Doctor Reviews Mason Rudolph Scary Knockout and POSTURING after CONCUSSION

Mason Rudolph of the Pittsburgh Steelers was knocked out in Sunday's game and appeared to show signs of abnormal posturing after sustaining a concussion. This is called a "fencing response" and indicates a more severe brain injury with production of abnormal reflexes from the brainstem. We'll look at the video and explain what is happening to cause this.

As I stated in another comment. Evidence based medicine has clearly shown immobilization on a back board is significantly more harmful than helpful. Patients in the field are allowed to self extricate and walk to the stretcher on a regular basis. That being said, a c-collar at least would be appropriate. If you work in a system where you still routinely immobilize on a backboard, you should speak to your medical director about a protocol change. It is hurting your patients.

Backboarding isn't our go-to anymore since a couple years ago, but a c-collar I absolute would throw on.

jefftkey if they ran the certain tests on the field he was fine enough to walk off honestly i think people over reacted to this because i see him getting hit in the chest pads then the facemack he has support in the neck it didnt extend back he wasnt hit in the head and the helmet im sure are better then when i played football. I see this as wind knocked out when he actually hit the ground was more interesting then the actual hit by the defensive players from the looks of it but still not that scary!!

It's easy to say that now. But at the time, they had to suspect a neck injury. Unless these "certain tests" included a CT scan, a back board or at least a cervical collar should have been used.

It was the ground that caused most of the damage. Clock anyone in the jaw hard theyll go down like that. It's just his he'd bounced. He can recover from this, the grounds a lot softer them direct helmet to helmet; which this wasn't. The ground caused the concussion not the hit.

I am wondering if one of the main reasons he was walked off the field, despite it not being the most medically sound choice, was to ease the drama and concern of fans and players? EMS was obviously there and ready to go, so I suspect that much of the choice made was for "non medical" reasons, though it wasn't perhaps the safest choice for his injury

Thanks for explaining this in simple terms. I was in a bad car accident fell out of the car and suffered a tbi. A nurse on her way home saw the crash and pulled over to help. She held my head until the medics got there. I’ve been told she probably saved my life and prevented anymore brain trauma. I’ve always wondered how she knew to hold my head and this explains it.

I had concussions in high school and college from football that were never caught/diagnosed because in the mid 90s it wasn’t part of the protocol for athletes like it is now. I also have competed in combat sports like boxing and MMA. I remember losing the ability to do minor tasks for about a week or two, like gripping a pencil properly and writing. Reading because a struggle because things were a bit blurry and my head hurt. I don’t know how severe it was. Never lost consciousness from any of them. Was able to finish out games and practices every time because the mentality back then was for us to shake it off and get back in there. I don’t want to think about what long term effects it has had on me, now that I’m 38. I do feel like I am more short tempered than I ever was before and will sometimes struggle to find the proper words when I’m talking or explaining something to someone. I have chronic headaches that feel like migraines. With all that being said, I enjoyed my years playing football and don’t regret it. I don’t tell anyone that they shouldn’t play or that they shouldn’t let their kids play. It’s a decision they have to make for themselves. My son does not play football but if he ever wanted to play I would probably consider letting him but wouldn’t want to make him fearful of injury by telling him what has happened to me. Playing football with fear is probably more dangerous than playing recklessly.

Bruce Gelman you can get a concussion playing soccer, hockey, volleyball, wrestling, etc. You can smack your head getting in or out of a car. There are plenty of ways to become concussed. It isn’t just in football or boxing. The risk can be higher but in many ways they have attempted to make the sports safer through rules and equipment. I’m not saying i would be in favor of my son playing, but at the same time I can’t say I would tell him he couldn’t. It’s not as simple as saying yes or no. There is way more to the topic than that. I certainly would never force a child to play a contact sport. I experienced more injuries in baseball and wrestling than in football and combat sports combined. I mean sure, those were bone breaks or tissue injuries rather than trauma to the head (I was hit in the head by a pitch in baseball; no concussion symptoms from that). Maybe I’m just super unlucky. Some athletes seem to never be concussed or injured.

Thanks for the discussion. In early 2018, I fell and hit the back of my head. I awoke forty-five minutes later. I could hear children chanting the old rhyme, "It's raining. It's pouring. The old man is snoring. He went to bed with a lump on his head and didn't get up in the morning." A minute or so after I revived, I threw up. There happened to be a bucket nearby, and I threw up in the bucket. When I did, I put my hand down and into a puddle that I'd vomited before I came back around. I then desperately needed to have a bowel movement. When I finished that, I considered going to bed, but the nursery rhyme made me think that going to sleep would be fatal. I called 9-1-1 and went onto my enclosed front porch to wait for the ambulance. While I was waiting, I threw up again and faded out again. I awoke to the 9-1-1 operator trying to get my attention. I eventually was able to talk to her and told her that I could see the flashing lights. I'd opened the door before I passed out again, and the EMTs came to me about that time. I have a disease that no one has really diagnosed anyway, so I'm not functioning well anyway. I was already trying to get disability when I fell. I don't remember having any symptoms after I got home. I was in the ER for about six hours before they released me. They didn't find any damage on the X-rays, but the back of my head has never returned to normal shape. Obviously, I have no idea whether I went into the fencing posture. I live alone, so no one could have known that. Now, I wonder about that. I wonder whether I would have come around within a few minutes if I'd had people talking to me as they talked to Mr. Rudolph. That he never threw up seemed like a good sign at the time. Either way, these things interest me a great deal.

Thank you to everyone for your concern. I'm about 99% certain that I fell because of a medication that I was taking at the time. I still get dizzy sometimes, but the incidences are much fewer and much less severe now that I've gone off the medication. I don't have anyone that I want to have living with me. I'd rather die than be in assisted living. For the time being, my situation is what it is.

To my eyes (definitely not a doctor) it looks like his hands are doing exactly what they do at the end of a throwing motion, coincidentally the moment he got hit. Right fingers extended, wrist turned out, left arm tucked in with hand clenched. Is this a thing sometimes with knockouts? I've seen several knockouts in MMA fights and other football injuries where it looks like the athlete's body is trying to do whatever they were doing the moment the lights went out.

@@punishedhuey4797 they're similar, but the QB's right hand is in a much more specific position. The end of a throwing motion puts the hand in that outwardly rotated spot with the fingers extended. It just seems more specific than a general fencing response

Meaning we have come to find since 1972 backboards are more damaging then helpful unless used for extrication. My question is why is sports medicine different in this aspect?

hey love your stuff. I'm a pro baller in Europe and I recently suffered a grade 3 contusion with tearing and all in my quad. It has since progressed into miositis ossifcants and I've read that its a common football and basketball injury. I was wondering if you could do a video with your opinion on such injuries and the approach you would take when it comes back to the return to sport while minimizing the effects/growth of the miositis ossification. I am also a casualty of head injury. Recieved during football back in '79. Knowing what we know now, we can look back and see that is when my personality changed. I went from an A & B student to C's and D's. Became rebellious, impulsive, my fight or flight went to fight or fight because even now I still don't realize the whole situation at the time. I have learned to deal with it and have made adjustments over the years to become more functional and successful.

Lipstick Zombie football gear is not 70+ pounds. probably around 12 pounds with most of it being in the helmet. shoulder pads are extremely light now a days

There has been little mention of the hit from behind flinging Rudolph's upper body forward and his head slightly downward. Thomas' helmet, IMHO, would have hit him below the chin absent that rear-side hit, not to mention the hit from the rear created something akin to the doubling of any impact, like a car crossing the centerline and hitting the car coming from the other direction head-on. Also, the hits which don't allow the hittee to recoil a bit, such as where the ground is on the other side of the direction of the force, or, as in this case, another player's mass inhibits any recoil, produce the most gruesome injuries in football. It's gladatorial combat in the 21st Century, fun to watch if you can de-personalize the players, but brutal to play. OTOH, if your most wonderful skill and money-making talent is to be a football player, who has the right to tell you to work in a factory, instead, and struggle to pay your bills and die without anybody outside your immediate family ever having known who you are and what you could do? The NFL should have a comprehensive lifetime medical insurance program in place and fully endowed to care for their players after retirement, and to save society the cost of caring for them. If they did that, and if the players were fully informed of the risks inherent in the game, blow the whistle and let the kickoff commence. But this incident is noteworthy as showing that all the new rules seeking to limit or prevent concussions will never succeed in their goal. There is just too much random mayhem on the field, and the guys just keep getting bigger, stronger, and faster year after year.
