Disabled persons may lack the ability to provide care su ch as this to themselves. They may even lack the ability to hire someone who can provide this for them. No insurance or minimal insurance could also limit someone’s access to healthcare. Maybe this individual is suffering from depression or another mental illness and struggles to fulfill their most basic daily needs such as food, water and sleep. It’s not always a matter of LETTING this happen. Any number of factors can influence the care someone is or is not able to receive. Those of us who aren’t able to understand how someone could “let” themselves get this way are quite likely privileged and/or fortunate enough to have such a lack of understanding. Not everyone comes from the same circumstance and that’s perfectly okay. Just a little empathy and understanding can often make all the difference.

My dad produced wax like crazy. He had his ears cleaned every six months.

Every time a patient comes to the Dr with loss of hearing. The nurse is the first contact. Any nurse worth her salt would check the ear before the Dr comes in, she would chart her findings. People will go 50 or 60 years without any problems and then one day all of a sudden their hearing would be greatly affected. The Dr would usually say you have a wax build up. They tell you to use debrox for several days to soften the wax, and then they come in to have it manually removed.

Just as your nose hairs prevent stuff from going up your nose. Ear hairs do the same thing. There is one exception to the rule. When these older guys go in to LTC facilities, when doing nail care or shaving I would trim their eye brows, nose and ear hairs as well. There's no need for 2" nose and ear hairs.
