The year of 1979, to be honest, was a terrible year for one of the world’s Largest passenger plane. The Douglas DC10, even before 1979 suffered multiple hull loses and even for a time, held the accolade of being the aircraft involved in the deadliest air disaster to have occurred at that time. It’s a plane which certainly has a divided history. It’s pilots loved the plane. Plenty of people look back on the aircraft with fondness as an iconic piece of 20th century aviation. On the other hand, it had developed a reputation among passengers and the media due to its string of high profile incidents. 1979 was the plane’s deadliest year. Whether or not this reputation was warranted is up for debate. Because as with the case of this accident, there really wasn’t any wrong with the plane. Still, Western Airlines Flight 2605 contributed to that long list of accidents on October 31st, 1979.

I saw a plane crash documentary where an NTSB investigator said that pilots who listen to crash tapes (generally to identify the voices of the pilots from the crashed plane, so the NTSB know who is who -- the NTSB version of going to the morgue to ID a body) generally quit flying within two years. That tape made it easy to understand why that's the case.

The weird thing is, it sounds like you can almost hear more voices in the last milliseconds of the CVR. Eduardo Valenciana suggests that it’s the sound of the first class cabin coming closer to the CAM as it continues to record milliseconds after impact, as the front cabin crunches into the cockpit…he may be right.

It reminds me of another that disturbed the shit out of me. I think it was a collision between a 737 and a smaller business jet. The 737 loses half a wing and begins spiralling uncontrollably, and you’d think the G-force would’ve knocked everyone out, until you hear a whimper break the silence of the flight deck. No hysterics, no panic, just despair and defeated fear.

The DC-10 was an amazing widebody plane for its time. I flew on them countless times in the 1970s and 1980s, but we never knew about the cargo door problems, or any of the other dangers. My family flew to Mexico on a DC-10 in 1975 and we had a great time. Ignorance is bliss, as they say. This crash has probably the most haunting CVR recording in history.

"The DC-10 was an amazing widebody for its time". You're obviously not an aerospace engineer. A DC-10 failed a fuselage test during development. Douglas knew about the cargo door flaw but reasoned that future law suits were cheaper than changing the design. On the Tristar the leading slats remain extended even if the hydraulics fail. On the DC -10 they do not. This was the main cause of the crash of AA flight 191 in 1979. The DC-10 outsold the Tristar by a factor of 2:1 because it was inexpensive. The Tristar was twice as good but twice the price!
@REPOMAN24722

@@tumslucks9781 Also all the redundant hydraulic lines are right next to each other which in one incident severed all or most of them when an engine exploded making the DC-10 lose control, even if the tristar lost 2 systems the plane would still have control of all surfaces.

People in the comments always blaming pilots... it's disgusting 😔 Humans make mistakes, it's completely normal. It's up to the other parties (ATCs, airports, airlines and the aviation authorities) to eliminate the risks.
@beenaplumber8379
@beenaplumber8379 5 ай бұрын
@@KoffinKat Yes, everyone makes mistakes, and that's all the OP said. It's not up to those other parties to "eliminate the risks". It's so much more nuanced than that: 1) Legally and practically, the captain has the ultimate responsibility for safe operation of the aircraft, no one else. 2) You can't eliminate risk, only reduce it. 3) It's up to everybody, even gate agents and catering, to reduce risk as much as possible. 4) The FAA is responsible for regulating safety, but they do a piss poor job of it. They are not required to adopt NTSB recommendations after accident investigations, and all too often they don't, and more people die. They are also charged with promoting aviation in the US, and the one conflicts with the other. They have developed dysfunctionally cozy relationships with Boeing and the airlines, again resulting in a lot of deaths. That said, according to the NTSB's practice, pilot error is not (or is no longer) a sufficient cause for an airplane crash. The NTSB keeps digging to learn why the pilot made the error, and usually there are underlying issues, such as airline policies concerning training and schedule pressures, medical issues, poor CRM, and other human factors (plus many others). Looking beyond pilot error is how we learn to prevent future occurrences. Crash reports now list causal factors, and if pilot error is among them, they will say something like "This accident was caused by the flight crew's failure to (insert error). Contributing to this was inadequate training by the airline, misleading instrumentation, and (insert other factors)." We learn a lot more that way.

I always feel safe whenever I went on board a plane with older pilots...usually just looking at their greying/white hair alone will instantly calm my nerves. And yes, my whole life I always found that older pilots gave me the smoothest flights and softest landings. This recording shows that even a captain with 31,000 flight hours in his hands was still a human being just like the rest of us. Hearing his screaming in fear in the end almost made me cry...a truly sheer terror from such a highly experienced airman, calling for his God, facing the inevitable. While it is a unsettling CVR, I personally think Pulkovo Flight 612 CVR is worse (there's a vid on this channel for it). At least the terror in this one was short, not a couple of mins like Pulkovo. There's a JAL recording where the captain is screaming the whole time. You hear the engines spool up and screech louder and louder behind him, you hear the trees impact the plane, and you hear the pilot scream in Japanese "IT'S THE END!" just as the CVR cuts off. That one and this one are two of the most disturbing CVR's ever, alongside the 9/11 CVR's where you can hear the pilots gurgle their dying utterances after their throats were slashed by the hijackers. I hope that they put in some very specific guidelines about when a go around must be performed on this side stepping procedure like, if you are unable to see the runway at some given distance and altitude, go around. I know this sort of thing exists for normal approaches but it seems like you'd probably want a bit more distance and altitude to sort out the whole side stepping thing since you don't want to bank the aircraft too close to the ground or too sharply if you find yourself way too close to the runway to bank more gently to land on the parallel runway.

Glad to hear in this instance that the DC-10's less than stellar record was not always a fault of the aircraft itself. Of the three accidents mentioned in the video, the Western Airlines and Air New Zealand flights were due to pilot error or a combination of pilot and tower/traffic control errors. The Western Airlines flight could have definitely been avoided if the pilots had simply followed published procedure to abort the landing if the runway wasn't in view at 600 feet of altitude.Then the question as to why Mexico City had an out-of- commission runway's ILS active. It's a definite liability. It may have made it easier for pilots to land by lining up on it and then doing the side-step procedure to land on the actual active runway. Both the active ILS and side-step procedures leave wide room for error. As for the Air New Zealand flight, again it was pilot error involving low visibility and white-out conditions due to the snow, combined with the aircraft's position having deviated from the original fight planned route, where the pilots didn't realize the deviation until suddenly rushing up on the side of Mt. Erebus. For all the people, both passengers and flight crews onboard all of these flights, my heart goes out to them and those they left behind.

Technically, the 1979 American Airlines DC-10 crash at Chicago O'Hare wasn't exactly the fault of the aircraft either. A short cut using a forklift was used by American Airlines mechanics to remove the DC-10 wing engine for maintenance from the engine pylon unknowingly stressing that pylon. This shortcut repeatedly used led to an eventual crack that caused the engine pylon to fail in the Chicago crash.

Another high quality video from you there about an absolute tragedy there that all of us who work in safety critical industries can learn from. Standard phraseology saves lives (makes it easy for us to build an accurate mental model) Beware the Window of Circadian Low (the hours between 2am and 6am where your body naturally wants to be asleep. I've worked enough night shifts to know just how easy it is to let stuff slip at those times. The sorts of mistakes you'd NEVER otherwise make) And above all else, if you're unsure about something, speak up. Ask, clarify and make sure you know what is supposed to be happening. In the mid to late 2000's I worked in Africa on mining and mineral exploration projects (I'm a geophysicist) we often had to fly in and out of various African countries on local Airlines. On one project I worked on in the DRC when I had finished my field shift I had to fly on a local Congolese airline from the town we were working in, Mbuji Mayi, to Kinshasa the capital city where I connected with a South African Airways flight back to Johannesburg. The local airline aircraft I flew to Kinshasa on was a DC10, as I walked across the apron to board I looked up at the tail and I could see the Air New Zealand logo as seen on the Air New Zealand aircraft shown briefly in this video, it had been painted over but it was still clear to see through the white paint covering it. The flight was actually really nice in spite of the aircrafts age it was very comfortable to fly in, the seats even in economy were very large soft leather covered seats with big leather covered armrests, almost like a big comfy lounge room seat and there was heaps of legroom. Other flights I took in Africa weren't so pleasant and comfortable, I once flew from Kananga also in the DRC back to Kinshasa on another local airline and the aircraft was a very old Antonov, an AN25 I think, a fairly large twin turboprop aircraft, it was part cargo and part passenger the cargo was at the front and there was a passenger section at the rear, the rear door that would usually be used to board passengers was inoperable we had to board at the front through a big freight door then make our way back through all the cargo to the passenger section, my seat just happened to be right next to the inoperable rear door which was labelled as an emergency exit inside but couldn't be opened and was bolted shut, my seat was broken and leant to one side and it didn't have a seat belt, they gave us a "meal" on the flight but it consisted of a single big bread roll, no butter or anything to put on it and it was just handed to me with no tray or plate or napkin, along with the dry bread roll I was also handed a large 750ml bottle of local beer but with no plastic cup to drink it from 🤣, I ate the bread roll and drank my big bottle of beer straight from the bottle, it was an interesting flight to say the least. Oh and the cargo in the front section was mostly some type of dried fish, I could tell that from the smell in the passenger section 😏

The only thing they did wrong was not bail out at 600 feet. The pilots had every chance. I hate to say it because of how horribly they died (that CVR is terrible) but they KNEW they were on the wrong runway, they blew through their minimums, and there is literally no one else to blame but them. The buck, and many lives, stopped with them. There's no call for a go around, no discussion of what what to do now that their on the wrong runway, etc. Total pilot error and no excuse for it. It wasn't even their first time landing with the other runway closed!
@jonchowe

I remember the DC 10 with fondness. I flew on that plane back in the 80s with American Airlines and Northwest Airlines, including overseas trips. Once McDonald Douglas fixed the problems with the DC 10, it became one of the safest planes in the skies. Unfortunately, it’s reputation never recovered. That’s sad. Watching this and considering that the runway configuration remains the same at MEX feels odd. Now at least now both runways have ILS, but Flight 2605 was one of the early signs the goddamn airport is well done. We almost had a collision in May 2022, and I know many people are blaming ATC but nope, any basic geography teaches Mexico City is a pain to land in.

... meanwhile, its sister aircraft was subject to a mad escape by John Matrix whilst on its way to Val Verde. Matrix managed to eject himself from the DC-10 whilst it was lifting off from LAX. Their flight attendants were Susan and Lance. Apparently, the perpetrator felt airsick and managed to climb down the front nosegear to escape. Unlike DB Cooper, Matrix achieved his mission in the end to save Jenny. The tragic tale to this story is that he probably killed as many people as those that died on this fateful air crash after he was denied the request for assistance from General Franklin Kirby.

That CVR of the final seconds before the final impact... It's amazing enough that most CVR in aviation disasters don't involve pilots letting out screams like passengers during a terrible turbulence-they often shout words in a desperate attempt of salvation but not produce unarticulated cries of impending doom. You would think that the most chilling final words coming out of that pilot would be something like "We're gonna fucking die" or "We're done for", but instead you actually hear a series of pure uncontrollable shrieking of someone so terror-stricken by the death quickly awaiting him. The English language doesn't do it justice to describe the noises coming out of that captain to those who are hesitant to listen to it. It was seconds of pulsed shrieking that's more prominent than the impact noise in the CVR; it's as though there was passenger in the cockpit is suddenly being stabbed to death by someone as the plane was about to make the final impact. Take the "One of the more unsettling recordings to be released" warning as an understatement. I believe it's one of the most horrifying CVR publicly made available. The captain was not accepting his deadly fate at the slightest and his final cries clearly demonstrate it. Words don't describe one's own overwhelming fear of dying quite as well as the inability to produce such words from being so overwhelmed.
