I am a ride-or-die believer that the * in Thunderbolts means alternate universe. It would be the easiest way to explain where Sentry has been this whole time, why Bucky is a Congressman who is shooting at his own teammates, why Val has white hair, and most importantly, why the whole cast is confirmed to be in Doomsday and Secret Wars. What stake would this group of bad guys, half of which have enhanced abilities, have against the multiversal war and Doom?

That would be literally a waste of all the character development that all of these characters in the main MCU has had. It's one thing for the F4 to be alternate universe, since we don't have versions of them in the main MCU, but all of the Thunderbolts characters have history in the main MCU, to just make a random elseworld version of them instead of exploring the characters that already exist in the main MCU would be illogical. And also, there are plenty of easy ways to explain where Sentry has been (he was just made), why Bucky is a congressman shooting at his own teammates (they aren't a team yet), why Val has white hair (she decided to change hair color), and why the whole cast is in Doomsday and Secret Wars (because they are a main hero team that will be existing when all the multiversal events are happening).

Regretfully if you take a step outside, F4 is not exactly as hyped as it seems. Most likely it went through so many iterations, and Reed Richards is a very one dimensional character. He is just Smart. And Disney sucks so bad at writing smart people. I personally love the cartoons during 80s and 90s, but current day and age? no, Mr. Fantastic just don't gives out confident to put up a good show.
