Oh, and one possibly important thing we covered in a Step 1 prep course I took was that alkaline pH of urine favors the formation of calcium phosphate stones, so idk if that might show up in a question.

Calcium phosphate stones are associated with primary hyperparathyroidism, alkaline urine like in RTA, and medullary sponge kidney. Also, apparently betel quid chewing according to UWorld.

Wouldn’t the diagnosis be most consistent with ureterolithiasis? Nephrolithiasis is inferring the stone is still within the kidney but given the pt’s ipsilateral testicle pain and hematuria, the stone appears to have moved into the ureter.
