Evaluation of the Primary Care First Model

All CMS Innovation Center Models are rigorously evaluated to look at provider and patient experiences with a model, transformation of the health care system, and a model’s impact on spending and quality of care. In this session, we will cover findings from the first two years of the PCF Model (January 2021 - December 2022) in the Second Annual Report. Learn about how practices use model funds to initiate care delivery changes, aim to reduce unnecessary hospitalizations, and how practices’ prior experiences and affiliations aided this process.
