Embryo Freezing ! Frozen Freezing or Fresh Freezing Transfer

Embryo Freezing क्या है Embryo Freezing? किन Cases में Embryo Freeze किए जाते हैं? और आज का जो बहुत Important सवाल है, क्या Fresh Embryo के Comparison में Frozen Embryos में ज्यादा अच्छे रिजल्ट आते हैं? और यदि दंपत्ति को Option दिया जाता है तो क्या उन्हें Frozen Embryos Transfer के लिए opt करना चाहिए? तो आइए सबसे पहले जानते हैं कि Embryo Freezing क्या होता है? Embryo Freezing जब से IVF की शुरुआत हुई है, तब से Embryo Freezing का प्रचलन है! शुरुआत में जो Embryo Freezing की Technique थी वो Slow Freezing थी! Slow Freezing या Programme Freezing मे एक विशेष तरह का डिवाइस होता था, Slow Freezer! जिसमें Step wise Embryo का जो तापमान है वो कम किया जाता था और -190 डिग्री तक इस तापमान को ले जाया जाता था जब Embryo Freez हो जाते हैं तो उन्हें Liquid Nitrogen में Preserve करके और रखा जाता था! Slow Freezing के दिनों में जब इन Embryos को वापस रूम टेंपरेचर में लाया जाता था यानी उन्हें थॉ किया जाता था! तो जो Craniocervical Rate है, यानी जो प्रतिशत है Embryos का, जो Survive करते थे, वो बहुत ज्यादा नहीं होता था, 60 to 70% Embryos Survive करते थे, और Pregnancy के Chances लगभग 20 से 30% हुआ करते थे! यह सब कुछ Change हो गया, जब लेट 90's और 2000 के दशक में Vitrification नाम की Technique इज़ाद हुआ! Vitrification का अर्थ होता है Flash Freezing. Flash Freezing मे बहुत ही Fast Embryo को रूम टेंपरेचर से, यानी 37 डिग्री टेंपरेचर से और उसे बहुत कम तापमान पे -196° पर ले जाया जाता है! Vitrification या Fast Freezing जो Technique है इसमें Embryos का जो survival है ये बहुत अधिक बढ़ गया है और अब लगभग 95 to 100% जो Embryos है वो Cryopreservation के बाद थॉ करने पर Survive कर जाते हैं और Pregnancy Rates की बात करें, तो लगभग 70% to 80% Pregnancy Rates भी इन Embryo Transfer करने के बाद हमें मिल जाते हैं! पिछले कुछ वर्षों में कई ऐसी स्टडीज की गई जिसमें ये देखा गया की Frozen Embryo Transfer जब किया जाता है तो वो उसमें Pregnancy का प्रतिशत Fresh Embryo Transfer के Comparison में ज्यादा आ रहा था! यही हमारा भी अनुभव रहा है! हमारे Clinic की यदि बात करें, तो हम Self Cycles की यदि बात करें, तो Fresh Embryo Transfer के बाद हमारा Clinical Pregnancy Rate लगभग 43% है परंतु Frozen Embryo Transfer, जब हमने सारे Embryo Freeze किए हैं, Fresh Transfer नहीं किया है, और Frozen Transfer के लिए हम गए है तब ये जो यह जो प्रेगनेंसी का प्रतिशत है ये लगभग 58% है! यह जो Difference हैं लगभग 10 to 15% का, यह बहुत Significant है, और अब हमारे Clinic में लगभग 70% जो transfers है वो Elective Frozen Transfer है यानी कि IVF Cycle के दौरान Fresh Embryo Transfer न करके हम सारे Embryos Freeze कर लेते हैं और अगले Cycle में जब Endometrium अपनी नॉरमल स्थिति पर है, Hormone Levels वापस आ चुके हैं, Endometrium की Receptivity अच्छी हो चुकी है, उस समय यह Frozen Embryo Transfer किए जाते हैं, तो जब भी आपके Clinic में आप डिस्कस करते हैं, IVF का आपका Procedure चल रहा हैं तो डॉक्टर से हमेशा पूछे कि, क्या Freezing की Facility उनके पास Available है? और उनके Frozen Embryo Transfer के Pregnancy Rates यदि अच्छे हैं और आप के चार या पांच अच्छे Quality के Embryos है या इससे ज्यादा है तो आप सारे Embryos Freeze करा के अगले Month में यदि Frozen Embryo Transfer के लिए जाएंगे तो अधिकतर Studies यही बताती है कि इसमें Pregnancy के Chance हमे ज्यादा मिल सकते हैं! क्या कुछ Cases है जिसमें हम Frozen Embryo Transfer नहीं करते हैं? हां, यदि Couples के Embryos की Quality बहुत अच्छी नहीं है Grade B या Grade C के Embryos है, या फिर Embryos का नंबर 3 से कम है उस केस में Frozen Embryo Transfer ना करके हम अभी भी Fresh Embryo Transfer करना Prefer करते हैं कुछ ऐसे दंपत्ति जिनके पहले Fresh Embryo Transfer के बाद Miscarriage हो गया है, उन Cases में भी हम Fresh Embryo Transfer करना Prefer करते हैं! कभी-कभी Logistics Reasons, जैसे यदि कपल अगले महीने आईवीएफ किसी कारण से नहीं ले पा रहा है या बाहर की कंट्री से आए हैं दूसरे गांव में रहते हैं उनके लिए वापस आना मुश्किल है उस केस में Fresh Embryo Transfer भी कर दिया जाता है तो आज हमने जाना Embryo Freezing और Frozen Embryo Transfer के बारे में यदि इस विषय मैं आपकी कोई भी जिज्ञासा है कोई भी डाउट है तो हमें जरूर लिख भेजें आपके सवालों का जवाब देने में हमें बहुत प्रसन्नता होगी धन्यवाद
