What about when programmers or those in the trenches have better ideas which project managers and stakeholders do not? Is this part of iterative planning--or discovery as the project unfolds? Or is it more upfront?

Here's a critical point to remember throughout all these processes. You must include programmers or those in the trenches into all of them. As a project manager, you create a framework and provide processes and tools. But you don NOT fill them out on your own. It's always back and forth between stakeholders and project team (or pre-assigned people, or SME you can reach out to).
