Agreed that LTx is the treatment of choice in suitable patients within the extended criteria but in our low& middle income countries LTX is not available to most patients as it is resource intensive and prohibitively expensive for patients as well as the National health. Hence the only potentially curative option is resection in patients who are operable and resectable. Even patients with un-resectable tumors can be down staged by prior techniques such as PVE, TACE or ablation. So the need for more HPB units with IR facilities to manage the burden of malignant liver disease.
