There are a lot of differences between athlete's heart and Hypertensive. Actually, in the early days of medicine, doctors were concerned that athletes were doing tremendous damage to their hearts because the chambers would enlarge similar to systolic heart failure patients. This was seen in physical exams, with the bradycardia, enlarged chambers by percussion, lateral PMI, loud S1/S2, etc. The stresses placed on the heart by exercise and Hypertension are also very different, and thus the adaptations are very different. Aerobic exercise necessitates a great increase in the amount and the rate of venous return, which actually leads to stretch of the chamber - the adaptation to the chamber stretching under tension is adding sarcomeres in series at the ends of the muscle and an enlarging of the chamber to accommodate the extra volume per contraction. In exercise, the vascular resistance may actually drop (usually it does) which allows the heart to contract quickly against low resistance and this favors a particular isoform, which I believe is the alpha isoform. In Hypertension, the vascular resistance is great, and thus the heart's contraction is very slow against the heavy resistance and thus a slower, more sturdy isometric contraction is necessary, which may be the Beta isoform. Animals are varied in their Alpha vs Beta isoform properties - in humans it may be the other way around. But most HTN, HF papers I read talk about isoform switching into Beta, which leads me to believe that Beta isoform is the slower, sturdier isoform while Alpha isoform is the quick, low-resistance isoform. Thus, Beta switching is not necessarily a better option, although it probably is better in HTN. Granted, I'm not a doctor although I hope to be one day. Besides that, this lecture is excellent! I had no problem sitting through the entire thing.
