this is a really bad keratosis, honestly i've watched an absurd amount of content like this and it might be one of the worst i've ever seen. forcing it would have been close to medieval torture.

It's just, I can be a control freak at times and I start talking to my screen during the procedure like it's some horror movie and you wonder why the friends go out to a dark, spooky place in the middle of nowhere, then they lose one friend and someone suggests; "I think we should split up to find them." The pressure from the large amount of wax must have been there long enough to form such a large cavity in the ear canal, it must have been years of discomfort and headaches, migraines and probably dizziness for such a deformity to have been made like that.😮 I con only imagine the pain😓😵💫

It appears to me a nasty case of Keratosis Obturans (possibly grade 3) which is a skin shedding issue and not necessarily ear wax. Good job with being persistent! Hopefully it brings them relief.

I appreciate the efforts here...the hook tool may have its place in other cases, but here it more so (has little resistance and) goes through the debris texture rather than push or dislodge it...I believe that a tool with a larger surface area (e.g., mini- scoop) could have been more effective for at least dislodging - and if not effective for removal, the suction tool could possibly help...

Yeah. I was wondering why not use the Jobson (spelling may wrong) horn and try the unpack some of the keratin/skin from the middle and then the standard suction to clean the sides. ..

@@fckyoperm253 Yes, the jobson hook/horn - which I imagine would suffice...still, a tool with an even greater surface area (edge-to-edge) would be best (as long as it both accommodates and does not injure the ear canal)...blessings 🙂

@@stefanhortell4046 Stefan, I appreciate your point...still, given the degree of ear canal contact with the hook tool here, I imagine that merely replacing that tool with a scoop or jobson horn could be more effective - and without increasing pain...but I am not a trained professional... I will add that, in watching different ear cleaning channels, I have noticed that specialists may use particular tools and not use other tools...for example, some specialists use a thin hook (used in this video), while some use a thick hook...I have seen thickness differences in jobson horns, too......I do not know if the differences are due to availability of tools or preference of tools... Why don't they use some hydrogen peroxide to fizzle it and to loosen it up and then they could use those little tools that look like spoons to scoop around the edges and use some tweezers to pull it out, I would think that would work much better. The tools seem to be big a lot of times it just pushes it back in. They need tinier tools.

It's not earwax. It's a keratosis obturans, which is keratin (dead unshed skin) that is still partially adhered to the ear canal.

Correct use the spoon and tweezers, safer, always safety come first. Now using This one with the "L" tip is too dangerous. Easy to hurt the wall in the ear.

Was this skin or wax? I actually thought from the thumbnail of the video that it might be fungal infection. I expected it to be smushy. Boy was i wrong! This stuff was tough as old boots! I would've been tempted to flood it with drops or oil and ket it soak...then again, i know nothing about these things LOL So glad you were able to clear it!

I believe this is a case of keratosis obturans. I'd even say it's a grade 3 with the widening of the canal and bleeding from releasing connective (granulation) tissues that have formed due to the trauma/pressure of the skin building up.

Since he doesn't narrate I'm going to give my own diagnosis as I've been studying Audiolody for the past 8 years as a hobby... I'm going to say that although I don't know if the patient presented with otalgia, but from the over all condition of the ear I'm going to say it's a safe bet. With the amount of bleeding especially in the beginning I'm going to say it is a Grade 3 Keratosis Obturans. Anyone agree? Anyone have a different diagnosis? I'm interested to know. Edit to add : Now that I finished watching the cleaning I'm DEFINITELY changing my answer to a Grade 4 Keratosis Obturans. That global widening and bleeding is absolutely bad enough to classify for a Grade 4 assuming as I stated before, that the patient did have otalgia (ear pain).

I think you are almost right,to me it looks like a grade 3 close to grade four. There was definitly pain,probebly severe!

This looks like a keratosis obturans. The patient is likely anesthetized because the area being worked is quite sensitive and would be painful for the patient otherwise

It is Keratosis Obturans. He always calls his KO procedures ear wax from he'll... confusing for some it's mostly skin! 50/50 as to whether the patient has any anaesthetic. Some use lidocaine or a local, but many are carried out without anything at all.

Tricky job. No idea where the ear drum is behind the impaction. And it's such a mixed bag. Soft and really hard bits throughout it. The canal in such distress that it was bleeding so easily and likely swollen and so painful for the patient. The safest and best options were th ear pick and suction. I don't think the flat scoop would have created more pressure and pain. You definitely earned you fee on this one. Patience and persistence paid off.

@@uwemattiss880 I sure would prefer anaesthesia or at least lots of strong opiats combined with midazolam. Guess they have naloxon if it should go wrong.

I watch a lot of these kinds of videos and this guy is an instant fav. I like his method a lot. One of the quicker ways to get the job done. Play at 1.5x speed for the best experience, imo. You gotta remember the ear canal is an insanely itty bitty little space, not even a millimeter can be the difference between comfort and a whirlwind of agony. It's like Operation, on drugs, with the actual life of a real person hanging in the balance. Depending on the person, one wrong move could even land them in the ER. So it pays to be as tedious and patient as possible.

Suction tool used to remove e it from the side of the ear. Touching it with the hook would have been very painful.
