Thank you for publishing! There is an error with the optional SYNC BOARD! C5 (100NF) goes from +12v to -12V! it should go from -12V to GND ! C3 (100NF,+12V to GND) is missing, it may not be that important that C3 is missing ? you didn't specify the PWM change of the resistors! It didn't work for me at first, it was on the 2N5458 for sync, my transistor came from China and didn't work, EVERYTHING didn't work, no sign of life, then I took a 2SK30A with a different pin design and it worked great right away!
