Beyond the Scalpel with Dr. Paull

Beyond the Scalpel with Dr. Paull

Күн бұрын

A few years ago I decided to leave the traditional insurance-dominant healthcare system and never looked back. My practice, Easy Orthopedics, is a cash-based practice. We don’t deal with insurance companies because we don’t want them telling us what to do. And by not taking insurance, we can see less people and therefore can spend a lot more time with each one of them. This allows us to treat the person and listen to them, rather than just treating body parts and hurling prescriptions. This is called relationship based medicine.
Many of our patients do not have health insurance or an insurance plan with a very high deductible. Paying for medical care by cash can make seeing a doctor very difficult. Many offices do not want to take patients that can pay only in cash, and as a patient it can be very frustrating to not know how much the doctor is going to charge you! By taking out the middlemen, insurance providers and healthcare administration, we are able to provide transparent upfront pricing. Having a cash-based practice means we can offer better care, better prices and never send patients any surprise medical bills.

I have one of those sucky ACA plans you mention. These people who work in the doctors office maybe see people who pay cash as a threat to their jobs. They tell me they don't take my insurance. I ask "How much would it cost if I pay cash?" They usually get this confused look say the dont know have to check with someone. I asked them how much for an x-ray at a radiology place. I wasted 30 minutes waiting her to hear back from someone with an answer.

So many doctors doing this. I don't have insurance but have been getting good care with doctors like this. I do have catastrophic insurance, however.

I hope and pray we see doctors bucking the system and opening up private practices again. It’s probably more affordable for doctors at this point.

They need to allow people tonhave catastrophic insurance and not age restrict it. Im a nurse and have a healthshare ministry just to keep emergency hospital coverage. I shop for docs who give me a cash discount

I am taking the dip into the cash only medical care and feel my way through. For the last 5 years I have thrown money away for nothing. Hoping this is a better option.

It's ironic that the American healthcare debate is always "capitalism" vs "socialism" when THIS is truly capitalistic healthcare. Insurance is still socialized healthcare.

Great video thanks for sharing! Is it a big Finacial hit for you as a physician taking this rout? How can we convince more physician to drop these insurance companies that rob doctors and patients day in and day out

It is at first, but it ends up being more profitable and sustainable I believe in the long run. Physicians are very risk averse, that's why you don't see many leaving the insurance world, even though they are miserable.

@@vincentkingsdale8334 I know a lot of direct care PT, it works. That being said, don't think you can see Medicare as a PT because I don't think you can opt out like a physician can

What's the best way to go about finding a doctor that is cash only? And what's some advice you have for someone paying cash to negotiate better pricing? Maybe the negotiations wouldn't be as necessary with a cash only doctor but with doctors that take insurances it feels like a sticker shock every time I get the bill.

Try jointhewedge.com/find-a-practice/ . That will help you find a cash practice, otherwise you need negotiate with whoever controls the billing which usually isn't the doctor

Just ask your current physician, I did. And I get a good discount for cash. Was in the hospital and got 75% off the bill b/c I was uninsured. Plus got on a payment plan. They are willing and happy to work with uninsured people. They know that they won't have to wait forever for the insurance company to pay them plus don't have to hire people to take care of the claims.

This is great. How this may work for surgical practice though?

In the US you need transparent cash pricing. For example, I recently did a distal radius all in cost $6,500. The hospital quoted him $27,000 + anesthesia + facility fee, which would likely come to 50K+. There are people and companies looking for transparent pricing.

It’s amazing! However I don’t quite understand how the hospital fee would work. Let’s say your pacient needs ICU after a big procedure, how would that work? I totally can see it working on office based specialties, I am curious about the specifics about surgical/procedure based specialties. I would love to see a video about it.

@@Igorvaz It can really only work for stable patients in the outpatient setting. Once the hospital gets involved it's ruined.

It would be the only way insurance would be beneficial and a different category altogether. Insurance company is another ways for the government to control and clean you out. Why not educate people on health and stop companies from selling poisonous foods full of sugars, fat, chemicals etc. America has become an obese country! It’s their cash cow. Make them sick, they get rich. On top of that fill them up with chemicals. A pill popping country! Great combination for the insurance establishment.

Does that mean you have to loose your existing insurance panels? Also how do patients find you? Any referral sources?
@orthopedics
@orthopedics 2 жыл бұрын
I started off not taking insurance so there were no panels, but some docs will convert their practice by removing one insurance at a time. Patients find me through the internet, word of mouth, and local referral sources such as chiros or DPC docs

Given at cash wholesale prices. They can even be less than a copay sometimes. This covers most drugs, more expensive name brand drugs are more difficult.
