It seems there's some sort of isolations under the capacitor. I assumed it was some sort of optocoupler like you see in SMPS circuits but there doesn't seem to be anything knocking down the voltage so just odd to see a coupling capacitor at HV for a display

Yes, you are right. There is a line under the capacitor for insulation purposes. At first glance it looks like an optocoupler.

Output current is limited capacitively using a high voltage (few kV) capacitor with a small capacitance (tens of pF).

What kind of display would be working with 5kv?

LCD TV inverter lamps typically operate at a voltage of around 600 to 1,000 volts. I guess the manufacturer couldn't find a lower voltage capacitor. :-)
