I remember this, watching this live. Faulty wiring was the reason for the signals giving wrong aspects and Anthony Hidden QC wrote an in depth report. Rail workers of all grades adhere to Hidden 18 rules which states that they cannot work long than 13 days, no more than 72 hours in a week and no more than 12 hour shifts each day. Seems the railways always wait for an accident to happen before implementing safety measures.

At least half the clips of trains in this video are either from lines away from the accident or rolling stock that has been introduced since 1988.

Many points were missed. Like the hidden rule that all railway workers have to follow or that it was reported that the technician who they reference to have caused the wiring error had reportedly worked 7 weeks in a row straight!!!! Or the fact that driver who worked around the Clapham Junction area would regularly disregard the rule book requirements for them to report any signal irregularities like a signal turning from green to red straight in front of the them!!!! Many would not report these incidents as it became so common in that area.

“This was the days before mobile phones” Yeah, trust me…if a driver wants to keep his job one of the last things he wants to do is turn his mobile phone on in the cab with the desk keyed in.

Maybe he was just a passenger with limited rail knowledge. 'Cab to shore' radio might be a more appropriate communication.

It seems to be the way we do things. The money is only spent for major upgrades and improvements only after a major disaster.

The work the technician was carrying out to the existing signalling circuits was a small part of a series of preparation works for forthcoming upgrades at that time.

Yep. You're right. Look at later rail accidents - Hatfield, Potters Bar, Ladbroke Grove. We never seem to learn the lessons.

rubbish report the signals changed aspects thats why the driver stopped and reported a signal irregularly

@@ChangesOneTim Couldn't even get the facts correctly right from the start of the programme. The driver didn't stop because he saw a red signal; he stopped because he saw a signal at green go straight to red. This should be impossible, so the driver knew there was a serious signal fault and correctly stopped to report the failure to the signaller in accordance with the rule book. If he had only seen a red, he would have simply stopped and waited for the signal to change.
