I don't mind NPC's not immediately running away. It'd be cool if we got a mixture of reactions: 1. Someone not paying attention, maybe on a call or listening to music on headphones, gets surprised by the explosion 2. Someone noticed that something was thrown, thinking maybe it was a rock or something. They could say 'what was that?' or 'huh?' 3. Someone recognises the sound of the pin being pulled and immediately tries to subdue you, get out of the way or help others get out of the way. They could shout 'get down!' or 'grenade!' They could even say 'what the hell?!' before you throw And then after one's been thrown, subsequent throws will have NPC's run in terror, try to stop you or help others. I do find it a bit more unrealistic in GTA Vice City and onwards that EVERYONE immediately knows you've thrown an explosive and tries to run away. If that happened in real life (god forbid) I doubt everyone would immediately recognise you've thrown a grenade. They may not have the experience with grenades or it may not have set in that you've thrown one. Maybe it could be a bit too disturbing or emotionally affecting but balancing the realism and the craziness would really sell these interactions
