I have nothing to do with OSHA or anything like that. But I still love to watch these videos. Very interesting and educational. I have started side eyeing all the fire safety violations at my workplace (the fire exit route for employees is partially blocked off by stock)

So, prayers and fingers crossed that the originally engineered post-gas wash vented contents were below the ethylene oxide's LEL before moving over AN OPEN FLAME to a catalyst??? ......nah

Although operator ignorance is a major factor, the system had several issues. First, there should not be an override for a critical safety process. There should also be blowback preventers, and proper gas monitors installed. Japanese manufacturers refer to the is Poka Yoke, or "mistake proofing". Americans would refer to it as "Idiot proofing" either way, removing the chance of human error should be a priority.

The explosion could be felt 30 miles away. To put that into perspective the bombing of Hiroshima could be felt as far as 37 miles.

Looks like they had a caustic fill line as well, if he had started pumping sulfuric acid into that tank that would’ve been worse i reckon

There are other ways to get the resin to kick faster idk why they wouldn’t try using if two part more of the catalyst or even stirring rapidly ? There are many ways to do this that would be very last resort especially in doors I pray someone pays the price that’s flat out stupidity

Republicans: This tells us we need fewer regulations! Oh, and our workers get too much time off and don't work long enough shifts...AAANND...we now need to reintroduce children into the labor force. We're gonna need 'em once we round up those 12 million illegals and ship 'em home!
