why you use 4mcg instead of 2mcg/min?

Older guidelines recommended 2-10mcg/min. Titrating to the effect that you're looking for. It's not uncommon to now see 0.5mcg/kg/min.

From where the 4mcg/min come from? From thr epi1:1000 or what ??
