If you're taking Vitamin D it's important to also take Vitamin K2 (MK7). Seems that metabolic disease drives aberrant lipid profile as well as harms the endothelium. When I cleaned up my diet about 12 (Plant based w/fish) years ago and started daily walking, my lipids plummeted (HDL from 30s to high 40s). Especially triglycerides which dropped to about a third, from about 200 to 70s. My hsCRP stays low (never more than 0.3) and HbA1c stay in 5.9 to 5.2% range. I do pay attention to ApoB though. Dr. Brewer, I’m 48, and you look about my height and frame, although I’m sure I am heavier, though I do not look fat. What is your height and weight? BMI calculators are not accurate. I have two adult sons. The three of us are all about the same height, frame, and weight. Yet, I am definitely “skinny fat”, while my sons have very good diets and work out at the gym religiously. They have more lean muscle, I have a higher body fat percentage (dad bod/weekend warrior type). Yet, our BMI calculations show us all as overweight. According to BMI calculators, I have to get down to 184 pounds to be at 24.9 BMI, which is the first number considered “healthy BMI” at my height of 6’1”. Yet, when I get around that weight, people start to question if I have cancer, and going through chemo. One co-worker once told me I look like an aids patient at that weight. Currently, however, I weigh about 197lbs.

The fact that heart disease remains the top killer does not mean lipid lowering has failed. That’s false logic, like saying I reduced my white bread intake and nothing improved, therefore low carb failed. The power of lipid lowering happens when you achieve physiologic levels, like an apo B of 40 or 50 mg/dL, before your plaque burden is large. At that level, it’s pretty hard for plaque to grow, even in the context of metabolic disease. When such levels are achieved in clinical trials, MACE reduction is significant. In theory, if it were implemented across the population-before developing a large plaque burden-heart disease would probably fall off the list of top 10 killers. Of course there’s only one way to prove it, but all the data points in the same direction: the lower your apo B, the harder it is to grow plaque. It’s also clear that improving metabolic health will slow atherogenesis, as you so accurately articulate. Getting people to “get metabolically healthy” is definitely a worthy goal. But it’s no small task, especially on a population level. In way too many patients, it’s just not going to happen. Furthermore, it doesn’t always stop heart attacks. I have plenty of metabolically healthy patients who’ve had events and keep growing plaque as long as their LDL is permissive. This is why Attia and almost the entire medical community is so focused on lipid-lowering: because it’s achievable on a population level, and because it works.

Not the solution, but something to consider.... diet first tho. Pycnogenol® and Centella asiatica to prevent asymptomatic atherosclerosis progression in clinical events

No matter what is cured something is going to be the leading cause of death. heart attack is at least fast in most cases. years of die from cancer is not better.

65+ years of try to target LDL. No luck, LDL paradox and so one, so one...

LDL doesn't mean sht, especially if you are older. Tri's divided by HDL is the most important ratio. ApoB is a good blood marker too. Have studied this intensely for 15 years. Inflammation is the source problem.
