Demodex mites are one of the most challenging problems that ophthalmologists and optometrists face in the routine care of their patients. These mites are microscopic parasites! It uses a seven-clawed organ (a “palpus”) to grab hold of cells. Then it feasts on the cells at the follicle, sucking out their innards with a retractable needle in the middle of a round mouth. How gross is that? What can you do about this parasite feasting on your follicles? Let’s get into it now . . .

Does ivermectin cream kill Demodex mites? Ivermectin shows broad-spectrum anti-parasitic activity. It kills the Demodex mites that reside in the pilosebaceous units of patients with papulopustular rosacea. Ivermectin also has anti-inflammatory effects, it decreases cellular and humoral immune responses. So, yes!
How often can you use ivermectin cream? How many times a day should I use ivermectin cream? How long can you use ivermectin cream? Ivermectin cream should be applied once daily for up to 4 months. If there is no improvement after 3 months, the treatment should be discontinued.
How do I use ivermectin cream? The treatment course may be repeated. A pea‑size amount of ivermectin cream can be applied to each of the 5 areas of the face: forehead, chin, nose, and each cheek. The cream should be spread as a thin layer across the entire face, avoiding the eyes, lips and mucosa. Ivermectin cream should only be applied to the face and hands should be washed after application.
Should ivermectin cream be applied in the morning or night? You should use ivermectin cream once a day at about the same time each day for the most consistent results. Some people choose to make ivermectin cream a part of their nighttime skin care routine, but it's not necessary to use it at bedtime.

Ivermectin topical works on Demodex. It did for me. Oral would work, but I didn't need it after the topical (daily for about 8 weeks, with a follow up of five weeks half a year later. The ivermectin took about three weeks to really start showing results, and the die off of the demodex caused temporary inflammation of the skin, and itched, and shed skin cells (about week four), then my face cleared up, healed and no more problem. Cool beans. A decade long problem cured. The die off is what irritates the skin, definitely. Not the Ivermectin. I did try permethrin, which helped and worked from day one, but it only killed off adults that exited the follicles at night for mating. The Ivermectin works better, but is slower, and affects all phases of the Demodex life cycle, ultimately killing them off.
