In Plague Inc, the game seems to ignore your disease until you have 2 Symptoms (at which point it has a low chance of detection). As you increase the symptom count/severity, the Countries in the game will take more and more drastic steps to stop you.

On hardest difficulty, the game will eventually detect you even if you have no symptoms (usually when you're infecting your second main country) Some of the base symptoms seem to be more noticeable too, but that's just a feeling I have from playing so much. Coughing tends to be the most invisible symptom, it doesn't get noticed any earlier than if you had no symptoms even on the hardest difficulty, so it's great when you start in China. Insomnia seems to be the most noticeable (and also the worst base symptom to evolve).

I know its an old video but the key to winning and beating the game is to make your disease as infectious and contagious as possible as quick as possible but keep the lethality to a minimum so doctors and scientists won't pick up on it or don't consider it a threat. Then wait until atleast 90% of the world is infected, then start dropping your saved up XP points in letality and make it as deadly as possible. That way the scientists don't have enough time to create an antidote. I beat the game several times playing it that way. ^^
