Although operator ignorance is a major factor, the system had several issues. First, there should not be an override for a critical safety process. There should also be blowback preventers, and proper gas monitors installed. Japanese manufacturers refer to the is Poka Yoke, or "mistake proofing". Americans would refer to it as "Idiot proofing" either way, removing the chance of human error should be a priority.
