I've used this on multiple occasions and would like to share my experience. During my second time using it I stopped the treatment early. I had an event coming up and didn't want a giant scab on my neck. Everything healed as I hoped it would and I thought I was in the clear. Well I stayed in the clear for a couple of months, but the lesion ended up coming back and it grew to the size it was before treatment fast! Needless to say, I had to start the treatment again from the beginning. Take it from me, it's NEVER a good time to start this stuff so don't even look at your calendar. You'll always find a reason to put it off or temptation to stop early. Just start it right away and stick to your doctor's prescribed treatment period until the very end and not a day sooner. It's not worth the risk of having to do it again. Unless of course you enjoy having oozing scabs on your body. That being said, it's excellent technology and very effective when used as prescribed. Remember to wear gloves or wash your hands thoroughly to minimize systematic absorption.

Let me remind you this is topical chemotherapy. Systematic absorption and serious eye injury are both very possible. Consult your doctor.

Used over last 15 to 20 years. At least 4 times on forehead about 3 years apart. Each time all keratoses repaired but some new ones finally appear. Usually treat for about three weeks and re-treat after two months finding a few small areas that have still not cleared. I have had regular surgery over 40 years for BCCs ,two SCCs and one 1b melanoma followed by follow up.

How are you now? I have been using Efudix for 2 weeks on a small spot on my face, I'm not sure if I should only apply it to the raised spot area or the area around it as well, GP is useless. The spot is dark red looking now and feels a bit sore, I suppose this means the Efudix is working, but when do I stop and let healing begin?

I am 89 and had a basal carcinom surgically removed from my forehead about 20 years ago. Several similar lesions appeared recently and have been treated by efudix cream over 4 weeks. There has been some bleeding, irritation and a less than pleasant appearance, but now after about 2 months the skin is clear and normal - a big improvement over the previous treatment.😂.

Very helpful video...Thanks! I'm just ending my second week of a 3-week treatment (+/-). My Dermatologist said my condition was slight and that I was fortunate to address my condition early on. That said, I was amazed to see redness on my skin where I didn't believe I had an issue, which tells me that fluorouracil really does attack both the surface and underlying keratosis. Know that you will see, and feel, various red patches on your face, which appears as if heat blisters from a sunburn have popped. You will be stared at and asked "what happen to your face," which is an opportunity to make up a really cool story, causing the inquiring party to bugger off. On that note, however, using this medicine is 100% worth it. Other than looking like I was beat up by an angry octopus, my skin is on the mend. I'm looking forward to allowing my skin and immune system to do it's thing and repair the blotched areas after treatment. One final thought for anyone who reads this: never fail to use sunscreen. My many years of hiking, kayaking, skiing, sailing, etc., without sunscreen is why I have to come up with cool stories about the way I look during treatment.

I used it this year. I was a lifeguard and am blonde with blue eyes and fair skin. I've had several basal cell cancers removed. I used this cream on one shoulder that had an actinic keratosis (precancerous) and used it on a spot over my eyelid and about one forth of my chest area. The chest area definitely flared up, as did the spot on. my shoulder and over my eyelid. It wasn't nearly as bad as shown in the video though and after 6 weeks once a day, I let it heal for two weeks. So I have a very slight scar that's still a little red over my eye. am very glad I did it and will use it on more of my chest area in the fall so it won't look quite as bad when I wear summer tops. I used makeup concealer when I needed to cover the red spots so no big deal. NOTHING like the surgeries!

I'm currently using this cream for a mild form of skin cancer. After my first few applications of the cream, I started to feel generally quite unwell, I was quite relieved to find this was a common side effect of using too much cream, so i reduced theamount i used. I was also pleased that my dermatologist warned me, that my skin would look worse before it got better. Because I've currently got a nasty scab on my face, a good sign that the cream is working. My scab also is prone to bleeding, but im determined to persevere in order to clear everything up.

I’ve used a one a day application of 5FU and Calcipotriol for 14 days - it became selectively red, only one or two crusty spots. But then the top layer of skin seemed to peal of leaving redness. It seems a bit superficial if I’m honest, and perhaps not that effective.. 🤷‍♂️

You will only see a reaction from unhealthy cells. If you reaction, congrats, you have no cancerous cells in that area.

Living in Florida my entire life, we used to go to the beach all the time, use baby oil and stay out all day. I can’t believe we did that. In 2011, I was recommended to use Efudex. This was my first time and I decided to apply it on my face, neck, and chest. What a mistake! My face started showing redness within 24 hours. After 14 days and having difficulty sleeping, I went to my doctor. He said, STOP. You got a great reaction all over. I still use Efudex for affected areas. This year I decided to do my entire face again since I was getting more cancers. It was nothing like my first. Use in moderation. Next year I will do my neck or chest again.

Wanted to share my experience...After having many aks removed by liquid nitrogen by my dermatologist, he suggested this chemotherapy cream to broadrange treat my skin damaged areas: entire face, upper chest, and back of hands; broken up into sections to be completed 1 at a time. I started with my forehead and temples. Boy was I glad I stopped there because it is horrible. The way this cream brings out the damaged skin even hidden underneath layers of skin is incredible but also a painful treatment. I did also have chemo related side effects: hair loss, appetite loss, fatigue. The lesions caused by treatment does cause intense itching at the healing stage. It is recommended to treat all potential areas that have had aks/crusting pre cancerous scabbing. Hope this helps! Thanks for the video, very helpful for those starting out!

@LAM-tg5no hi! Sorry, I only just saw your question. I hope your treatment went well. I have not gotten to my hands yet... but the areas that I did treat on my face were very similar. You will not have a reaction from your entire skin. I have read articles that have said this cream will only respond to sun damaged skin or skin cells that are replicating quickly, meaning precancerous. I have personally treated for a minimum of 3 weeks depending on response. All of my research indicates that if you do not treat long enough, the ak's will return. And who wants to repeat this process...not me! Personally, I treat the entire area 2 times a day until all red reactive areas have erupted into scabbing lesions. I actually had my face get infected and I needed antibiotics and steroids to calm the reaction down. Keep an eye on your skin and the way you feel. If you have any question or are feeling unwell, like me, I would highly recommend to go in to see the doctor. I had yellowish puss seeping out of my scabs and had a general feeling of unwell, like something was wrong. I'm so glad I went in. The relief was a huge turn around in 24 hours. Unfortunately, my insurance took away my dermatologist in the middle of my treatment, so I had to do my own research and determination on my own. Truly unbelievable, but by the grace of God, I made it through. I should say, if the treatment does need to be repeated, I have heard it is much less aggressive. Thank you for sharing this. I've had Mohs surgery to remove basal cell carcinoma in 2012. Been consistent on sunscreen daily since then. Been having aks removed by cryotherapy every two years on my face. I have been prescribed this cream by a new doctor in March. Putting off using it to November when days and sunlight is at it's minimum at that time of year. The more I watch videos on it and reading people's comments makes me more and more reluctant to start treatment. I will maybe try to find a doctor to treat by cryo as I did before.

Over the past 5 years I've have had a succession of BCCs and SCCs, some of which have required excision, mostly from my face, requiring a skin graft in one extreme case. I was prescribed Efudix by my consultant dermatologist and used it to treat smaller areas of skin traumas. I found that it does work at keeping these lesions under control. I am currently taking a 10mg daily capsule of acitretin as part of a program to see if it will do the same job as Efudix. It requires more supervision by way of regular blood tests and there are side effects. Too much exposure to direct sunlight in a childhood spent in the Middle and Far East in th 1950s has obviously caught up with me in my 70s! I keep on at my grandchildren to do as I say, not as I didn't do, use a sunscreen or sunblock cream.

My Derm recommended a combination of Fluorouracil and Calcipotriene cream. I applied it twice a day for 7 days and watched it work for 7 days. So after 14 days I look like I have a minor sunburn. Worth considering if you dont want to look bad for a long time but everyone is different.

I have been using efudix on and off for 25 years. My original dermatologist called it a "search and destroy" medication. I use it a couple of times a year on the advise of my current dermatologist. It works so fast on my skin that I rarely can tolerate it for more than 10 days at a time. I am starting to bleed by then. The end result is excellent. I try to use it in the cooler months of the year. I have used it on my scalp, nose, ears, cheeks, and hands. If you don't mind looking like Quasimodo for a few weeks, go for it.

DIFFERENT RECOMMENDATION: My dermatologist and other scientific/medical sources recommend against applying cream or sunscreen over the skin during treatment. Consequently, I had my treatment done in the middle of the winter and I wear a wide-brimmed hat, face mask and large sunglasses when outdoors.

Thanks - this was a really excellent and well-organized video. Super informative! I was prescribed fluorouracil 5% (Efudex) for treatment of actinic keratoses all over my scalp. I'm at the end of the third week and my dermatologist has told me to "stop". I've been tolerating it pretty well except the last few days there has been itching and some soreness/sensitivity, like a bad sunburn. My main issue was that my original course of treatment was to be 2 weeks - and the medication really didn't start to "kick in" until day 10. I saw little effect up until then. My doctor then told me, "well, you may have to use it for 3, or 4, or even 6 weeks" which I was pretty unhappy about. So I'm glad that, for me, 3 weeks was sufficient for my head. I didn't have any trouble sleeping, interestingly - I purchased a few cheap cotton knit head caps (for use under cycling helmets - they look a bit like Muslim prayer caps) and put them on after the evening application of Efudex. I slept great, kept the bed linens clean, and the end of the cap came down over my ears and eyes, so it was like ear plugs and a blindfold. Been getting 7-8 hours of undisturbed slumber 🙂 Next step will be 2 weeks of triamcinolone 0.1% ointment to settle down the treated area - and hopefully that will be the end of the story.

Maybe to get their patients to start it right away. For a bad sunburn, you're less likely to check your calendar and look for reasons to postpone the treatment. There's never going to be a good time to start so they probably figure it's best to just get you on it right away and deal with the "you lied to me!" phone call later 😆

I have a small area to treat on my leg and I'm only on day 2 so nothing's happened yet but I'm planning for the worst and hoping for the best so this video is a lifeline. I personally think DR's should be upfront with their patients. I'd rather have it cut out or frozen off because that's one day of treatment ....this is 1-3 months when you factor in any prolonged healing but there's a 45 week wait in my area for referral. The bit I'm going to struggle with is when to stop....not sure if it's ok to carry on if you get blisters or sores which seem quite common....the lack of support from the NHS is quite worrying tbh.

I have started using efudix for the second time in my life. 7 years ago on the left side of my nose. Now on the right side. This is a good prevetative treatment and im glad ive been recommended it by my doctor as im doing a wider area to see if there are any other problem areas but not my whole face. Skin has started to break so there is a problem area and some more are inflamed but my doctor suggested i keep on applying so i can see if any other problem areas occure. Both me and my mom have this problem mine is minor than hers but efudix does help a lot and im thankful for it

Very useful info packed video - thank you very much. This cream worked well for me on a variety of lesions on my face. My biggest problem was using it in the summer months, getting hot and sweaty, and the inevitable itching this can cause. I found it almost impossible to avoid automatically wiping or rubbing the area and transferring it to my eyes, which was quite unpleasant. I never really cured this reaction but found that once it got into my eyes, it was best to wash the whole face with plain soap, give it a couple of hours to settle down and add another light application. wearing a cotton sweatband also helped. I also learned to avoid using at night as I seemed to spread it everywhere. Probably not a textbook solution but it worked for me. I found that freezing seemed to work much better for me and was far less hassle in the long run, but I guess not all lesions are suitable for this method.

Thanks for a most helpful and informative video. I am currently two weeks into a three week course of Efudix, applied morning and evening to treat Actinic Cheilitis aka 'Sailor's Lip', in my case the upper lip. It is in fact the second course, the first was applied about ten weeks ago, if the second course does not work then I will need some other form of treatment. Anyone reading up on the use of Fluorouracil is likely to be wary when using such a potent ointment on the lips which are anyway so sensitive. I was told to expect a robust reaction and, after about a week my lips have become very sore with lesions which are prone to split and bleed. After the first course of treatment it took at least a week before the lips began to heal. Whilst the treatment is not pleasant I would certainly advise anyone prescribed it for Actinic Cheilitis to go ahead. I have a rather puritan streak in my nature and reflect that the problem is pretty much self inflicted and probably reflects years of rash over exposure to UV without sensible precautions. I will be more sensible in the future, and will encourage others to

@Dr O’Donovan So pleased I found your video! I have just been prescribed Efudix 5% cream for a patch of Bowen’s disease on my lower leg. The dermatologist at my local hospital told me to apply it once a day for 6 weeks, using a cotton bud, to the red centre of the lesion. He stressed not to get the Efudix on the surrounding unaffected skin. After the 6 weeks of treatment have finished, I’ve got to wait a further 6 weeks to see the full results. I was told that a further 6 weeks of treatment could be considered if the Bowen’s lesion hadn’t been fully eradicated. Your video has really highlighted what side effects could be experienced - I now feel more comfortable about starting the self treatment. Thank you! ☺️

I have just completed this treatment for the second time. Originally I used the straight Efudix. I couldn't do past 17 days and the pain was pretty bad for weeks. The combination treatment only lasts for 4-7 days and the side effects are not as bad. The Harvard study indicated excellent results and far superior to using straight Efudix.

I have used about 90% of a 40g tube for moderate superficial Basal Carsinoma over the last four years. I have chosen to do areas like my face, arms, back of my hands, the triangle on the upper chest at differant times because of the discomforrt. Mostly because of the early-on insane itch during treatment. Anyway, I have found the recommened "pea-size" is massive overkill. Fluorouracil 5% is powerful stuff. Only a VERY thin covering/coating is needed over the treaded area. I often use . . maybe. . . a B-B size amount for around six square inches for 3-5 sessions 12 hours apart to start the insanity.

I am currently using Fluorouracil Cream, the sun doesn’t like blue eyes and fair skin. Anyway, I am treating a small area on my forehead close to my hairline. I am on day 8 applying it twice a day. I was given a 3 week treatment plan. Absolutely zero problems. I have used this cream before and had no problems. I apply a very small amount, that may be the key to not having a reaction. Wash your hands before and after also. Keep out of reach of children and pets. It’s definitely powerful and will works well.

@@DannaQuintana Actually my skin reacted as it should because I did have some pre cancerous spots. The spots turned red and were slightly itchy. The part of my forehead with no pre cancer had no reaction. That’s normal too. I feel many people are afraid of this cream (I was at first) and I wanted to alleviate those fears if I could. I completed the 3 weeks, saw my Dr. he said I am done. Then said will treat another small part of my face in a few months. Once you stop the cream, your skin heals fast and looks the same as before. Hope that helps.

I'm certain that I have used this before and the only side effect was a mild stinging so I did not have to stop using it nor apply the hydrocortisone cream that was prescribed at the same time.

Then yesterday I went to the pharmacy and asked for a painkiller and I was advised to apply an ointment that soothes a very dry and irritated skin. It seems to help a little. Yes, this a good description of my experience. It does look terrible while you are going through this treatment, but I explain to family and co-workers what is going on so nobody gets scared off thinking I have some horrible disease. Because they know me well, they are understanding. But stringers and acquaintances don't understand so I try to keep everything covered up and if they look uncomfortable, I quickly explain and that often helps.

I'm on day 9 of a 14 day treatment period and this video was very helpful. I'm applying fluorouracil (5%) to my forehead and scalp. Last night was my first horrible one, I couldn't sleep at all because of the pain. At six in the morning I took some allergy medication and finally got to sleep for a few hours. My doctor prescribed hydrocortisone 2.5 % cream to be used AFTER finishishing the two weeks of Efudex. I assume that this is because the hydrocortisone might reduce the effectiveness of the Efudex. I notice that some people are applying hydrocortisone cream to help with the itching and pain while using Efudex. Hmmm?

Just finished 3 weeks of Efudix twice daily . Now day 4 after stoppage , skin peeling , weeping , scaly skin not a good look , I have used this befor with good results at completion , I forgot how painful the crusting , inflammation , oozing , hard to sleep , but I will persist with it as I have committed and I am confident the end result will be worth it . I have taken leave from work for 1 month and think I may need more time , I would be interested in more info about post stoppage care such as aqua type appropriate moisture options ? that help the painful after effects , it is uncomfortable and hard to sleep so post care info is helpful , my doctor did not really mention anything about post care options to make more comfortable , and I was reluctant to use moisturiser with out confirmation. But it appears there are options which I think would make a huge difference ! Thank you

Using for Bowens disease spots on arms...followed the prescribed 2 weeks on 2 weeks off course for 5 weeks and have had to stop following blistering over a far greater area and a lot of weeping this has now spread to my head neck and face although no cream was applied. I have been unable to sleep due to intense itching and stinging together with continuous weeping. Am going to let it heal up and see what results are and see whats advised at follow up consultation in 3 weeks time. The videos have been really useful as I don,t believe I was sufficiently briefed regarding usage and potential side effects by the NHS dermatologist (Light House Clinic IOW) And now seem to be having an allergic reaction beyond treated area.

Would there be any reason not to use it on my nose, given the sun damage I've collected? I'm a week in and so far, it's pretty red and getting sore. I'm on a two to three week course, so hopefully that will be it.

I'm sorry the doc didn't answer your question. I wanted to give you advice from my own situation. Yes, absolutely you will want to treat your nose and other areas that have had excessive sun exposure. After having many aks removed by liquid nitrogen by my dermatologist, he suggested this chemotherapy cream to broadrange treat my skin damaged areas: entire face, upper chest, and back of hands; broken up into sections to be completed 1 at a time. I started with my forehead and temples. Boy was I glad I stopped there because it is horrible. The way this cream brings out the damaged skin even hidden underneath layers of skin is incredible but also a painful treatment. I did also have chemo related side effects: hair loss, appetite loss, fatigue. The lesions caused by treatment does cause intense itching at the healing stage. It is recommended to treat all potential areas that have had aks/crusting pre cancerous scabbing. Hope this helps you! God bless🙏✝️

Thank you Dr Odonovan. I have used this for precancerous spots on my face, ears, arms and scalp. Yea, I'm Irish too!. My treatments with this was twice a day for one day a week for 15 weeks. This keeps redness to a minimum (or nothing) but eliminating the spots. Some spots take awhile longer but still effective. My older brother was treated with this same medicine but not so gradual. He used the "all on" attack treatment which turned his head and face into a pizza for a few weeks! But it worked too. I'm younger and would prefer not to go through that. I watched gradual elimination with no inflammation of the spots. Thank you for the video. This medicine is a miracle for us white guys/gals w/ too little melanoma!

The problem I had was tiny blisters on my forehead that I've had for a couple of years that weren't clearing up and have recently been getting worse. I went to my doctor, she took a photo of my forehead and a week later I got a text saying they'd prescribed this. So strangely, I don't know what I've been prescribed it for. I'm guessing it's cancerous or pre-cancerous sun damage? (I have abused the sun during my life). Having just finished the course, once a day for four weeks (I did 25 days), I now have a very red, sore forehead. It got gradually worse over the 25 days. As I understand it (from reading up on the internet) this is expected, shows the cream is working and it's very much worth going through this. I'll update this comment in a month or so.

I just finished my 2 week course today. It's wasn't really bad until the 9th day. I have trouble opening my mouth all the way, I feel like I have a severe sunburn and the face itches really bad. Some times the pain is so bad that I can feel my face throb. Worse part is applying the cream in the condition. Now I understand the term "rubbing salt in the wound". If I go out i where a mask due to me looking the way I do. I'm sure it will be worth it in the long run, but this is definitely difficult.

My experience exactly. But BTW no need to wear a mask when you go out. I’m 52 and found that honestly, nobody cares. Seriously. No one cares what you look like. It is rather obvious that there’s something going on and so no one has looked at me funny. Not in the market, not at the veterinarian, not at restaurants or the coffee shop. Masks are itchy and hot. Just be brave because you’re the only one who cares what you look like. I look like a leper. Like my face is bright red and covered with corn flakes.

Best only in winter, no sun here to speak of, and cold. About to do one ear and opposite cheek now. November… scabs come off and looks great , seen no scarring even when „over treated „ by accident.

Thank you very informative video. I am at the two week mark of a three week treatment (twice a day). For face and forehead. My nose (and 5cm's either side) are red and crusty, and wondering if i can stop treating that area now? My dermatologist, and instructions that came with the cream, did/do not really cover this scenario. I get the sense from this video i can now stop applying to that area, but might call my dermatologist Monday and check.

Efudix works really quickly for me. I apply a field treatment once a day and within the first week the area is becoming red and sore as the visible and non-visible damage is attacked. By the end of the second week it's very noticeable; very red and blotchy, itchy and starting to erupt in the surface. By the end of the third week some areas are very dark, turning almost black. The blotches are cracking on the skin surface and some will be open. I avoid those open sores and the instructions are very clear that you MUST NOT allow it to enter the blood stream. Great care must be taken around the eyes, nose and mouth. By the end of the fourth week the whole area will be too "open" to continue use and by the end of the fifth week it is healing fast. Sometimes the whole process completes within four weeks. I always wear sun block and a hat when outside, but even then the sun damage will occasionally reappear. Nonetheless Efudix is a miracle cream.

You need to show examples of how white or translucent the cream should look on the skin after applying. I want to know if I am applying too thin or too thickly. Basically, should the white cream on the skin be opaquely white, show through a little bit of the redness or be so thin as to be a mild whitenening of the skin. Currently I am doing opaquely white or almost opaque and am worried if this is too much or just right.

Thank for this information . . . this has helped a lot with understanding this treatment [on the side of my face -temple area] as my doctor did not explain anything whatsoever about the treatment except the actual '3 week course of treatment' and that it will sting a bit . . . . . but due do this (negligent)lack of actual treatment info & symptoms thereof which is vital to know from my doctor as to what to expect, I knew nothing what-so-ever with the result is that well into the treatment and all the symptoms of the treatment that is quite horrendous and looks horrendous and the pain, the discomfort and and etc etc left me in a panic and even hopeless in despair . . . . . but now I have done some research and watched this very informative video(including the link to this case-journey) it has been great for helping me now to understand this treatment and I can now "relax" and let the treatment take its' course with all it discomforts that it is meant to take place . . . so thank you!

Thanks for the video, really helpful but I have a question about how much to apply. I have a small patch of damaged skin (solar keratosis) on my nose and have been applying Efudix for two weeks every evening with very little visible effect so far. I was told to apply sparingly and as the patch is only about 1cm2 I have been using much less than a pea sized amount - more like a large grain of rice sized blob. If there’s a visible layer of cream on my nose after I’ve applied it and gently spread it around, I generally dab off any visible excess so I don’t risk it spreading and getting it in my eyes. I’m wondering if I’m perhaps not applying enough or if I should apply it twice a day. I’m obviously also wondering if it’s not working or whether it’s normal to not have much effect after two weeks.

​Please know that derma-skin on Ears, lips and nose are much thinner than other areas. My Own experience was using Too Much; ie; believing more applications and thickness of cream was better! Which is Not true. Being more conservative rather than liberal would prevent the massive reaction that I had! Today, we have more data, more doctors speaking about this and more patients having been thru the protocol..Please follow your own doctor's caution and recommends. My own derma-skin was damaged more than probably 90% of Persons who have done this. Best of success Michael

I’m not sure you will see this since it’s been 5 months. I was diagnosed with Bowen’s Disease last month. I also put Efudex on my arm for 30 days. It never reacted. My dermatologist said we could wait a few months and see what it does. He could cut it out; but it’s a large area. Could you tell me what was your course of action after Efudex didn’t react. I’m not sure what to do. Thank you.

I have had to have two treatments. One on my arms and chest and another time on the front of my thighs. The first treatment I applied the treatment twice a day. When I got toward the end of my treatment, I was in agony. Dry skin, itch and pain. It's more than a bad sunburn that I was told would happen. The results were new, smooth skin. I do have some scaring on the back of my hands but I think that's because I overlapped treatment there. The second treatment was on my thighs. I was told to apply the cream once a day instead of two. This treatment lasted/as long as the first but the pain was not near as painful. However, the pink spots have returned and I will be begin treatment again later this month. Oh how I wish I would have listened to my mother.

Followed liquid nitrogen treatment with 14 day prescribed treatment 2 x day RX on lower lip. This erupted into major blistering and now lips are throwing down pus covers about 6 times a day! Quit applications until this heals up at about day 8. No sleep due to painful mouth! Went back thinking I was getting a secondary infection to dermatologist who okayed stopage and now hoping I will get this to heal up bleeding cracking areas. Vaseline helps but isn't allowing fast drying of sores. Zinc oxide will be my new best friend hereafter! (And I always apply sufficient sunscreen when outdoors- have for decades.)
