What a horrorific situation, Tam! I cannot fathom how an individual with no medical training can have the bravado to provide medical treatment to a patient in distress. In addition to that, the fact that when you discovered that he improperly intubated the patient and wouldn't let you take over the case...what a piece of work! Excellent video as always, Sir!!

I've faced challenges at MSC due to my "chief doctors" speaking negatively behind my back, the funny fact is like only my fellow countrymen did that, triggered by a mix of envy because of I´m younger and I got into thanks to the strict filter nowadays. Can you offer advice on handling this situation, given your cruise ship experience? I would like to consider other cruise companies, but I don´t know which ones. As a doctor you want to stay clear of MSC, Princess, Carnival and especially Virgin Voyages like Superman stays away from Kryptonite.

I fully appreciate this video is about the faker, however just wondering if you had access to EtCO2? May have helped identify the oesophageal intubation? or perhaps iGel first as many ambulance services go for is ideal until in a place of safety?

We did not have a portable capnograph on us at the time. Only had one on our ventilator and Lifepak. LMAs are great for difficult intubations, or people not comfortable with ETs, but not so great for vomiting patients.

@@deck4forward Which is well acknowledged, but surely the lifepak would be there at an arrest? And in terms of outcomes, AIRWAYS2 proved no difference between in regard to neurological outcome... In the case you have shared I'd be surprised if there was a good outcome as likely a long time with suboptimal/no ventilation... surely a small vomiting risk is worth taking? Again, I appreciate this is not the point you were trying to make, but having worked prehospital for 9 years and now in hospital I've seen it from both sides!
