There are four yugas in Hindu scriptures, the one we are in is the last yuga called Kali Yuga. This yuga (age) is the darkest age where all the people have become selfish, greedy, and brutal to each other and it will get worse as this age progresses. There's a prophecy that when the dark age would be at its peak Lord Vishnu's 10th avatar, Kalki will take birth and end all the suffering of this Yuga and free the people. That would be the rise of a new Yuga. The story of Mahabharata is connected to this, I'll try to give you some context of it. The war bits, piles of bodies and the woman getting shot in her womb are all from Mahabharata which happened between 1500-2000 BC. It was a huge war that had lots of bloodshed. It was basically among cousins for the throne, but the intention here was a lot bigger actually. It started as a feud for throne but it escalated to be a war of Dharma vs Adharma (Dharma here means duty and not religion). The two parties of the war were Kauravas (100 brothers) and Pandavas(5 brothers). Kauravas had done a lot of Adharma (bad deeds, ethically and morally) and refused to give anything to Pandavas, who rightfully deserved to be on the throne. At that time, the eldest brother used to be the crown prince and then king. According to that, Yudhishthira, the oldest of Pandavas, was actually the oldest of all Pandavas and the Kauravas and should've been the king. But Duryodhana, the eldest of Kauravas wanted the kingdom for himself so he tricked the Pandavas and their mother a lot of times, attempting to kill them. They even tried disrobing Draupadi, the Pandavas' wife (It was a Polyandrous marriage) in the royal court in front of all the ministers and elders of the clan but Lord Krishna saved her. One thing led to another and finally, the war of Kurukshetra happened, the Mahabharata war. Lord Krishna, the 9th avatar of lord Vishnu, was the charioteer of Arjuna, one of the Pandava brothers, the undefeatable archer of that time and Ashwatthama, Karna, all the 100 Kauravas, dronacharaya (he was the teacher of Kauravas and Pandavas) and Bhishma, were from Kauravas side. Mind you, all these people were mighty warriors but were from the Adharma's side so they faced death as a fruit of their bad karma. The war went on for 18 days killing people left and right, the piles of dead bodies are glimpses from the same war. The war ended, and Pandavas won. Ashwatthama, who was the son of Dronacharya was seething with anger at the defeat. His father had been killed by cheating and his friend, Duryodhana, lay dead in front of his eyes, he wanted revenge. He sneaked into Pandavas' tents, their sons were sleeping there, he mistook them for Pandavas and killed all five sons. Enraged, Arjuna summoned Brahmastra, the most powerful celestial weapon that could destroy the earth, and as a response, Ashwatthama did it too. Lord Krishna, advised Arjuna to not use it as it would cause a lot of destruction so he took it back. But Ashwatthama had half knowledge, he only knew how to summon it and not withdraw it, so Krishna told him to shoot it in some other direction away from mankind, but he directed it towards Uttara, Pandavas' daughter-in-law, who was pregnant with the only heir of their clan because all of their sons were dead. The scene of the pregnant lady getting shot is Uttara only. That scene is from the past. Ashwatthama wanted to end Pandavas' lineage which would mean the millions of people who fought would've died in vain. Agitated by this, Krishna takes away Ashwatthama's divine gem that was adorned on his forehead and made him almost invincible. Krishna curses him for attempting to kill an unborn child. He curses him to roam on this earth till the end of Kali Yuga, his body covered in sores, puss and blood. The wounds would not heal and keep bleeding and well up with puss, and gangrene, rotting and smelling so bad that it would repulse people away from him. He would suffer from all sorts of physical diseases but not die. This curse would haunt him for 3000 years and wait for Vishnu's 10th avatar, Kalki, who would free the earth from all the atrocities and dark times. Ashwatthama is supposed to help Kalki fulfil his mission. The old man with the gem on his forehead is Ashwatthama, repenting for his mistakes all this time and finally, Kalki is about to be born so he's doing his part to protect him. Deepika is pregnant with Kalki and all of the fuss is to protect the unborn avatar of Lord Vishnu. The movie delves into the future of this Yuga (age) and how stuff from the past is connected to stuff in the future. Kalki is said to be born around 2898 AD, 6000 years after the Mahabharata war, hence the name. Also, Ashwatthama is alive all this time because of his curse. P. S. There are a lot more details connected to the story of Mahabharata because it's literally a story with thousands of characters and their various dharmic and adharmic acts. I might have made mistakes in the comment but that's because it's been a long time since I read Mahabharata. There is a background story to the OLD man you saw. He has been cursed by Sri Krishna during the end of "Mahabharat" war, because the OLD man as a warrior has committed a sin/crime. So, he gets a curse that he will roam the earth without death and he will suffer, because killing him would have been an easier punishment. That OLD man play the character called "Ashwathama". He is destined to live forever, but will ultimately be responsible to protect the "to be" born child "Kalki". According to Hindus, when the world has extreme crime, Kalki will be born as a savior and restore "DHarma". I cannot type much more... But, its a very fascinating story. According to me the director did a fantastic job to bring this to the screen. LOVED every bit of the movie. If you get a chance, pls watch it. You may need a bit of a context to watch this movie though, to appreciate it.

If you go and watch the movie, I would suggest to get a little background knowlege about Mahabharata, and the characters showed in this movie, namely Ashwathama, Karna, Arjun, Shri Krishna and their role in the Mahabharata. This will help you in realizing the importance of some of the scenes in the movie. Also, Kalki is the prophesied tenth and final incarnation of the god Vishnu. He is described to appear in order to end the Kali Yuga, one of the four periods in the endless cycle of existence. He's described in the Puranas as the avatar who rejuvenates existence by ending the darkest and destructive period to remove adharma (unrighteousness) and ushering in the Satya Yuga, while riding a white horse (Devadatta) with a fiery sword.
