do expect 2500 euro (almost same as USD). I hope you get a good sollution for your failed project. Dentals specialists are very good at their job.

You forgot to mention that there has to be a bone graft in the jaw bone and that takes about 6 months to heal!

I cracked a molar (#18) a month and half ago and according to scans the crack extends approximately 8 mm below the gum line which I was told by endodontist that tooth can’t be saved and has to be extracted😢 Now I have a week to decide whether I want have implant later so I should get bone graft at the time of extraction. I am 52 in good health in general and my teeth are straight after Invisalign treatment more than 10 years ago. Don’t have any issues until recently a big portion of my other smaller upper molar chipped off. My wild guess is I had used tetracycline when I was a child growing up in China which is commonly known for yellowing the teeth and possibly weakening them. I had all my wisdom teeth removed long time ago so the cracked #18 molar is the very back one. From what I understand I don’t have to have that tooth replaced. My concern is the long process and painful recovery. Also I am definitely allergic to metals (no earrings, skin can’t touch belt buckle…)Even though the implant is titanium but I am not sure how my body will tolerate foreign object being infused with my own bone/tissue. What’s your expertise on my case? Also do I really need bone graft if I decide to get implant later? Assuming I have enough healthy bone left. TIA

Hopefully this helps... 1- Yes - you don't necessarily have to replace the tooth. I have a lot of patients who are missing this tooth and do just fine. The top teeth could drift down, but the main takeway is you don't always have to replace it 2- The process can be long. It takes about 3 months for the bone to heal after the extraction and a couple months for things to heal after the implant is placed. In my experience taking out the tooth- patients have more discomfort with this compared to placing the dental implant (we do a lot of guided surgeries that are very precise and honestly patients experience minimal discomfort). So although it takes time to heal, it most often doesn't change their lifestyle, habits, etc... 3-To graft or not graft - per research- the area will typically heal better with a bone graft compared to not grafting the site. However, if the socket is in good shape after the tooth comes out and you haven't lost a lot of bone then grafts aren't always necessary. I have a lot of patients who elect not to do the grafting and we're still able to place an implant when everything heals up. For a 2nd molar -#18- your jaw bone anatomy, shape, etc... can be the trickier determinant in placing an implant versus did we graft or not. If you're leaning towards an implant I typically encourage patients to graft just because things typically heal better with it. Keep in mind though it's not always a make or break thing-I'm just trying to give you peace of mind and not feel like you have to make all of these major decisions within a week. You're dentist should do what's best for you :-) 4-With the metal allergy- this is a bit trickier. Have you done the MELISA test? They do make zirconia implants for these types of situations, they are new and more research underway. I have had one patient in 7 years who did report a metal allergy, after we placed the implant and we did in fact have problems with the implant integrating successfully (this patient also had other medial conditions that contributed as well, so not sure if this was purely a metal allergy problem or just all things taken into consideration). Each patient is different and needs to be treated as such. 5-I hope some of this helps, let me know if I can help in any other way or you have any other questions.

I have been wearing my retainers religiously unless there’s something wrong that I can’t for example I am having infection and swelling right now that the lower retainer is just too painful to wear. So I assume if I continue to wear my top retainer ( the bottom one may not fit anymore) after the extraction, it might help lower the chance of the top tooth drifting down. Good to know the bone graft helps with the healing after the extraction. Is there any chance someone could be allergic to the bone graft material? I will discuss with the oral surgeon before the extraction. I do intend to get implant so I can maximize the chewing power of my teeth and avoid using the other side more than I should. But the last thing I want is to get an implant then find out it’s more trouble than benefit. So I will get tested and do more research down the road. B.T.W. I have a square jaw so I should be a good candidate for the implant even without bone graft. Again thanks so much for your time and help!

So far I'm done the 1st step. I had the extraction done just a couple of days ago. In another couple of weeks the stitches will be removed, then I wait at least 3months, to start the implant procedure. Not really looking forward to the implant itself being done; the drilling into bone strikes me as being very painful, but I guess I'll find out. I wanted to have a clone done but the technology isn't there yet.

@Athena: After seduction tranqualizer injections there is no pain. Injection needles are pain, but you have to under go it. The drilling at mine were 3 drills and screwing in was like a 2-step, no pain at all. There is also a cap on it. Later it will be opened for puting a crown on it.

If it’s screwed in it will probably need to be tighter at some point. Make sue you know the tool required to tighten it, multiple tools and not all dentists have all of them. If the dentists chooses cement then there is no room for error, if something goes wrong you will have to have the implant drilled out. If the cement spreads to any part of your gums you will have bone loss, a very big deal. Most dentists don’t give the choice, they decide for you without telling you the cons. ALL IMPLANTS SHOULD BE DONE BY AN ORAL SURGEON. I’ve had both types, I would always choose screw it in only, no cement.

I had this procedure done. I had a failed root canal so the dentist had to extract the broken tooth and put the implant in. The doctor gave me a prescription for hydrocodone for pain. I only needed two pills. I’m the biggest baby when it comes to pain too. I have had bruises that hurt more than this procedure.

I have a serious question. I have a molar implant, I have no issues with the post that is screwed into the jaw bone, however, the crown. The crown is attached to the post with a screw, this screw would come lose "every" 3-4 weeks and at $125 to re-tighten it it was quickly becoming very expensive. Is this common, having the screw holding the crown in place to come lose this often? Per the Dentist, it was because of what I was eating? Now, I'm a 60 year old man about the "crunchiest" thing I eat are apples. I have a feeling that when the crown was 3D printed there was a defect that the Dentist wasn't willing to acknowledge. Today I'm searching for a new dentist that "hopefully" can fix this issue and I'm just trying to become more knowledgeable on the subject so I can have a better conversation with what ever Dentist I decide to visit.

Great question and sorry you are dealing with this... regarding the restoration loosening up every month. Some things that come to my mind regarding this... Something with the abutment/implant interface isn't fitting correctly (this has happened to me and I could tell it wasn't engaging right away and have had to have the lab remake it), your bite/occlusion (if the tooth is very large relative to the space or you are putting a lot of stress on it it could cause it to loosen up prematurely), implant location/angulation and torque level of screw. It depends. On average start to finish (getting seated, numb, post op etc...) Maybe about 45 minutes to an hour. We do a lot of guided implant surgeries at our office which speed up the procedure and increase accuracy of placement- the actual procedure sometimes in these cases only takes about 10-15 minutes because of the work that's done on the front end (before it's placed). The biggest issue that I have with implants is that most dental insurance here in the US does not cover it at all. I know someone who works for one of the major dental insurers and they do not even cover it for their own employees. We're talking in the neighborhood of $5000 per tooth out of pocket. I know 2 people who told their dentist "pull them all out and give me dentures" and lived happily ever after - no more dental bills.

Hi Doctor, thank you for the video I really appreciate it. But I had two failed implants on the same tooth. The first time I went I thought it went fine but couple weeks when I’m chewing. I keep hearing like a cracking noise didn’t hurt whatsoever but when I’m chewing on that tooth it sounds like I’m chewing on sand I went back and they took it out.and said the implant was shaken. So I have so the whole procedure all over. had to waited another 6 months. Went for the second time for them to do the 3-D and the were taken the small little screw out of the implant and he was tighten it and the whole thing came out now I have to go through this procedure again again for the 3rd time. I was thinking this time he probably tightened it too much that’s why it came out? They mentioned that this is very odd that it fell twice in a row, but I am just frustrated right now about the whole thing. Any suggestion Doc? I would really appreciate it.

I'd be curious to actually see the implant site- is there enough bone/bone quality ,are they doing it as a guided surgery, do you have any health problems, metal allergies, etc... Can you put too much pressure on the gauge to cost the implant to come out? The reason I’m asking is because when they were removing the healing cap the implant came out. Now I have to go through the whole process all over again. Any feedback or suggestions for me? Please

If the implant came out removing the healing cap a couple things may have happened. 1-implant didn't integrate and was failing/failed 2-Changing to abutment early in the healing process with possibly too much force. Which tooth #was it? and How long after it was placed did it come out with the healing abutment? I have had this happen to me before it wasn't from too much force but the implant had simply failed. We did what you described, cleaned out the socket, healed, and placed a new one with success. Assuming the socket heals well and the area is a good site, and you are a good candidate health wise... A guided surgery would be ideal to bullseye the next implant, place appropriate sized healing abutment and basically let it do its thing for at least 3-4 months with manipulation/changing, etc... I went in to get the 3-D scan so they can make the tooth. And he used the adjuster to take out the healing abutment. And when he was putting it back inside my mouth, I guess the implant got loose and came right out. I was thinking he turned The adjuster with too much force. This is my second time going through this process the first time the implant and the tooth went in OK I had it in my mouth for like a month but every time I chew on something it feels like I’m chewing on sand like something is cracking slightly cracking. I went back to the dentist and he checked it. I noticed that it was slightly loose so they did the process all over again. This is my second time around and this time everything failed while putting healing abutment into my mouth.

Thank you for the comment and checking out the video. Great question. 1-Most commonly a bone graft done prior to the implant is performed when you extract the tooth that is going to be replaced. The day the tooth is extracted the dentist would place bone to preserve the area as best as possible. 2-If the tooth/teeth are already missing then sometimes you can perform grafting to 'plump' up the area and prepare it for an implant. 3-Sometimes we will perform grafting the day of implant placement/surgery depending on what is necessary. Regarding if it's necessary or not. I always advise patients that the best thing to do is graft bone the same day you extract the tooth. Not because it's more expensive but when done properly it should preserve the area (bone/tissue) better in the area in preparation for the implant. I hope that help! Thank you

@@enricopallazzorecords8628 This is a great question. It depends.... On average the bottom arch usually takes ~3 months to heal after an extraction. On the top arch usually between 3-5 months (the more posterior on the top the longer it typically takes). So in general ~3 months for bottom and ~5 for the top arch before things are healed and ready for the implant in a delayed approach. Of course every patient and extraction site is different so these are general estimations. With that being said. You can definitely place implants the same day you extract if the area meets a certain set of criteria. This is most commonly done in the front (anterior) and premolar region. This is fairly common. You can also place the implant 'early' where you don't place it the same day you extract but also don't wait the full 3-5 months. This is fairly common as well. In short. It varies :-) There are a lot of ways to do this. The decision will be made by your provider regarding which route to go depending on your bone, health history, extraction site, etc... a whole list of things. They should make the appropriate recommendation as far as ideal timing. I hope that helps. Thank you!

After a couple months we then go back in, numb the area and uncover the implant which includes removing the gum tissue right on top of it. We then place the healing abutment and let that shape the tissues for anywhere 1-2 months and then start making the tooth.

@marcel: Yes, I was amazed too, that the jaw bone was strong enough after (several) dental root removal. The specialist took time and analized x-ray photo's. Both he and my dentist said go for implant. My implant is in, recovering, anti-biotica, no infections yet. I think it looks good. It is only very expensive.

What happens if the implant keeps twisting round and doesn't take place? She left it like that and said my bone will lock it in place once it starts healing.

I’ve heard of similar cases like this and the implant healed ok. Ideally the implant is threaded well into the bone. Keep me posted if you need anything

Thank you for the question. I can do a video on this but in short... Depending on where you are trying to place them if there isn't enough bone you may be able to go in and do grafting to prepare the site for an implant. If there has been too much bone loss and grafting is going to be difficult in order to get enough bone then sometimes the options are limited and may not include implants.

Thank you for the comment and checking out the video. Same day can mean a lot of things.... Regarding a single tooth implant typically we place the implant, let it heal for a period of time (amount of time will vary), and then fabricate the tooth to put onto of it. Sometimes on front teeth (anteriors) we will place a provisional/temporary tooth 'the same day' and then fabricate the definitive (permanent) at a later date. In short... for a single tooth implant--> place implant, let heal and then make tooth for it.

It can take 6-12 months before a crown can be placed over the implant. The bone has to heal.

I’ve had 2 implants and there is much more to the entire process than this. My advice is consult with your dentist and if it’s a tooth not visible with your smile just have it removed. An implant takes several office visits.

It took a year for my husband. Sinus lift, bone graft, healing time, abutment, healing time, then crown. So far, $6,200 before the crown. Who knows how much that will be.

Hello doctor , my implant was failed, with in 2 weeks, after placing the implant, after a week it was infected severely, then after one week I lost the bone and the implant came out , so what do u suggest me to do next

Do you mind me asking which tooth number it was? Next steps would seem to be to let it heal and then reevaluate options in ~3 months. Could either try for another implant, bridge and or consider something removable. It might be a good idea to ask your providers/office what type of policy they have in place or if they will apply any of your funds towards future or alternative treatment.

@@nikhilnallagonda4301 I can't make an official recommendation without seeing your case, X-rays etc... The front teeth are a bit more complicated when it comes to implants...technique wise to make sure the implant is placed well and your tissues respond/heal how we want as well. But based off of what you're saying. It seems the scale may be weighing towards doing a bridge. It would seem appropriate to ask about policies regarding cost and how good of shape the adjacent teeth, bone, gums are in. I hope that helps!

This is typically precautionary to reduce the chances of post op infections and most providers will do this.

​​@@sct4040I have Delta Dental. Looking at my own personal Pre-Treatment Estimates Delta Dental actually pays quite a bit more than I imagined they would. I see more 50% than 0%. Even the Surgical Implant Body-Endosteal (most expensive part) they even pay 50% 😀. September 22, 2023 I had extraction of tooth #29 and Bone Graft. And today 12/28/23 at 3:30pm I get the Implant Endosteal inserted. I am very happy I got the necessary bone graft, with the long term in mind. I do hope DD will pay for more for you. This procedure was very important to me. More because I want to chew my food properly so I can prevent any digestive problems that can appear later as result of not chewing my food properly due to missing tooth. I'm no Doctor. But I avoid and don't like to talk while eating. I'm always saying people should be quiet and chew their food up properly lol.

I spent £4000 to have 2 front teeth implants .. I was so happy and took my mates clubbing to celebrate.. we got into a fight and I tried to help Charlie who was getting jumped. I got both teeth knocked out and they fell in the road drain along with the keys to my car !! Total loss £4800😂
