Good stuff guys. This can be really challenging to many stroke patients with hemiplegia. You mentioned the quality of movement, which is really think is important with the above the head activity. Not that I'm a big believer in the Bobath concept, but good shoulder stability and scapula mechanics are crucial to doing stuff in an open chain over head, which very few stroke patients are. Therefore I would not start of with stuff above the head, but first create humerus and scapula stability in positions where the patient is in less risk of rotator cuff impingement.

I think they mention that these exercises are for the higher level CVA (or maybe it was another video of theirs that said so.) Anyway, stability of the joints is always very important in the beginning. Maybe forever depending on the level of debility the CVA caused.

You can also use a speed bag for stroke rehab and CIT (Seattle Children's Hospital does this..). A speed bag can easily be adapted to use here. You don't have to stand up to hit it, it works perfectly well sitting down. You don't have to hit it like a boxer on three rebounds (repetitive punching from the front), you can use Five rebounds, or no rebounds. Just hit it, stop the bag, target the punch and hit it again. No matter what speed or power they have, the bag will move and they will see and hear the results instantly. You can work the biceps by doing reverse punching, (hitting the back of the bag and punching toward your face. Don't lean in and bag will not hit face. I have a video DVD "Rhythm and Rehab" to explain this. PM on yt or go to speedbagcentral - I'll send you guys the DVD. Your patients will LOVE it. ps: the auditory cueing of the repetitive bag beat can help entrain the sound with their movement. External auditory cueing has been found useful in stroke rehab. Great video, thanks for posting.

Thank you guys.My Sister suffered a stroke in November 2014.She has come along way at rehab,and using the exercises you teach.The ''chair sitting and standing'' exercise really helped,as she is strong enough now to get up off her seat without my help.Where can I purchase those cones?I know you said you can use cups.The only plastic ones i have are red solo cups which are flat on the bottom.

I had a stroke in 1972. My arm and hand is in flexed( not sure what to call it) mode. Fingers curled and arm bent. Is there an exercise to help straighten fingers to use them.

Could you please provide advice when you are stuck inside during a pandemic and perhaps you are attempting to start getting your movement back alone. For instance, how about can I use items around the house. I'm guessing I can do this but it's nice to hear a professional suggest it, then just assume and do it. Also, how many reps. How many times per day.

my father had a stroke 15 days ago. His right side(hand and leg) is paralyzed and he can not speak. we are trying our best that he could recover as soon as possible. there is some movement in his leg but his hand is completely still. it would be very generous if you give some tips. And also how long it will take for him to walk again. Thanks.

I have handslint. I am alone so getting arm and /or hand into these is a major effort.
@PineIslandSeeds
@PineIslandSeeds 8 жыл бұрын
It is easier to put on the splint if you sit up at a table, very close to the table with your affected arm very close to your side or against a wall. Or just roll your wheelchair up against the wall parallel to it to stabilize it, if you have a wheelchair. If you have peripheral vision issues on the affected side make sure you turn your head to that side when putting on the splint. It helps you see what your doing better. It is very difficult to put on a splint by yourself while in bed or sitting edge of bed. Best of luck to you and keep workinghard!

Great videos and I really appreciate it. I'm a physical therapist also and as of now im handling a stroke patient which main problem was wrist and digit control. The rest of the UE strength was good already that patient can reach an object, but was unable to open the hands due to beginning spasticity of hand flexors. Patient is on 2nd month post stroke. I would appreciate greatly your suggestions. Thank you

I a total "Leaner" now. I not notice, but PT ask me why I lean to left side in wheelchair... How we Learn to Not do that ?? I think I do because my body SO tired all time. I have Left side stroke in 2013. Then 3 more since, all Left side Stroke.... Effect speech and right side body.

Hi, Lisa. I'm an OT. Repetition of the exercises you have learned in OT, PT and Speech is very important because that is the only way the brain relearns that you still have a right side of your body. There have been studies showing you can get return up to 2 years, but I tell patients to keep up their exercise program because of the repetition needed to retrain the nervous system. It takes a long time for nerves to heal...and if/when they do it may be limited. Nothing is written in stone that movement will come back but it is almost guaranteed that if you do nothing you will become weaker, more tired, have an increased chance of contractures (a permanent tightness in the joint where you can't fully move the joint), increased pain, decreased ROM (range of motion) and hygiene/breakdown issues, especially around the web spaces of your right hand, making it smell "cheesy", which is really bacteria that can cause breakdown of the skin and, if left untreated or if poor or no hand hygiene is performed can cause painful ulcers. If you were given a splint by your OT follow his/her instructions for the wearing schedule that he/she gave you and watch for redness or pressure areas, which are an indication that the splint is not fitting well and may need to be adjusted. See your neurologist as she/he instructs. I really have hope for stem cell research and, if it has a good efficacy rate in the future, hope it can eventually become more available and affordable for people suffering from all types of diseases.

P.S. - By the way, it is great you can type and decipher words. Many people who have had strokes cannot even do that. I may ask them to write an "A" and it comes out like an R, or some other letter. It is great your can express yourself through writing. I am imagining it may be difficult to speak like you used to before the strokes, though.
