Something to take into consideration first: It's a Tv Show XD. And by that I don't mean that "all they show is fake"; I mean that the production has time and money constrains that prevent a most accurate depiction of the cases presented. Also the production has a goal that is not entirely focused in medical realism, it's romance coated with medicine, but not a medicine manual coated with romance. That being said, let's check some facts: Once in a while (more regularly han one would think) people end up impaled by objects that need to be removed carefully. Some medical journals (or newspapers) might have some more detailed accounts of the challenges of how difficult the process is. In general, it depends on several factors like the object, the damage done and the equipment available to deal with the situation. In this case the object is a large metal pole and that's a problem. When you use large hardware to cut a metal object there is an unwanted side effect: the whole object heats. A careless cut would end up applying searing heat not only to the wound, but to the internal organs around it. Second, metal's are not flexible: they vibrate as they are cut. That means that a quick careless cut that would (in any other way) split the bar in two would cause unknown internal damage (the reduction of the metal object is commonly don by firemen at the scene of the accident). You normally don't want to leave a big piece of the object sticking out of the person, but you can't cut too close you produce internal damage. The problem of heat leads you to either use something to dissipate the heat from the object before it reaches the body, or you can use a less powerful tool to cut (which in turn increases the time the victims will be impaled, but you have to be very precise because the pole will still move with each cut and you may need several attempts if you are not in a good position to cut). And that's mostly why they can't get away with not splitting the bar (in this specific scenario, with the resources given): they are just too close to safely manipulate any tool in the space between them without the risk of killing them both. The other thing to keep in mind is that, once they begin to work on the wounds, they have to have a plan for each victim because once the pole moves, things more likely will happen quickly (most likely the first goal is to remove the pole and stabilize the patient and then repair any other damage done. Could they have had a better plan? Yes. Would that end be more satisfying? Perhaps. The hospital worked with what they had and what they could do (and the script writers did the same in creating the episode). Each time this happens in real life it's a challenge for everyone involved. One case study [Warning, graphic images inside]. The victim had to be operated twice because the doctors didn't dare to extend the first surgery more than 60 minutes after removing the object. This show constantly challenges my ability to believe that these neurotic people have enough intelligence to be doctors in the first place. That episode was crazy. The thing that surprised me was that with all the yelling going on between Izzy and the rest of them that no one came to check to see what the hell was going on. I get it that it’s not real life but I found that whole episode profoundly disturbing on so many levels.

There actually was a case like the people skewered together. Two guys in a car accident had a similar injury. They wound up sliding them down the pole to get some room between them, draped the poll with frozen, wet towels to keep the heat and friction of the saw from causing further damage, and had every firefighter, paramedic, doctor, and nurse they could spare hold the bar to keep it from vibrating as the fire department sawed the poll in half. Both wound up surviving.

My mom used to work in a level one trauma center, I would remember sometimes she would come home simply crying, because she had to choose who life to save. The amount of respect I had for her was astonishing, but I also felt really horrible because she would feel as if she was the murderer, which I knew she wasn’t but she just felt that way sometimes.

Sadly, I once heard that when the local hospitals heard about 9/11, everyone scrambled and was really for a huge influx of patients, but when there were a lot fewer cases then expected the hospitals were confused; it was because there weren’t a lot of survivors from 9/11 alive to be treated in the first place 😔

That really puts into perspective like I understood that 9/11 was very traumatic and had a lot of death but that just shows how much they were prepared to probably have doctors overnight and have to file in and file out patients constantly and then I bet there was no relief when they found out they wouldn't be filled with patients cuz that just means so many people died just shows how much of a travesty 911 was

-S11 Ep 21 is pretty medical and the love stories are to a minimum. A massive event happens though so I won’t spoil any more :D -S8 Ep 24 is a really great one too, it shows the doctors having to use SUPER limited resources to treat each other and it would be interesting to see a real doctor react to it (edit: so yeah the first one would be major spoilers but i think pretty much everyone knows who dies :/)

The Ferry Accident two-parter, Walk on Water, has some of the most bonkers medicine in it and I've always wanted a doctor's reaction. There's some Mcdreamy romance in it, but it also shows the doctors as first responders going to the scene of the accident to treat people in the field.

In that scenario it's best to assume their heart stopped than to waste time trying to find a pulse in vain (specially if you're not a health professional and never have felt a femoral pulse).
