A) The problem will dealt with soon. X NG B ) It isn't clear who ought to be tackling the situation. C) It is hard to know what the best course of action .. NG D) Many pople refuse to acknowlege. There is a problem. NG Dixon-Woods compares the issue of patient safety to that of climate change, in the sense that, it is a problem of many hands, with many actors , each making and contribution towards the outcome anc there is difficulty in identifying where the responsibility for solving the problem lies. Many patient safety issues arise at the level of the system as a whole but policies treat patient safety as an issue for each individual organisation.
