Wow this was super helpful. I am completely asymptomatic but the tests were ridiculously expensive and they prescribed also expensive antibiotics for it. Seems to me the risks of taking antibiotics and ruining my immune system are far higher than “treating” the mycoplasma hominis which exists in the body anyways.

@ariannateodora5469 not what I mean. If you have symptoms, yes you should be tested, but your symptoms should also contribute to the management and treatment plan the doctor puts together. If no success then the investigations can be more intensive.

This is a big problem. I tested positive for mycoplasma G and was treated with Azithromycin, I still showed positive after that treatment and we suspected that the bacteria was resistant so I was treated with moxifloxacin. That treatment didn't work either. Mycoplasma H is not affected by Macrolides so therefore I still had my infection and lots of symptoms that negatively affected my life. Therefore, it is important to test specifically for mycoplasma h/g etc. to avoid taking a lot of antibiotics unnecessarily and cure the infection faster.

I'd disagree. Your doctor knew you had MG, so in the UK, We treat with doxycycline then azithromycin. If symptoms still present then treat with moxifloxacin. However it all depends on what your symptoms are. Doctors should treat symptoms and patients not test results. Kind regards Dr Bret Palmer

yes I'm from Sweden and they don't treat with Doxycycline here in the first place (which they should). If I had initially received pre-treatment with Doxycycline and after Azithromycin, my symptoms would have disappeared a long time ago. Here specifically in Sweden, knowledge about Mycoplasma in particular is incredibly poor. Some healthcare providers don't even know that both women and men can have the bacteria in their lower regions but only in their lungs. Due to the doctors' lack of knowledge here, I have suffered. So that's right, if I had been on doxycycline first I would probably have been better. Now the doctors have discussed the drug Pristinamycin... which I do not recommend..

See if you can get 2 weeks of doxycycline, this can be recommended in the European guidelines, especially if you also have pain in the testicles? While sexual health or genitourinary medicine isn't a specialty, there are doctors who do specialise in sexual health... their usually based in the larger cities. Let me know how you get on. The PCR test shows not only Mycoplasma Genitalium but also other types of Mycoplasma that can cause severe symptoms and consequences for a woman. It is important to specifically test for which type of Mycoplasma you have in order to cure faster and without lots of different antibiotics. Even if a person does not have symptoms, they should still be tested for mycoplasma H because if a person who has Mycoplasma H does not have symptoms, they can still infect someone else who then gets serious symptoms instead.

Thank you for your question. The evidence shows that Mycoplasma genitalium cam be a serious infection, but not Mycoplasma H. However if you have symptoms then you should be treated accordingly and treatment for, for example, PID, then many antibiotics will also clear up Mycoplasma H. I hope you get better quickly. No question is silly. Bacteria can change your body odour. Best to wash with an antibacterial emollient. I'm not aware of a particular Bacteria causing a particular change in body odour.

Ah, okay. No sex until the doxycycline course is finished. Partner should also be treated otherwise you'll get it back again. In the UK we don't treat this infection. Take care.

Hi Doctor. I was Positive for Ureaplasma Urealyticum and negative to other STD's. My Doctor prescribed me doxy and polyderm 3 for 7 days and extended both for another 7 days. The problem is my urithiris isn't go away, the redness on my glans still there with irritation and I still feel inflammation or soreness on my meatus. Is it possible Doctor that I have balanitis or Penile yeast? By the way Doc I also have pain in my bulb of the penis/ pain in my penis except on my testicles.

Hello doctor, I did a ETS PCR pannel (male, urine sample) which included other mycoplasmas, gonorrea and clamydia. Mycoplasma Hominis came back positive, everything else was negative. I'm having symptoms and I would like to know if this bacteria can "activate" and cause infections out of nowhere or how does it work?

Hominis can be passed sexually but maybe past by other means. It isn't considered to be an STI and I the UK we don't regard it as one and don't even test for it. If no symptoms then nothing to treat if you was in the UK. Your doctor may have a different opinion.

If you have a sexual health clinic go there. If you have PID symptoms then you will need different therapy.

I'm tested positive for Mycoplasma Genitalium a few days ago, but I've been having symptoms on and off for more than 3 months (I have non-gonococcal, non-specific persistant urethritis). I was taking doxycycline twice and moxyfloxacine once. The symptoms always re-appear after 7-10 days of finishing the course. Last I took moxyfloxacine and had a control urethral swab 2 weeks after finishing therapy and I tested positive for Mgen. As of yesterday the yellowish discharge is back again. It is possible I got re-infected as of the last course of antibiotics, but that would most likely be from oral sex. Would you recommend taking a 2-step therapy at this time? Do you think doxy + moxy combo would help even though I already took those separately from one another? Would you recommend an alternative? I find the advice you give here very helpful, thank you.

Hi there, I follow the UK guidelines, a test of cure, should be done 4 weeks after treatment as you get high false positives. Either way you have a discharge and a urethritis regardless of cause. I think treatment with doxycycline then Azithromycin would be best (if susceptible), if no improvement or still positive then moxifloxacin 400mg once a day for 14 days... no gym and stop if you get tendon pain. Sexual partners do need to get tested and treated otherwise MGen will be returned. I hope this helps and sorry for the late reply. Take care

@@DrBretPalmer Thank you very much for the reply, doctor. My doctor prescribed me doxycycline + moxifloxacine therapy as there is no resistance testing available. I'm on my 4th day of therapy, the discharge became thinner, and has almost disappeared by now. I was also advised to get re-tested a month after finishing therapy. Hopefully, the symptoms will not reappear. Can you please advise why should I avoid going to the gym now?

@nemanjamitrovic2392 sure, moxifloxacin can cause tendinitis and in rare cases tendon rupture. Best do swimming instead.

Hello Doctor. I was positive for ureaplasma urealyticum. Doxy didn't help. I have only had unprotected sex a few times, but the women i am with will always get some type of infection soon after, and they have to use a cream. I test a lot and always have negative everything. Could the ureaplasma be the issue, and how can I get rid of it if doxy won't work. Thank you for a response.

Depends on the infection they get. If it's thrush then no, nothing to do with ureaplasma. Don't wash with soap use an emollient. Reason in the videos. Can leave it as can be cleared natural in many cases. Drink plenty of water, tea, fluids etc.

It is difficult when you have an infection causing discharge which won't shift. You should be re-investigated. You doctor may consider doxycycline for 2 weeks then azithromycin or may choose minocycline 100mg twice a day. Good luck and hope it improves.

Is the black menstruation at the end of the period? Are you in pain? If black blood at the end of menstruation and you are not in pain then should be okay. Have a full sexual health screen including Syphilis, HIV, gonorrhoea and Chlamydia. If all negative then that's great. The two bacteria you have we don't test for as you can pick these bacteria up from your environment. So really depends how you are actually feeling.

No need to worry. If you have symptoms then get treated. If everything is normal then have a standard sexual health screen. If not producing children after a year then get a fertility test. Please speak to your doctor if you have other concerns.

Thanks doctor. I took a complete STI test as a normal follow up. MH was positive and all the others not detected. After a week of receiving the results I’ve start experiencing very mild symptoms (like a little burn when going to pee) but nothing that bad or painful. I took doxy for a week and azithromycin for 3 more days. Just saw your video after completing the treatment (otherwise I wouldn’t have done it because symptoms were very mild!). 2 weeks have passed since treatment and I can still feel a very small burn still but again nothing painful. If I wouldn’t have taken the test not sure if I could be able to tell that I’m experiencing symptoms…maybe I was more paranoid when the test showed a positive result and start reading so many negative things on the internet…

I have tested positive for Ureaplasma urealyticum no symptoms. I’m happy to leave it, except that my partner’s mother has MS and he has some reason to believe he may be autoimmune compromised. Is this a risk?

Dr. Burt, is it possible that the bacteria ureaplasma urealyticum and parvum are pathogenic and can lead to death? What is the risk factor if they are pathogenic? Then you said that it may go away on its own within 3 to 6 months. Do you mean after symptoms appear? Is it possible to treat it when symptoms appear without antibiotics, just by raising immunity? Thank you for the question, but you have it the wrong way round. If you have symptoms then you need to be treated regardless of what the tests show. It could be all kinds of pathogens, but could also be something you picked up by another route and the only thing that's showing is a ureaplasma type infection. So get treatment if you have obvious symptoms.

Any bacteria in the wrong place e.g. the brain, can cause death. Bacteria in the genitals usually are harmless. Adhesions are the body's reaction to trauma, e.g. surgery, radiation, infection, inflammation, autoimmune etc, so yes bacteria can cause it but there are many other causes. It would be completely incorrect to assume is a person has adhesions to then say, that's caused by ureaplasma type organism. I hope this helps.

I think this is a serious issue causing serious health problems . That’s been my experience. I appreciate your medical opinion /advice . This video helped a lot . Why does it cause me so many problems . It almost kills me and the pain is horrific ! I think it should be part of standard testing and treated because people passing it to others that fall ill to it , is an issue , and a risk to fertility and life long health issues . I was 40 when it started . And any exposure to it gives me major problems . Could menopause be the reason why it is causing me issues? I had to have a hysterectomy and was diagnosed with interstitial cystitis, even after a negative result. But anytime I’ve tried to be intimate I get exposed. And it makes me bedridden in pain and illness and discomfort, it is one of the worst things in the world and the most pain in the world. I would not wish it on my worst enemy.

From where I’m standing, it feels like the urea plasma is what caused me to have to have the hysterectomy and caused the bladder to be very sensitive, resulting in interstitial cystitis, whether or not now my test comes clear from ureaplasma . I think it can cause major issues .

I’m a little confused on the guidance here. I’ve had urethritis for years and tested positive (privately) for Ureaplasma parvum. Nhs doctor told me at the time not to bother treating it as it’s normal. However I’m now pregnant, still symptomatic and still being told not to bother treating, even though guidance seems to suggest Ureaplasma can cause stillbirths and miscarriages. The nhs website itself even says Ureaplasma can cause this so doctors guidance does not seem to make any sense? What’s your opinion?

The big difference between the guidance and yourself is that you have symptoms. Regardless of the tests, if you have symptoms, then the symptoms should be treated. Usually, ureaplasma is treated with standard antibiotics. You are pregnant so you may be given azithromycin, does very much depend on your symptoms. For people who don't have symptoms, then they shouldn't even test for it. I hope that makes a bit more sense. You are entitled to get a second opinion at a different sexual health clinic. Take care.

Surely if the infection can cause pregnancy complications it’s irrelevant if it’s symptomatic, pregnant women should be testing and treated regardless?

@Jenniferstone242 hi there Jennifer, one study may show this relationship but other studies haven't. There are many bacteria in the body and most don't cause any disease, some will only cause problems if there is too much of them. We need bacteria in our bodies in order to live and too many antibiotics is not good for us. So it is not as simple as this was found... get rid of it. The cause maybe another bacteria but the only organism being detected was a very common one usually found in the genital tract. Either way, if you have symptoms then your symptoms should be reviewed and treated accordingly. I hope this is helpful. Kind regards

I have mycoplasma hominis and ureaplasma uriliticum i took two packs of doxycycline and azomithricyn and diflucan and flagyl.i had symptoms like yellow vagina infections and lower belly and back pain.even i did an operation i had asherman,that was from 2 years ago.so after taking the treatment i did the STD's Pcr and all was negative.and now i got married the sypmtoms came back.so what do u advice me to do??thank you doctor

Your doctor can do tests but should treat the symptoms and you should be examined to exclude PID.

@tatahg934 if your partner was not treated then it will return. Also can return as it is suspected that there is a non sexual route to obtaining some of these infections.

Hi Dr Bret. Iam having recurrent bacterial vaginosis. Due to which I went into premature labor. Unfortunately, my baby couldn’t survive. They are treating me for metronidazole everytime. This time I went to the gp. He didn’t check me and advised me metronidazole and doxycycline. I went to the sexual clinic to get myself checked. They said that you just have intermediate flora, no bv and no sti. From the month , Iam not on any antibiotics. I was wondering that can bv and ureaplasma block the Fallopian tubes?

Hi there, Very sorry to hear about your baby, very difficult time for you. To answer your question BV doesn't, to my knowledge, doesnt block your fallopian tubes. Currently, Ureaplasma is considered to be harmless. However if you have symptoms then these should be investigated and treated accordingly. I wish you well. Kind regards Dr Bret Palmer

One last question dr, I have pain on one side of spine and pain for a second in my lower pelvis which is gone now. It lasted for week. Could it be pid? The pain was not all the day. Sometimes a day

There's a reason why the NHS doesn't test for ureaplasma, it's not been proven to cause any problems, I'd advise against it. With regards to the pain. Unlike to be PID. PID is usually diagnosed with an internal examination. If the pain is increased when the cervix is moved and the cervix has pus under the microscope, then you should be treated for PID, regardless of any organisms found on testing. Hello doctor, I had a risky sexual relationship (without a condom) with a person who was diagnosed with mycoplasma hominis (MH). 10 days after sexual intercourse I started to feel testicular pain. I have no other symptoms, no discharge, no fever, the pain is sometimes sharp and sometimes "dull". I went to a medical center and they gave me doxycycline 100 mg twice a day for 14 days. The first week the pain persisted, the second week the pain became less evident. Two days after completing my treatment with doxycycline, the testicular pain returned so they gave me 1 g of azithromycin as a single dose. The two days after azithromycin I had no testicular pain, but on the third day the pain returned. What do you recommend and why is the antibiotic not working?

What about ureplasma with symptoms that could be caused by something else (itching & burning during intercourse / just stopped periods -- so could be caused by meno)? Would you treat these with antibiotics?

Sorry for late reply, I run the YT channel after work. Ureaplasma generally doesn't cause any problems. However some private clinics test for it and then treat it as a sexual infection. In the UK we don't test for it. Either way Ureaplasma can be treated, usually by standard antibiotics like Doxycycline. So if you have no symptoms I wouldn't be concerned. If you have symptoms then you should be treated according to your symptoms e.g. PID symptoms would usually get treated with standard antibiotics which would also treat the Ureaplasma. @@DrBretPalmer thank you SO much. It is so hard getting answers. I do have symptoms. But they could be explained by another issue. Specifically: labia itching, pain during & after intercourse, very heavy periods (ended 3 months ago bc I'm going into meno). Itching on and off for years. Pain during intercourse started 3 months ago. Just last week, I started having pain just above my hip bones on both sides, intermittent. Not severe. I had a *horrible* reaction to very strong antibiotics 1.5 years ago (clindamycin, 11 days, amoxicillin, 7 days, and metronidazole, 4 days). My previously very healthy gastro system was seriously almost destroyed. I know that I should have recovered but I never did. Then I caught covid and things got worse. I have been dealing with alternating reflux / constipation for the last year. It has changed many things about my life, can't eat most foods, have to sleep sitting up, can't sleep often due to side effects from constipation meds, etc. etc. I'm starting to get those under control *finally* after trying just about everything in the world. So having me take antibiotics for a "maybe" diagnosis is like telling me to go get into a car accident because it might help. That's even worse given that many people don't respond to the first round of antibiotics (or ever). But even one would be completely devastating to me, I am pretty sure. Seriously. I also have lower ab bloating which I assumed was gastro related -- but maybe it relates to this infection?? Is that possible?? My thought is that these symptoms could all be caused by meno. I just last week started premarin. I'm hoping maybe increasing the pH will help the itching and pain during intercourse. Not sure about the bloating and new pain above my hips. Do you ever see people get better from these symptoms (who have ureaplasma) without antibiotics? Or is the research just not there?

Yeap it does say that. But a symptomatic person should have their symptoms treated, as could be caused by another bacteria. So the end result is, if symptoms are treated the chances are so would any ureaplasma as well. I hope this makes sense. Kind regards Dr Bret Palmer

This infection can pass naturally in 6 months for many but not all. Keep hydrated. If you symptoms then you may need antibiotics to get rid of the symptoms.

Forget it is MH, just work on the urethritis. How bad is it, out of 10? Does it come or go or does it stay permanently? What antibiotics have you tried, have you got better after the antibiotics? Did your partners get treated as well?

Yes husband treated. Doxy twice then i started losing vision from intracranial pressure, azithromycin, clindamycin and allergic to fluroquolones and have mtfhr and cannot take those and would never take those even if i could my sister is paralyzed from cipro. Guess ill die from it or live with it

I'd ignore it, if symptom free. In the UK we don't treated as its a common bacteria, unlike mycoplasma genitalium which can cause a few problems in people. Take care.

Hi Nick, Good question, I'd say because many people have it so lots of positive tests, therefore further treatment, more money, and many people will get it again so more treatment. In the UK NHS, we don't test for it in sexual health clinics.

Depends on your symptoms. If you have symptoms and provided no allergies, can have doxycycline 100mg twice a day for two weeks if male. If female then can have the same treatment if no pregnancy risk. I hope this helps.

@tomitomi8538 not necessarily from you but it doesn't matter. I'm assuming you and wife have symptoms? Hopefully this treatment will be effective and then retest 1 month after treatment. Ideally no sex in the mean time.

Depends. Don't do contact sports or sports that effect the testicles e.g. running. Allow them to rest, also wear supportive underwear. Also, did they take sample to see if they can identify and ideally grow the bacteria? Otherwise you may have to have a different type of antibiotic, e.g. minocycline or co-trimoxazole.

@@DrBretPalmer No, they didn't take any samples to identify the bacteria. The doctor told me to wait 4 weeks after the last dose of antibiotics to take the urine PCR test that identifies more than 11 bactericidal agents. The pain has decreased significantly, but it is still persistent. On the pain scale I think I am always between 0.5 and 2 out of 10, and I am very sensitive to touch, but again, I have no other symptoms. What is the recommendation in any case?

@RomanAmor-b3x your doctor is right. Sometimes best to wait. Cut out coffee and alcohol from your diet. Eat more fruit and veg. Some people get a great response drinking green tea. Retract foreskin regularly and wah with water only, do not use soap.
