I know im super late nut what really gets me is that very last scene. He says "they break my heart." It's one of the few times he doesn't say 'hearts' and it always seems to me in that moment he feels completely human and broken and sad and that's why he says it. I'm probably wrong about it and it's just a line that they didn't think that deep into but I'd rather see it my way.

what if the line was, They break my heart... But the other one keeps beating... (symbolising how, despite losing those he loves, his timelord body keeps going and the doctor is forced to suffer. which i'm sure you got, its not a very hard thought to come up with, im going on too long, anywayz L8r.)

Well it doesn’t have to be biologically accurate... it’s not like our human hearts literally break either It’s just a saying, that what he did. He used a saying to portray how it felt. You are bring up an issue that doesn’t exists
