For pizza, they may also ask what size if you don't say! ex "Hi, I'd like to order a large pizza for pick up." Some places use small, medium, large or some use measurements - ex 12, 14, or 16 inch pizza" (usually refers to the diameter across) - most have how many people they feed or images on their websites now (and for lots of places you can do most of your ordering fully online)

Pretty much the same thing. But "take out" can also refer to a type of food as well. ex "I don't want to cook, let's get take out." "To go" is more specific ex "Could we get that (ex, a specific meal) to go?" - Some places also call it carry out. I think its mostly to mean not Delivery, which is another option. Ordering "to-go" or carry-out means that you plan to go and pick it up yourself instead of ordering delivery (where you give your address and there is usually a fee for the driver)

Take-out is usually used in a context where it would be a noun, while to-go is usually used in a context where it would be a verb. These are interchangeable if used as an adjective. I could describe a food order as a take-out order or a to-go order, and it would mean the same thing. In British English, take-out is called take-away.
