You are a PHENOMENAL dental artist. I wish i could go to you! I had the exact same cavity in the same tooth! My substitute dentist didn't finish the root canal as planned and gave me the flimsiest temp filling, esp for the size. My entire tooth fractured in half two days later despite me not even eating on that side. I had to have it pulled bc I couldn't afford an implant. It was a terrible experience.

Man, I am right there with you. My tooth isn’t that bad but my dentist told me my root canal didn’t take I’d need retreatment. “That’ll be another $1000, sir” I had to gtfo and I’m sure my tooth will eventually eat it.

I have never been interested in videos such as this but I recently had a dental treatment and now I've watched a few dentists doing fillings. It is quite clear that dentistry, when done well, has an aspect of artistry to it. The best people seem to produce wonderful results which will no doubt last a long time. I have white composite fillings which have lasted almost 30 years and I never truly appreciated the skill required to make that happen until I watched videos such as this. It's really quite fascinating and wonderful to see this as so many people, me included, often overlook the complexity of these procedures.

@@olusesanfesobi5219 Cavities resulting in pulpitis in malformed wisdom teeth (makes root canals borderline impossible), sometimes their placement in the Jaws can be unfavorable due to lack of space, they're non functioning and can cause harm to adjacent teeth. for more information read up on impacted wisdom teeth, partially impacted wisdom teeth.

@@Mblnlight Sorry you have these problems so young! I was 32 when it happened. Doing much better so there's hope even if you can't afford dental work maybe your body will fight past it. I only went to the dentist once for the first abscess and they did an x-ray and wanted to send me to another location to get the tooth pulled. $300-$1200. I left the tooth in and it split in half down the root, and the gums swelled up around it and it's stuck in my gums now. Doesn't hurt much so I can live with it.

I was amazed with the dexterity of this Dentist. He saved a patient's tooth from dying. Had I known that this procedure was possible decades ago, I could have saved my 4 teeth from extraction. I wonder, why most of the old generation dentists didn't do this procedure. They just simply prepared their tools for removal. Always like that!

This actually makes me hopeful that my tooth bothering me now can be saved. Especially since I was able to find a company that takes my current insurance for more than just extraction only. You do quite beautiful work, in sculpting the tooth to look natural and real after so much decay. I do wonder if this was done with the patient under anesthesia, or with them awake with numbing.

Usually just awake and numb unless doing major work or the patient requests to be under anesthesia. Of course, anesthesia is more expensive and insurance may not pay for it.

Looks like 14A Hygienic clamp, i think, in this protocol, first etching, later clorhexidin, clorhexidin is a temporary metaloproteinase inactivator, which can be activated through self etch as well as etch and rinse adhesive, and degrade collagen fibers, which can lead to a negative influence in the hybrid layer formation and the bond strength, compromising the bonding longevity, so its not useful in selective etch (enamel doesn't have collagen fibers)

try reading the principles of biomimetic dentistry.. they believe that good adhesion will protect the pulp better and if you use 4th gen adhesives it will be less sensitive

Very enlightening yet relaxing case handling, doc. I appreciate your philosophy to do it gently to prevent unnecessary damage. Root canal isn't the solution to everything, and you showed it. Also I like the way you try to mimic the filling to be as natural shape as possible; for every design must have its purposes. We learn new things everyday

I am having a big filling soon. And they rebuilding the tooth. Very anxious as they said it could change to an extraction if the crack is too far down. Though mine was partially not my fault. I bit on a piece of stone that should not of been in my cereal. Needless to say the company responsible will at least pay for the tooth damaged. It fractured a second tooth but it's on watch at moment thankfully not as bad. Part of the filling needed though was poor brushing. I have autism adhd and various other mental health issues that made it hard to remember to look after my health. Thankfully I have a better routine now though its amazing the work they can do!

Do we need to put the saliva ejector inside the rubber dam too or only on outside or how it's gonna be look like? Do we put bite block and do local anesthesia before putting the rubber dam? Nice job. I guess many colleagues of you would have considered the tooth a lost challenge to save it alive and devitalized it instead. You made a great job saving it. A so big cavity looks a good candidate for a ceramic inlay, but I suppose a composite reconstruction is a more cost-effective solution

I have some questions dr. the first: It is assumed that normally such a deep cavity is very probable that the tooth cannot be saved, because the decay infects the dentin, which through the dentinal tubules can reach the pulp. the pulp has its states of irritation which are reversible and irreversible. In this case, how would you classify the state of the pulp? Did the treatment not cause postoperative problems afterwards? which would mean that it was in an irreversible state. The following is regarding acid etching, it is known that acid etching should not be placed on exposed dentin due to the same thing that I mentioned before, which are the dentinal tubules through which this chemical can enter them and cause sensitivity in patients. Why do I use it on dentin? and even more curious, on such a thin layer of dentin on the pulp. What happened later? postoperative? Did the patient have sensitivity or not? I am so curious
