I had the nmr in 2019, no heart disease present, in 2023 cac score showed 70, I am now learning about keto and started 13 days ago.

Every doctor i have asked to please order an NMR test has basically laughed in my face. The first one, she flat out said she had no idea what that was and therefore would not order it. I explained to her what the test did and she said, well I have never done that and when i asked her if she would at least learn about it because it could help her patients, she brushed me off. That was the year she did nothing other than check my heart beat for my physical, that's it. ( Johns Hopkins doctor too! ) I fired her. Next guy yep he didn't do any of the tests i wanted him to do. He said if I wasn't sick with something I didn't need to have those tests. no NMR just a lipid panel. At least he did order the vit. D test first one i ever had in 70 years.

I have visited 4 different doctors that brushed me off too! 😡 I am a very peaceful and respectful person, so their behavior only means that they feel challenged by someone else who’s not a doctor and didn’t like it. 😢 I haven’t have support at all.

@@negantoni I think the doctors feel defensive when they realize that we aren't nearly as ignorant and stupid as they think we are. That's why doctors like dr. Boz, Berry , Eckard ,Fung, Lustig hope I didn't kill his name, there's a few others. They are a breath of fresh air. But they have had to find a totally different way of practicing medicine. They too have to buck the system so I'm sure it's not easy. Where I live there are NO. doctors that I have found that really promote health through diet, but boy if you want one that promotes pill popping there are hundreds of those. Literally.

I've listened to other doctors discussing this topic and this video is by far the best one. I had an expanded lipid panel done last year and learned what the numbers meant, but this video made it even easier to make sense of it all. I love your graphics and the way you explain things.Thank you!!

Thank you, Dr. Boz. I've been trying to educate myself as I did get a CAC based on high LDL on a regular lipid panel. My triglyceride/HDL ratio is right at a 2. My fasting morning glucose has taken a year of keto & fasting to drop below a 90. I do have more plaque in 1 artery than I should for my age. Of course that is scary, but I see that I am still insulin resistant and it must continue to be my focus. I added CoQ10 & I will get an NMR to tract my particle sizes. I am so grateful for your work and your commitment to education. ❤

I will never fast again, 61 female, been doing high fat carnivore and the weight is just dropping off, at this age I would have to kill myself to drop a pound, the first few days were a little ruff, but now I feel better than I have in years, sleeping better, just eating when I'm hungry, make a great coffee in the morning with full fat cream and 4 egg yoke stevia monk fruit sweetner, I just feel like I have a new outlook on life, I can't even explain it.

Great video DOC B! Interesting. I have a high particle count 2000, but all of them are big ( size 22). Less than 90 are small.

My triglycerides were 70, and I was pre diabetic at that time. My CRP was also normal. I got to pay for a CAC done in order to keep my dr from fussing me with statins. My score was 0. I am curious what my particle size is. I may have to pay to get it done too.

Just a heads up: CAC tests do not reveal the newer soft plaques that are actually more deadly. Calcified lesions are older and more stable. These older lesions kill you very slowly by gradually reducing blood flow and oxygen delivery to the body tissues such as the brain and vital organs. Statins are not entirely protective but they are often used because people are not informed/willing to address the root cause of ischemia through more optimal nutrition and exercise. Ruptured plaque heart attack Rupture of the plaque surface, often with thrombosis superimposed, occurs frequently during the evolution of coronary atherosclerotic lesions. It is probably the most important mechanism underlying the sudden, rapid plaque progression responsible for acute coronary syndromes.

Ok, Dr. Boz records an A1C of 5.6. How is that possible? She has virtually no sugar in her diet. She is constantly starving herself (days of fasting) and still has high A1C. There must be a different diet to still have good metabolic health, no insulin resistence, no high levels of small particle LDL size. I think Keto is a temporary fix that is absolutely necessary for people with severe metabolic disorders but not a long term diet (way too much stress on your body as you are literally in a survival mode all the time (high cortisol levels). Ask Dr. Boz to look at hormone impact in long term Ketosis. I can’t find anywhere that she discusses this!!

I had an NMR in 2021 and i had been relatively not as active as I usually am during that year because of covid. I'm a personal trainer and have exercised all my adult life and my triglycerides were relatively high for me at 101 when they were usually in the 50's and my A1c was also relatively high for me at 5.6 when they are usually in the 5.2 range. What alarmed me was my ldl particle count at around 2300. My c-reactive protein is below 1 and my HDL was 67 and I'm lean and my LDL size was 21.3. I'm curious about the LMH factor (lean mass hyperesponder). Even though during 2021 I wasn't as strict on my diet and not as and because I'm lean and fit if the elevated ldl particles was a response unique to in shape people? All of my other numbers were within reasonably healthy levels but maybe I'm insulin resistant even though my A1c is usually around 5.1, triglycerides in the 50's, c-reactive protein below 1 and hdl in the 70's... I just turned 65 so maybe I need to try even harder to keep the insulin levels down.

Please seriously consider a coronary artery calcium scan. At age 65 I twisted my doctor's arm to get one and the results hit me like a ton of bricks. My score was in the severe range (over 400 Agatston units) with no family history of early events, never smoked, BMI of 21-22 for the last 40 years, BP of 115/75, hs C-reactive protein

How do I have such high LDL particles( LDL P) if all other numbers are very good (HDL, tri/HDL ratio, triglycerides, insulin, glucose, and low small particle size)? My LDL P is double the "acceptable range." Would this be a concern? Of course doc wants to put me on a statin.

So what does your example patient do? Hold the course bringing A1c under 5? My doctor mentioned a genetic component, when do you consider that? Thanks!

I see my cholesterol total number when I donate whole blood every …. 56 days..been on keto since mid 2021. Cholesterol numbers have gone up and down..last reading was 245. Lost 33 pounds total…blood sugar has improved dramatically.

Speaking from experience? ... we rarely just dropped dead. Death is preceded by years of decrepitude, pain, depression and suffering. If there's anything I can do now to ensure my healthspan it's small price to pay to avoid that misery later.

According to the last question Dr. Boz answered in the video, she said that Lp(a) was the small LDL with “the horns.” I thought that Lp(a) was associated with HDL. I had mine tested and it was 46. May I have your opinion?

What would you expect HbA1c to be after many years of keto, mine sits at 5.6 which appears higher than I would have thought it should be for that lifestyle ?

I have not verified this but I heard that without the glycation red blood cells can last up to 4 months instead of 3. If this is accurate it could skew the A1C number. Make sure to test fasting Insulin as well since that may provide some additional insight.

5.6 is a very good score . You don’t need to go lower than that by doing a vary tight regime.

Has anyone shown a higher risk of anything with higher cholesterol? If someone is not eating glucose, fructose , alcohol or omega 6? When you are in ketosis there is lots of lipid particles moving in the blood. High cholesterol is associated with longer life. We are always taught how to make ourselves unhealthy and diseased.

Those are great numbers on less than 60 hours. That takes me 5.5 days of a Dry-fast and another 3-5 just water.

@@johnhubbard6262 dry fasting is the most dangerous type of day possible. It can kill in a day. If your doctor's disagree they are stupid or every keto/fasting/science content on fasting I've ever consumed are wrong. I mean the risk of dehydration is real. I'm guessing by 5 days you mean like you rehydrated every night? Or was it 5.5 days completely straight dry fasting. I get that cancer was trying to kill you, but you could have literally died trying to beat it if you had no liquids at all for 5.5x24 hours straight with no liquids and even worse with like no showers. I'd only dry fast in air conditioning and on lazy days.

My doctor says i need to be on a Statin drug even though my Triglycerides are 35mg/dL, HDL is 88mg/dL. He says my LDL 180mg/dL and total cholesterol 275mg/dL are TOO HIGH; and A1C is 5.4%. They don't do NMR tests at this hospital. Does my doctor know what he is doing?

Nope neither does mine but it forced me to prove things to myself like learn the risk ratios Chol/HDL Tri's/5 gives you the level of small ldl particles sorry it's not in front of me but my CAC is zero and my other numbers look just like yours my BMI is 22 perfect and my BP also is fine but she sent me to a cardiologist cause I was happy with my bloodwork and she wasn't the cardiologist let me get the CAC

Diet Doctor, Saladino, Shawn Baker and Ken Berry say that if you have low triglycerides and high HDL they are not concerned about very high LDL on low carb as they are most likely large benign LDL.

The best predictor of cardio health is your Trg/HDL ratio with 1 or less being good and yours is great.

The HbA1c looks to be irrelevant. Also on my blood works My HbA1c was good but my fasting insulin was high --12.
