600 dB is not a 4 times louder. Decibels are logaritmic, so if train horn with 120 dB has to be 4 times louder, then it will have only aprox. 127 dB.

It could also help people who aren't able to easily get in and out or move around outside their vehicle. A lot of these cheap direct-to-consumer marketed products that don't seem to be solving a real problem are intended for use by people with disabilities, but they can't advertise them as medical devices without falling under the regulations of the FDA. It's much easier to just create a black and white scene of an old guy doing a slapstick routine and the people they're trying to reach will understand that it will be helpful for them.

I have one of those sunshade umbrella things from Amazon. It's works pretty well! 2 things to keep in mind. 1 - yea it needs to fit your front window and 2 be careful with the handle/shaft. If the handle is resting on any part of your dashboard it will leave a mark/indentation.
