I think most practitioners prefer not to put them in the forehead, unless there are strong static lines that Botox alone cannot correct. But I'm sure botched lip-filler induced necrosis is much more common than blindness, simply because that's the area most often injected by non-medical people who shouldn't be doing it (beauticians, etc.) - at least in the UK, where the industry is still inexplicably unregulated. To put it into perspective: cosmetic expert Dr. Tim Pearce had performed thousands of lip fillers without any problem at all before he had his first case of vascular occlusion. He corrected it with Hyaluronidase and the patient felt so reassured that she even booked in for her next treatment on the way out! These doctors spend a lot of time dissecting cadavers before they go near real patients, you know. But facial anatomy differs between people, which is why no practitioner can swear that an occlusion will never happen.

I loved this. I would also enjoy a video on inflammatory/(hyper)sensitivity reactions to fillers, which some studies seem to cite take place in 0.5% or more of patients.

Yes I would imagine it’s more common than we realise because most people don’t report it as it happens so far after the original treatment was done

Thank you Dr Sarah. I appreciate the information given by professionals. It's probably more than we could get in a consultation. Do you think you could do a little piece on scar revision please. I live in Ireland and while we have some good derms I think I would have to visit them all and still not find a good procedural derm.

I had this happen to me. Nose filler using a cannula. Luckily, I noticed immediately something was wrong and the [experienced] nurse injector flushed it ASAP. I was left with nasty bruising, necrosis (now healed, minor scar about 1cm long on nose), and (luckily, only) a blind spot in the outer corner my right eye.

Is cheek filler a safer procedure given the anatomy avoids some major arteries? Like injecting on the bone.

The 'danger zones' are usually around the mid-line of the face which avoids the cheeks but to be perfectly honest if you see an experienced practitioner then all danger can be severely mitigated as they will know how deep to inject to avoid the blood vessels and they will use the proper technique to minimise any incorrect placement. And even if you did get unlucky and something went wrong then they would know exactly what to do to reverse the treatment to avoid any damage. Hope that helps!

Personally I feel us women are so over charged for fillers and botox that we have almost been forced to DIY these things,, the industry has gotten so greedy knowing that women and indeed men want to look better and feel better about our appearance that we readily put our self at risk,,, if the price tag on fillers and botox sessions were not so overly expensive then maybe more people would use these "Professionals" and have our treatments done "Correctly". Greed and overpricing has forced people's hand.

Yes, agree. The place I go to actually has great botox prices, but fillers are expensive. Just to do my tear troughs and mid cheek, not the cheek bones, just flat in the center. Costs 2000.00. I need temples as well, but not sure, because van cost another 1 to 2 grand!! I'm an athlete, if won't last a year for me, maintenance would be a problem. 6,000.00- 8,000.00 a year, can't do. ☹😬

Dr Dray makes a really excellent point in one of her videos about the costs behind doing a treatment. In her video she discusses that people often assume the whole cost is going into the pocket of the dr performing the treatment, however there are fixed costs such as premises and staff, variable costs such as materials and advertising for example, and of course you are also paying for the years that that dr or other healthcare professional was training without receiving any income at university, costs of ongoing training and soforth. So its a funny one, but I can totally see where you are coming from though.

and what about simply injecting hyaluronidase with great pressure into the main blood vessels near the nose that lead to the eye..?

Very hard to cannula the vessel in a regular clinic environment as it is very tiny. This has been done in a hospital and vision was partially restored but you’d have to have specialist equipment and be trained in that particular procedure. Maybe an ophthalmologist or occuloplastic surgeon could do it

Is it possible to use ultrasound to see where the blood vessles are? If so, why is this not used?

yes but... you cant see ones which are very tiny, also this is a specialised technique and injectors arent often radiologists so the test would be useless unless you knew how to interpret it, and you'd probably need a third hand to do the scan as you need two to do the injections

Not threads, but theoretically Profhilo, however I dont think its ever been reported

@@DrSarahTonks Interesting. I assumed that Profhilo being inadvertently injected into a vein wouldn't be a problem, because it's so runny. You can even use it topically as a serum. Could it really cause a blockage?

You can still have filler but more risk of slow healing

I have lots of very visible veins in my temples, is this a high risk area for blindness? I talked to an aestictic nurse, she said she can do my cheeks and undereyes, but the temples are only done by the doctors. There are 2 plastic surgeons at this office. Guessing because it's very dangerous??

Is it possible to get Retina Detachment after lots of fillers in the face? 6 months after I first started having fillers I started having left eye issues I also had Plexr done twice. Could this have caused it?

No the filler treatment won't be responsible for a retinal detachment

@@rachnamurthy2817 thank you. What about Plexr ? I had it as well during that time twice on the upper lids. As well as Rhinoplasty that affected the bones near the eyes.

The most common cause of a retinal detachment is due to myopia. I can't really comment on whether the plexr or rhinoplasty is related, but if uncomplicated procedures then i think it unlikely

Yes, it has been reported from injections anywhere on the face

@pj 596: If you're not squeamish, Google pictures of facial dissection. You'll see that the whole face is a tangled web of blood vessels! A good medical practitioner will take precautions such as aspirating and injecting very slowly and cautiously, while monitoring the patient for any adverse response, but nowhere is entirely safe for fillers.

And still not guaranteed to work! More of a last resort if Hyaluronidase at the site of injection hasn't worked, I guess.
