Sir, in paget disease you mentioned that if mass is not palpable then the underlying cause is DCIS .but it is mentioned that even in DCIS it produces palpable mass ...

You are right DCIS initially can begin as non palpable then can go on to become palpable mass too.. So if underlying mass is not palpable then it could be DCIS which is not palpable one If it is palpable then no doubts...it is ductal carcinoma.. could be in situ invasive Hope This clarified

A 50-year-old woman presented with a 7-month-old right breast mass. 【 Case Introduction 】 The patient was a 50-year-old female. A right breast lump was found for 7 months. Physical examination: a hard mass of 2cm×2cm×1cm was palpable at 5 o 'clock on the right breast with unclear boundary. Color ultrasound examination showed that the right breast had uneven weak echo area, irregular shape, no capsular echo, suspected of cancer. There is an enlarged nodular shadow in the right axilla. Ultrasound-guided puncture was performed and the pathological diagnosis was invasive carcinoma of the right breast. He was admitted to the hospital and underwent right mastectomy and right axillary Ⅰ and Ⅱ level lymph node dissection. During the operation, it was found that at 5 o 'clock of the right breast, 1cm from the nipple, there was a gray-white hard mass, size 2cm×2cm×1.5cm, ill-defined boundary, irregular shape, fish profile. There were multiple lymph nodes of 0.5-1.5 cm in diameter in the right axilla. [question] 1. In the HE staining images of the tumor, morphological changes supporting the tumor as invasive carcinoma include (multiple selection) A. Tumor cells formed irregular mass nests and grew infiltratively B. Destruction of normal breast tissue C. The tumor cells were densely arranged, with increased nucleoplasmic ratio and certain atypia D. Tumor cells constitute the parenchyma, and fibrous connective tissue constitutes the interstitium E. Tumor cell morphology was mild 2. What clinicopathological information of the tumor needs to be confirmed in order to predict the prognosis of this patient (multiple selection) A. Histological classification/grade of the tumor B. Axillary lymph node status of the patient C. Human papillomavirus (HPV) infection D. Whether the tumor has distant metastasis E. Whether the patient has a family history of breast cancer 3.Pathological diagnosis?

Sir how come papillary dcis can lack myoepithelial cells... It is pre cancerous condition and should have both basement and myoepithelial cells right ??

You explained the prognosis of DCIS is based on grade... How is DCIS graded ? Are some subtypes of DCIS intrinsically high grade, like in the case of Brain tumour ?

It's nuclear grade.. Low grade when the nuclei are regular and smooth contours.. High grade if highly pleomorphic... Intermediate if in between! 😊
