Too often this is too true @Amed Tajan It should be made a mandatory part of having a license to operate such facilities that you have on file with the community (city, state, province, prefecture etc) cooperative response plans.

I saw some concern and rumors in the comments about why CSB hasn't uploaded in awhile, some people even saying the agency was defunded. I'm relieved to say that's not true! They just submitted a request for funding from the US government in 2020, and they want only $12 million, a drop in the bucket as far as federal agencies go. I looked at their budget requests for the last few years, including for 2020 and they are actually spending more on making videos this year, up from $300k the last few years to $350k. Or at least they want to...the budget has yet to be approved! If you are an American resident, please consider calling/emailing/texting(?) your local congressman and senator and urging them to approve the full 2020 budget for the CSB! I think everyone feels these days like we can't do much about the government, but this is something that we can make a real difference and help out with something that we love and learn from.

When studying accidents like this, it is always a good idea to point out where things went right. This is a shining example. Let's hope more refineries study this aspect as well, and copy what works well for incident response. And thanks to the good folk at CSB for putting out videos like this. It's worth every penny of their budget and then some.

Extinguishing the flames is one thing, but cooling everything off so it doesn't restart is very much another. Good on everyone here for managing a scene like this.

The BP Refinery here in Western Australia runs several ERT training events a year with the local career firefighters and police.While we've never had a major incident like this in our 60ish years of operation, I like to think we have the best responders in the world at hand if the worst case scenario ever happens. Also, USCSB your animations are always 1000%. Great work as always.

Perhaps there are strong regulations that BP adheres to that have prevented a major incident, too. Certainly BP hasn't done all that well in the US or UK, where regulations are insufficient. I applaud your company and state for their diligence.

I always enjoy these videos, so it's very refreshing to see an incident where nobody was injured or killed. Obviously, something failed that shouldn't have, but at least in this case, the company was prepared for things to go wrong.

Sorry to rain on your parade but if you listen to the beginning of the video Emergency Medical response which usually means injury was required. Plus it doesn't seem that the asphalt leak was discovered as quickly as could have been. A fire in a tank farm could have had resulted in a chain reaction of exploding tanks with lighter hydrocarbons like diesel or gasoline.

It's nice to see this stuff is easily accessible to anybody. There is a lot of useful advice that one can find even if they don't necessarily work in a chemical or industrial environment. The number one thing that prevents accidents is situational awareness being aware of one's surroundings at all time. Never being complacent in the workplace knowing at anytime something dangerous can creep up. I have watched several other videos and find these videos very instructive and interesting.

Exactly. Just the simple point that is being hammered home in all of these videos is that small problems, when unattended or ignored, will eventually pile up in a massive disaster. It does not really matter whether we talk about town-sized chemical plant or a marriage.

The Emergency Response Team showed great bravery in putting out a fire inside a tank farm. Those tanks could have contained diesel, fuel oil or gasoline and heat from the asphalt fire could have caused them to hurst. Hitting a tank full of asphalt was also lucky because hitting a tank of anything else could have possible sent the whole refinery up in smoke.

All of these large industrial centers should have redundant sets of inert-foam flame retardant pipes which automatically burst during an explosion, which would flood the area and slow down the spread of the fire.

This should be a standard in the way companies working with toxic, explosive or fire hazard level materials, should always work with local emergency departments. It boggles my mind the way half of these companies had no real contracts or "line to line" communication method with their local emergency departments.

J R, I googled "AB Silicones CSB" and found a notice that the CSB had deployed a team in May to investigate that explosion (I live about 7 miles from the location). If they do a video, though, it won't be anytime soon - their investigations seem to be so thorough that they take a long time to publish.

Daloki thanks for the update. Yes they do take a while but their videos and investigation are so good it’s worth the wait. Glad they are looking into it I believe there were several fatalities. Another company I would like to see is KMCO east of Houston that had two fatalities recently. Some say there was time to get people out of there others say not so.

Today, 7-31-19 in Baytown, Texas, 37 people were injured when a fire occurred at an ExxonMobile facility. It burned polypropylene, and was allowed to burn itself out. I hope you'll make a video about that.

Defunding The Chemical Safety Board Is A Bad Idea And Likely To Increase Chemical DisastersUnfortunately, the 2019 budget proposed by the Trump administration zeros out funding for the USCSB. Its requested fiscal-year funding, $12 million, is modest for a government agency. Likewise, the 2018 budget also proposed to defund the USCSB. This sustained effort reflects an ongoing de-emphasis on chemical safety - as a second example, Environmental Protection Agency Administrator Scott Pruitt has indefinitely delayed bans on the use of three hazardous chemicals, shown to be toxic to human health. Chemical production is an essential component of modern society. This does not mean that there is not room to improve practices in manufacturing, storing, and shipping chemicals, and in ensuring the safety of those who work in or live near chemical plants. The vantage of an independent group is crucial for identifying those aspects that can and should be improved. Defunding the USCSB, which provides this indispensable independent perspective, is likely to hinder efforts to identify the causes of chemical accidents - especially in low-regulation locales. Moreover, it is also likely to worsen our ability to respond in previously unforeseen events, such as the heavy flooding of Harvey, that may be exacerbated by climate change. Finally, it is likely to cost lives in future incidents.

I wish Congress would give the CSB rulemaking powers. It’s insane that all they can do is recommend, given all the work that goes into their findings.

I think it's more that their ruling would contradict the authority of OSHA. However the USCSB is being plagued by politics regarding its existence let alone expansion of authority. Ideally the USCSB would be better funded as an independent body like the ESRB and get funding from the industry at large. It would be like the video above but on a larger scale.

@@russellmoore8187 the USCSB's budget was around 20 million and scheduled to drop further. From comments on this channel it's mostly individual units neglecting safety culture and some companies like the ones above are genuinely interested in promoting workplace safety. It would be literal peanuts for companies to even double or triple the current USCSB budget for them to give guidance to the companies involved. Many companies have voiced their support for the USCSB and acknowledged their work but most people who don't know the difference between OSHA and the USCSB feel like they are a waste.

I disagree. Like the National Transportation Safety Board, the Chemical Safety Board should only make recommendations, not legislation. We do not want a conflict of interest to infest the investigation process. The more independent they are the more people will take their recommendations seriously.

You should do an investigation of the Ziggenfelder Popsicle Manufacturing Plant in Wheeling, WV. It is happening now, and ammonia pipes were not fireguarded.

Google "CSB [name and location of the plant]" and you'll probably find it. I found out by searching "CSB Waukegan ABC Silicones" that they are investigating an explosion 7 miles from my home that happened 2 months ago.

This seems to have turned out great but I see problems with this. Why did the asphalt go all over the place shouldn't there have been measures in place to contain it even if the tank fails? I don't like the idea of putting all these fire department personnel inside this burning tank farm full of combustible liquids either. Perhaps they could have monitors (nozzles) mounted within the tank farm that they could pump water or foam through to allow first responders to stay back at a safe distance. That or a dry chemical fire suppression system.

They said at the end that the investigation was not yet completed. It takes a long time to get that done, and then I'm sure it takes a long time to put together one of these videos. That's probably why they do interim videos, to get a head start.

ROFL well at a certain point they lost funding, but it was then restored. Not sure how well though...Hopefully with this new found wave of people watching the youtube videos it will become bigger. I've been watching since...2011 or 2012... Seeing the views go up brings tears to my eyes... back in the day some videos would get no more than 5000 views on a good year...

Very good question everyone else seems to think that was a great and good thing to happen. The emergency responders showed great bravery by fighting a fire in a tank farm. The CSB by releasing this video before its full report shows questionable judgement.

I thought that too but this video seems to focus on the teamwork of refinery & local firefighters as opposed to conclusions about the cause of all this.

They had an entire team of on site fire techs. This is honestly the best company portrayed in this channel and the company behind the Arkma plant is a close second.

Justin Koenig It seems both the company's fire team and the public fire dept did an outstanding job given the circumstances. They seamlessly were able to coordinate their efforts, reducing impact to both the plant itself and the surrounding community.

There are onsite firefighting measures, that's what the ERT and the stationary monitors are (Plus fully kitted out fire fighting trucks specialsed in the site's hazards) But it can't hurt to have the local firies involved to maximise the manpower and limit the risk. The only extra precaution I didnt see mentioned was the containment berms around the asphalt tanks, these should be pretty much the standard on all sites with above ground tanks. Would have stopped that spread if they had been in place/working.

No one doubts the bravery of the first responders fighting a fire in a tank farm that could have gone up in smoke and burned for days. The real question is how something like this happened in the first place.

@@kimobrien. And the CSB is investigating that. It's important to praise companies for doing something right, though, even if they didn't get it all right. There are companies out there who would've let the whole place go up in flames before they spent money on cooperative training efforts.
