Having blood doesn't necessarily mean a monster has determination ,Most monsters probably have that,or it was just a ketchup bottle The only monsters that ever had determination were the amalgamates and we know how that went ,and Undyne(although it wasn't mentioned if I remember correctly, it was hinted though) ,who was only able to sustain it for a small amount of time before starting to melt,One can say that Undyne would've probably been able to handle it longer if we didn't attack her,and flowey who was revived by it,other than that we haven't seen any monster deal with determination before

Monster body cant handle raw DT even if its the strongest monster (undyne) So if DT is injected to monster body the *WHOLE* body will melt no matter how much the DT is The only Solid DT is the DT souls itself thats why Asriel doesnt melt after absorbing chara's soul But in this case the only Sans's body part that melt is left face so its not DT Afterall only the comic creator know

I have two reason why Sans may be melting 1. Determination This one is highly unlikely due to the fact that Sans never has been shown to be determined at all. But there is a chance since determination does cause monsters to melt. It could’ve been that he was injected with determination before, and has been able to keep it from melting him (like Undyne the Undying) till now (may explain why he so strong). But it’s highly unlikely that’s possible. 2. Fallen Down This is a condition where monsters are either too old or their soul or magic is too weak to sustain them (like old age or organ failure) so the monster slowly starts to dust away (or melt) till they are no more, this was hinted at in the original game where Alphys used determination to help monsters that were “fallen down”, and also the title is a great giveaway. and considering Sans has been around for who knows how long, and also the fact that Sans only has 1hp and 1Def, he most likely will get the illness.

I might have a theory... Mabye the power of Sans' left eye has been leaving side effects. See how his right eye is melting. His left eye is the same eye that holds the glowing blue/yellow eye. Mabye, that "eye" of his has been leaving small fragments of determination. Which we all know, monsters cant hold properly. So thats my theory of why sans' face is melting.
