Hi Dr Randy thank you so much for the informative videos. 2019 I had a miscarriage at 2months in 2020 I had a chemical pregnancy in April another chemical pregnancy in August 2020. This year july I went for an IVF a fresh cycle all was perfect BUT! The embryos did not stick. This August I had PRP🤷 my husband and I are baby dancing every 2nd to every third day. We r trying so much to be pregnant, we have been trying for 5 years no luck. Regards Nancy

I had two failed FET with two high quality embryos transferred each time making it a total of (4 embryos) . My Dr. Suggested we do an ERA test along with EMMA and ALICE. I read a lot about the ERA and wasn’t quite sure if it was worth it. I ended up doing it mostly because of the ALICE and EMMA test. How common are abnormal microbiome and chronic endomteritis ? And do they really cause implantation failure?

We have been doing the ERA for almost 10 years. As a result of the most recent data, however, we are no longer recommending it. As for tests of the uterine microbiome, there are no clinical studies to indicate a benefit for these tests. Maybe in the future? Hello doctor, How can we decide how much dose of progesterone in oil injection is needed for successful pregnancy? I have done multiple FETs , most of them failed implantation, 1 chemical pregnancy & 1 this month showed small 6mm gestational sac at 6th week then my hcg levels dropped & everything ended. We had 3 PGD embryos those didn’t implant at all. I have taken 1ml/day of progesterone injections (500mg/10ml vial) in all of the cycles . Why I am not getting successful pregnancy. Do I have to increase my progesterone injection dose ? Please help. Thank you.

Do you not conduct any ERA testing anymore then because of these studies? Do you offer the gene testing for uterine lining to find out if the lining is ready for implantation? I am concerned since I have had 4 failed transfers in the past about my timing. Also in normal cycles my luteal phase seems to be short and bbt always drops day 4 after ovulation for some reason.

This is exactly what my question has been.. each month would the genes not differ, regardless of it being a medicated cycle (same amounts of meds). My first FET transfer failed and this test was recommended for me, but i think i will go ahead and try my luck and just have a transfer without the test.

Well done study which supports its conclusion which was "Among patients for whom in vitro fertilization yielded a euploid blastocyst, the use of receptivity testing to guide the timing of frozen embryo transfer, compared with standard timing for transfer, did not significantly improve the rate of live birth. The findings do not support routine use of receptivity testing to guide the timing of embryo transfer during in vitro fertilization."

Hey doctor question my other doctor from the infertility clinic in Atlanta ga said everything is good with me but I have pcos so she told me to go back and give my doctor my medical records so he can provide me with clomid but still he keeps putting me on other medications but the clomid and the other 3 he keeps saying metformin but I'm telling him it's not working I've been dealing with him for 4years and I'm still at point A like I'm trying to get pregnant b4 I'm to old but hes not wanting me to do anything but metformin and it's not working like move on to other medications and yes I'm doing everything I'm supposed to do I've lost 20 pounds and I'm eating right and exercising and taking them meds whe I go to him everything is not good but when I go to other doctors they let me see my stuff and its good my testosterone levels was high to though but I'm doing everything right am I ever going to get pregnant am I

Hello doctor, I have had 4 natural chemical pregnancies then finally opted for IVF. In my 1st ICSI cycle I had 4 embryos out of which 2 were euploid. Got those transferred but did not result in pregnancy. So went ahead with ERA test in October 2019. Its result was 'EARLY RECEPTIVE BY 12 HOURS'. In my 2nd ICSI we got 3 embryos but unfotunately all were aneuploid. Now I am planning for 3rd ICSI cycle. So my question is- 1) Should I go for PGS this time ? 2) Should the transfer be done according to ERA test result? 3) Its been 2 years can ERA result change? Is a repeat ERA required? ERA results are supposed to stay the same over time but do the results matter? As the video states, maybe not.

But are these studies you have invoked relevant? It seems these studies compare things that can not be compared: people doing ERA with people not doing ERA. I would do a study like this: take 2 groups, both with women who have a misplaced window of implantation than do an ET at 5 days (so outside the WOI), in one group and at the established time through ERA, in the second group. Only this way you can obtain relevant results.

There are no large scale studies on the impact of anesthesia on the success of embryo transfer. We have done it on rare occasions when a patient cannot tolerate a speculum in the vagina
