It does not lead to an inactivation of the NA/Ca exchanger but rather the increase in sodium concentration activates it and the sodium is pumped out of the cell in exchange for calcium that leads to increased intracellular calcium and positive inotropic effects.

yeah !!! he miss a point .actually na-Ca exchanger is inhibited , so that Ca doesn't goes out of cardiac muscle in exchange of Na. but the ca concentration is increase by influx through L TYPE Ca channel

The secondary active transport sodium-calcium exchanger will depend on the concentration of sodium outside the cell (extracellular). The more sodium outside, the more it is active in pumping sodium inside the cell in exchange of calcium which is pumped outside the cell. The extracellular sodium concentration is maintained by the primary active transport sodium-potassium ATPase pump. Potassium is pumped inside in exchange for sodium which is pumped outside. Therefore activation of the sodium/calcium exchange (secondary active transport) will indirectly depend on the sodium potassium ATPase pump (primary active transport). The mechanism of Digoxin is that it will inhibit the CARDIAC sodium potassium ATPase pump, decreasing extracellular sodium concentration, leading to the inactivation of the sodium-calcium exchanger. Calcium will not be pumped out therefore increasing the calcium concentration inside the cell. Calcium triggers muscle contraction, hence it will cause the heart to contract. A phenomena we call positive inotropism. Digoxin is an example of a positive inotropic drug.
