When the first officer's air speed indicator was relied upon for a successful takeoff then logically it should've been relied upon when the confusion started.

There is also a third which is a mechanical instrument not from the pitot tube which would’ve also been correct

To be fair, who remembers details like that the auto pilot uses the captain's indicator? That can be forgiven, but not reacting to a stick shaker and pulling the yoke further back and leaving power at idle, that's unforgivable.

Exactly, I don't understand why they didn't trust the FO's instruments...they were correct the entire time.. when the stick shaker activated and the FO's instruments showed their speed to low they should have been able to figure it out.. instead they literally allowed the plane to fall out of the sky...

I still blame the pilot captain 1) Took off with key instrument failure, and didn't suggest return 2) Relied on Autopilot despite knowing failure 3) Never made changes to airplane attitude/ lack of speed 4) Didn't listen to suggestions & recommendations

1. Not an issue, there are three airspeed, horizon and altitude gauges. You use the third one to determine which is correct and use that one, or in the case of complete failure as they are not electronic. 2. Depends on the autopilot. It was fine until he screwed with it. 3. He did. Plane computers were displaying overspend warnings. Which were of course erroneous. 4. This one you are correct on. But not all his fault. Any one of them should have checked that third indicator, and it is baffling that NONE of them did.

​@@Demonslayer20111the autopilot was not fine until he screwed with it. The autopilot uses the captains gauges pay attention to the vid. They had to fly manual whole way through to have any chance

@@Tightness8 the autopilot will rely on whatever data you tell it to rely on. That's what flight directors and source switches are for. No aircraft in commercial service has an autopilot that relies on a single data point, that would violate redundancy regulations

This is probably the most frustrating episode of Mayday for me. This was so easy to avoid. The crew knew that the First Officer's air speed indicator worked. Then, as soon as they got the stick shaker, they should have throttled up and put the nose down....problem solved.

I agree that this episode was frustrating. I was just thinking, ¨ this is the most heartbreaking episode I have watched." It said that at first the captain saw that the first officer´s speed instrument was working but later he thought it was not working. I find the fact that the first officer did not take over most disturbing. I think that if people around the world are going to fly on airplanes from around the world training needs to be standardized for all pilots. Maybe a force that has international scope and jurisdiction needs to be formed that all pilots and airlines must answer to. Just so sad.

They could have and should aborted the take off. Unbelievable that they then continuously made so many other mistakes. Ignored a problem at take off then ignored the stuck shaker. So frustrating

Had a co-worker who was flying back to the US from Belgium. His flight ran late and when he landed in London he wasn't able to get to his flight. He was incredibly upset since that would mean he would have to stay overnight and catch another flight in the morning. When he woke up he turned on the tv and saw that the flight he missed - Pan Am Flight 103 - had exploded over Lockerbie Scotland.

The captain was clearly dazed and confused and his brain was no longer working. This is panic and the worst thing a pilot can suffer. Training and experience can mitigate this type of panic, but nothing can substitute for a cool headed pilot. 🌵🌵🌵

Captain relied on the FO's instrument to reach V1, but during the most critical part of the flight he ignored them, and the excellent advice he was getting from the other members of the flight crew.

There's a third IAS, indicated airspeed, gauge in the middle of the console, a long with an altimeter and artificial horizon. All airliners have it. It runs on a separate pitot system, and should be used to verify which instruments are correct in the event of conflicting info. They didn't do that.

@@Demonslayer20111 this is also one of those cases where seniority was an issue because unlike american crews this was a turkish crew and their culture is different the captain doesn't rely on his co pilot like americans do and the first officer in this case had better situational awareness but was hesitant to take control

@@cigarsgunsanddiesel8032 actually the tower doesn't easily have that ability , the information displayed on their screen is actually transmitted to them by the airplane ,ATC uses secondary radar to track the airplane ,,sent by the IFF transmitter they don't use skin paint to track them known as primary radar ,, the lack of communication on the pilot's part was the main cause of this crash because he knew his instrument was faulty and didn't keep using the copilot's instrument which was working ,,I've never flown a jetliner but i am an experienced pilot in both single and twin engine aircraft and a failure of my primary airspeed indicator is an immediate abort takeoff ,,or a go-around and land if i've reached the decision point to get the issue fixed

That shouldn't have mattered. All airliners have a third IAS. It's in the middle, and is completely mechanical, as well as a mechanical altimeter and horizon. The bare minimum for instrument flight. Why they didn't compare to that I don't know.

@@Komputar They needed covers over those tubes that were installed immediately after shutdown and maintained. Inspection is visual in daylight after removing the tube covers, flashlight at night. This accident didn’t have to happen.

First of all, the thing that upsets me most about this is that the pilot could have, and should have, stopped this whole thing BEFORE V1! That would have taken care of everything and eliminated the accident! Second, once he realized there was a problem, he could have IMMEDIATELY turned back around and landed. Third, when the stick shaker started, every pilot knows to increase speed and push the nose down! That is 100% a given once that stick shaker goes off! That shaking means you have SECONDS to correct and push that nose down and pick up speed. He didn't do anything you would expect a captain to do to protect himself! The maintenance guys should have checked those pitot tubes, BUT the captain could totally have avoided this crash at multiple points!

For one you dont know that and everyone is great at monday morning quarter back 20 years later including the genius online who claim he knows everything when the video clearly stated that the pilots were confused when it happened in the simulator. I am 100 percent sure that they added a training for the tube problems because when the over speed warning is conflicting with the stick shaker it confused the pilots in the simulator so the nonsense you claimed that every pilot knew whatever you claim IS NONSENSE because the recording stated otherwise!!!!!

You don't know what you are talking about and you didn't pay attention to the details in this video. When there is conflicting information from the instruments (overspeed vs stick shaker) it is impossible to know which one to rely on.

What gets me is that the captain didn't try something different at each turn of events, like when he throttled back the engines and the stick shaker was enabled. To me, that says you should try to reverse the action you just took because clearly it made things worse. Pulling back on the controls wasn't working, so the logical thing to do was to try pushing forward, doesn't matter what horns are blaring, the idea is to make a change, any change. If what you're doing isn't having any effect, TRY something else. They teach pilots that and it's been said in other videos on this channel.

I see that passengers were waiting a long time and it could be possible the pilots wanted to get them to their destination, but this should have been an aborted takeoff in my opinion.

Yeah captain seemed to be too used to trusting instrument and froze when they failed he failed with them and he didn’t take to any attempts at CRM or abort takeoff, throttle up to correct the stick shaker so many things ky feels like doing just 1 thing right woulda maybe saved the plane but he did everything wrong he possibly could.

In a very roundabout way, this type of documentary helps my anxiety. I don’t know if there is anything more daunting than trying to figure out why an airplane accident occurred. Even with black boxes and technology, there are so many pieces that must come together. It helps me to remember to be methodical and logical, and not just jump to conclusions

I’m still curious as to why the maintainers were not questioned more about the covers. If it’s required for the pitot tubes to be covered during extended periods of ground time, why was this aircraft an exception? Or was this common practice for the maintainers?

I don't get the oversight either, it's basic, you learn this in flight school. One of many reasons we do preflight walk arounds, to check for damage, any anomalies etc

currently 21:26 in and I can already tell where this is going. Pitot tubes were obstructed.. wasn't actually over speed. Slowed down too much, stalled the plane and fell out of the sky like a rock. If I'm correct, I'm sending my application into the NTSB tomorrow morning.

so sad, I'm an avionics technician on 757's/ 767's and first thing i screamed out was compare the Capts and FO's airspeeds to the standby airspeed indicator !! See which one is in error....Then bootstrap or select capt's air data source to F/O's air data computer !!! Center Autopilot gets its input from capts air data computer or FO'S if capt's selected to Alt. This is why when I give air data testing training when hooking up pitot static test box you have to have cross bleed valve open if you're bringing up only altitude ( static pressure ) if the pitot doesn't see the same static pressure the airspeed will climb as altitude goes up until it pegs out high and possibly damages air data computer , with the 14.7 psi air trapped in pitot side and thin air on static side ( airpseed indicator sees both pitot and static pressures ) . It's why they had no airpseed in capts side when accelerating on runway but it started working when gaining altitude. This video is a great training aid for maintenance trainees that don't understand this process. Pilots should be trained on these principles too. Also yes as said below pitot covers are a must as wasps or mud dobbers will build nests in the tubes over time and blocking the air passage . Really sad to see such a simple problem lead to this.

I remember this one, dumbest captain I have ever seen on this show. Didnt abort takeoff with an airspeed mismatch. Knew the airspeed indicators were screwed up and put no effort into diagnosing the problem, just turned on autopilot and pretended everything was normal. Then he flys the plane using his airspeed which they 100% knew was broken before they even took off, never attempted using the FO's airspeed, never attempted to get a 3rd reading to compare to. 0 critical thinking just flying on habit, horrible airmanship, even pulled the nose up in a stall.

Your comment is key! He should have aborted AS SOON AS there was an airspeed mismatch, before they reached V1! That would have avoided everything. Then, if he saw the issue once they were airborne, he could have easily turned right back around and come back, instead of going out over the water.

Shouldn't an experienced pilot have an idea how fast the plane is moving by the position of the throttles? I get it, head winds, tail winds but, if throttles are at idle they must be going too slow, this plane just dropped once going to slow, why wait so long to throttle up?

The way to get out of a stall is not only to increase throttle but to lower the angle of attitude of the nose in order to get put of a stall so you are partially correct 🙂. This restores the lift on the wings which allows the plane to fly and to continue to regain airspeed until a safe speed is attained and then attempt to resume climbing as you do lose airspeed when climbing due to gravity

With regards to the throttles it’s not necessarily that simple. A plane could be in a dive with speed influenced influenced by natural force while the throttles are at idle. A plane could be be slowing down with its throttles all the way up because it’s pitched up at an altitude too high for it’s weight. Pilots are also usually trained to trust their instruments, especially in low-visibility conditions such as flying at night. Keep in mind the instrumentation was persistently telling the captain the plane was moving too fast just before it did a complete 180 and engaged the stick shaker. It’s reasonable to be confused in the moment, at least regarding airspeed. That being said, what the captain also unfortunately did was allow his confusion to cause him to panic as regardless of the conflicting feedback regarding the airspeed, he neglected to correct the trim even as his two other co-pilots were trying to draw his attention to the plane’s attitude (something arguably as important as airspeed in an aerodynamic stall situation). That’s just evident the captain in his panic wasn’t thinking clearly and lost critical awareness of the plane. Combined with the decision to even takeoff in the first place knowing his ASI was malfunctioning that’s just poor airmanship. The only thing which may have saved that plane in that moment of stall was the first officer prioritizing the value of his own life enough to take control of the plane from an impaired captain and do what he evidently knew needed to be done.

Things haven’t changed much. You don’t question a senior person in Turkey no matter what profession you are in. The captain was clearly overwhelmed and not thinking. He literally froze which is rather unusual considering his experience. He couldn’t have seen that the pitot tubes were covered but there is another sped indicator on the console inside the cockpit that runs on a completely autonomous system. He could and should have relied on that at least given it a glimpse. Nobody knows why the captain froze. I truly believe that there are circumstances that started before the flight that lead to his lack of reaction. Maybe it was a lack of sleep, argument with someone. Yes, none of those should- in the best case scenario- cause such devastating consequences but it’s never one thing that is the reason of a crash but a chain of small little things that lead to the loss of human lives. It’s a cultural thing but even in Western countries a co pilot would hesitate to take full control over a captain.

@@Demonslayer20111 - the stick shaker operates off the flight control systems, NOT the gauges!! The stick shaker activated FOR A REASON!! They were going too slow!! After the stick shaker activated, the captain should have pushed the yolk forward, while increasing power to the engines! Instead, he made it worse by pulling back!! By the time he pushed the throttles to full power, it was too late! The plane was uncontrollable!

If a pitot tube is blocked, it becomes an altimeter. As you climb the air inside expands and acts like the plane is accelerating even though it showed you werent moving at all as you took off. They used to teach this in Airplane Driving 101. Bugs LOVE to build nests in pitot tubes, thats why those "remove before flight" covers were invented.. I used to wash airplanes in high school. We would tape over the pitots and the static air vent. We made VERY sure to take the tape back off when we were done.

I live in Georgia and the GA airport where I live. The hangers and airplane parking is surrounded by woods. (Pine trees mostly. Dirt Dobbers love pine) If you don’t have covers on your pitot tube or tubes. Dirt Dobbers will get in them and build nests. MAN! is it ever a pain in the A$$ to clean one up after getting them out.

When you step back and look at the culture of "revere and respect your elders", you realize the absurdity of it. We are all just monkeys who get 100 years if we are lucky on a planet that is 4.5 billion years old. You telling me "I have 30 years experience, so do what I say," is objectively laughable. Once they realize that the Capt and the FO airspeed was faulty and showed different readings after takeoff. They should’ve communicated to ATC to turn back and land immediately.

Why did the pilots continue their takeoff roll when they knew something wasn't right? I mean, they had every opportunity to abort takeoff and they took off anyway, even with their incorrect readings at their sight. What a shame...

Get there-itis...I noted there was a 4-hour delay beforehand. A monumental waste, yet incomparable to their lives. Unfortunately, it is a common factor for most crashes.

It’s too bad that when the captain took off, he didn’t have somebody out there on the runway with a tape measure that could’ve told him that it was OK you have time to stop. This is something that will always be a problem with flying. Unfortunately all humans are susceptible to making errors and mistakes or miscalculations. So we probably won’t see an end to these types of occurrences for the foreseeable future.

The captain is absolutely to blame for this crash. Sure the tubes should have been covered, the takeoff should have been aborted, but even so, he failed flying 101. The plane told him they were stalling and the first reaction to that should be nose down. Apart from that. It seems idiotic to me to have a plane with 5 ways to measure air speed but only 1 of them being used as the input for the autopilot. I could write perhaps 20 lines of code which could handle all 5, filter out any that deviated significantly from the others and use the average of the remaining, apparently that is too complicated for the people at Boeing.

There are two autopilot systems. One uses the captain's instruments, the other uses the copilots. They could have switched, but that captian had the worst kind of tunnel vision. Saw his airspeed and couldn't comprehend anything else at all.

What annoys me about this episode is there is no criticism of the airport maintenance for leaving the pitot tube exposed without a cover for weeks. They do what's required and this crash most likely doesn't happen. At least not for reason of air speed misreading.

Even with conflicting warnings, knowing that I have one airspeed indicator showing too slow and one showing too fast, I feel like anyone should know which one is true when the stick shaker went off.

Senior pilots should be pretty smart guys to. How the heck was this not figured out quick. This is scary. Hope pilots have had more training then this. Only God know when the end is. Still unbelievable

The captain's brain completely shut down. He allowed his confusion and fear to influence his judgment from jump. He didn't abort the flight when he first discovered his equipment wasn't working properly.

It might have been scary going down but they wouldn't have felt anything when they hit.. it woulda been over instantly, id rather go like that holding on to someone i love or even the guy next to me, than alone old and helpless. It's still sad for the families but at least they know their loved ones didn't suffer.

It baffles me when co-pilots let a plane crash than take control from the Captain. CRM shouldn’t have had to be introduced for that to happen. The CP is also responsible for passenger lives.

Talk about an amazing set of circumstances that led to this .. the pilot could have aborted before V1 especially when he knew his airspeed indicators were malfunctioning. And then it was a solitary wasp that likely built a nest in the pilot tube because it was left uncovered for a few days ... at least they can now turn the pilot tubes on and off to get one that is working.

Yes we have the mud daubers here in Texas as well and they do plug holes everywhere. Air conditioning drains, vent lines, etc.

All the pilot had to do was lower nose and put the thrusters in Climb mode. If i am in a plane and joystick starts shaking you know is a stall. The speed clock can fail, the joy stick raddle is always 100% correct

Clearly the problematic air speed indicator tube could be redesigned. It doesn't take a engineer to realize it will happen again. Anything could cause a blockage.

Maybe but probably not. You just need much better mechanics. Any mechanic who doesn't know the importance of pitot tube covers MUST NOT be in charge of aircraft maintenance.

Plane sits there for three weeks, and peto tube blockage isn't literally the very first thing you think of?? I'm not an aviator at all, and that's the first thing I thought of, having heard similar stories in the past. I'm amazed it wasn't the first thing that came to mind knowing the plane had been sitting there that long.

I was taught that a blocked pitot tube will cause the airspeed indicator to act as an altimeter, showing the reverse of what is correct. Even knowing that, it can be confusing. I was told that if that happens, cover it with a post it note so it doesn’t continue to trick you. To be fair, I don’t fly big planes, only little ones.

Flight 626 had only 1 survivor. That’s the only one I know about in terms of an uncontrolled crash. There have been survivors of very hard, somewhat controlled ocean landings though.

It's always easy to second guess someone's action(s), or inaction, when not in that unbelievably stressful situation. However, at least as presented, the captain appeared to totally ignore his AH (artificial horizon), which, at that point, was his most important instrument, and it was showing that the plane was in a dangerously nose-high attitude, which obviously caused the stall and subsequent _graveyard spiral_, which would have been unrecoverable at that altitude, if at all. Even after ignoring the attitude indicator, once the stick - shaker activated the captain should have known to immediately lower the nose and add power, while making sure the wings were level, which would have required, of course, looking at his artificial horizon! I would have to believe that an experienced pilot, that didn't panic, would have been able to use his/her attitude indicator, obtain straight and level flight, and make it back to the airport, even if they weren't sure exactly what the problem was at that point, yet their simulator testing seemed to show differently. The number one priority in aviation is always - *FLY THE AIRPLANE.* Truthfully, the captain became little more than a bewildered passenger. What few actions he did take, were wrong. Sadly, this could have easily almost been a 'non-event.' Just a return flight to the airport for maintenance to locate the problem. Of course, the approach and landing with no airspeed indicator would have been interesting, to say the least. I do believe with a little more time they would have figured out that they merely had a single erroneous airspeed indication, however. Once they knew the problem, there would have been no problem. If anyone is new to these videos, lookup the crash of Eastern Flight 401 (Florida Everglades). Where 101 people lost their lives due to the entire flight crew focusing on a $12 burned-out light bulb, allowing the plane to fly itself into the ground. The flight crew went the final four minutes of the flight without bothering to check the flight instruments, *assuming* the autopilot had everything under control.............it didn't.

In my mind, if I think I'm going to fast and I slow down, then stick shaker goes off, I then increase power back to where it was. At which point autopilot is turned off because of the stick shaker event (should be recognized it's off), I know I'm not traveling too fast at that point then I pitch nose down. After alarms stop, then I try to figure out what is going on

I agree with the other commentators. The captain was incompetent and negligent. Everyone is taught to abort a flight if instrument failure is suspected. It's better to have angry, irate passengers than dead ones.

Now when I watch this episodes I wait for "Terrain, pull up!" and the stickshaker rattling for aerodynamic stall, and I just go Oh no! because I know the pilots will probably assume it's an instrument error. You can do something dangerous so long you forget. Like the guy who jumped out of a skydiving plane hundreds of times until one time he jumped with his camera bag rather than parachute unwittingly. RIP.

If you know you pulled power back…why pull power back more? You know the only way you have flight is with adequate power. I’ve rather fly a little too fast than too slow - wish they were able to save the situation

I do not understand this and it’s happened more than once. Wouldn’t some basic understanding of dynamics give you a clue? Look at the throttle settings, look at your angle, how could you be magically overspeeeding? And you have another indicator telling you your performance matches your configuration. Why would you go with the data that is obviously an outlier???

when I first watched this episode and they said what blocked the pitot tube, my husband and I both looked at each other and he said "I didn't know that's where the name came from." there's a character on King of the Hill named "Mud Dauber".
