You missed a non standard Game Over in Minish Cap. You can get Game Over without Link unaliving if you let Vaati unalive Zelda. There was also the one in Twilight Princess if wolf Link didn’t get out of the exploding shed fast enough

The train game over screen still haunts me, especially the dark train version. Zelda screaming out in horror, the whole thing bursting into flames as the camera just holds the shot, it's pretty heartbreaking when you think that Link either died in the initial explosion or got knocked out and suffocated in the smoke. Even worse when you're carrying passengers and you wreck the train, there's unique dialogue for each one and some of them can be quite sad.

Most of the 2D Zelda games (besides for Adventure of Link, Phantom Hourglass and Spirit Tracks) involves spinning around in circles and collapsing starting in Link to the Past. In the first game, Link instead grays out and vanishes.
