Ultimately up to the individual; semi formal or formal…could be a pantsuit & blouse, a skirt/shirt, jumpsuit, etc., as long as it looks pleasant & neat.

If I am asked on my I-20 if I have applied for a visa before and the answer is yes. Will it affect me on my day of interview?

You mean on the ds160 form? If you’ve applied for a visa before and you’re asked such question, you should answer truthfully. As long as you prepare well and there are no other concerning factors, it should not affect you on the interview day.

The visa officer will know our CGPA are they doesn't know.

They do not automatically have access to your results, so they won’t know unless you tell them.
