The hidden sand does things differently than the hidden leaf. In the hidden leaf, the next hokeage is specifically chosen by the current hokeage. They don't have to be blood related. In the hidden sand the Title of kazekage is typically inherited by a relative of the current kazekage when they die. That wasnt the case with Rasa though, because the 3rd kazekage died without any known relatives. Before Gaara became Kazekage, the elder council wanted kankuro to become the next one, since he is Rasa's oldest son. But he didn't want the Title, & gaara proved himself to be a great leader. In boruto, temari has a son with shikamaru, who is in line to inherit the Kazekage title after Gaara. But the elder council doesn't want him to because he was born & raised in the hidden leaf. It never made it into the anime, but the elder council actually arranged for Gaara to be married. So he could have heirs. But it didn't work out with the woman they chose. They were gonna arrange for him to marry a different woman instead, but he met & adopted shinki. & the elder council have accepted shinki as Gaara's heir.
