A rather disquieting crash, that one was. To have an urgent medical issue combined with poor weather and worse, a faulty autothrottle system is among the worst cases ANY flight crew could possibly have. Nice work on the animations again, Allec, and I send my condolences to both the victims of TAROM Flight 371 and those affected by the Taal Volcano eruption. Seems like a more logical response to losing his captain would have been to call a flight attendant to deal with that emergency rather than forget he had just become 100% responsible for flying the aircraft alone. He could have landed the plane single-handed and been a hero.

Hs Hs Shouldn’t one of the recommendations as a result of this crash be, better training for recovering from unusual attitudes when flying in IMC conditions as this was a TOTALLY PREVENTABLE accident. Even with the incapacitation of a pilot and auto throttle malfunction this accident was virtually 100% preventable. Simply if the co-pilot had rolled the plane back to straight and level when he saw the steep bank, the plane likely would have made it back to Bucharest. But no, the co-pilot didn’t believe what he was seeing on his instruments was correct and did nothing, Allowing the plane to roll 170 degrees to the left and reach 60 degrees nose down. That’s Pilot Error right there. The French BEA audited the Romanian investigators report and stated that the primary cause was the inadequate response of the co-pilot. I talked to a former 747 Captain about this crash and he told me that the co-pilot should have gotten the cabin crew in their to attend with the Captain’s body. Thank you

You MUST be committed to believing your instruments unless you have clear & present reasons to think they're malfunctioning. I feel sorry for this first officer having three emergencies to deal with simultaneously, but falling back on the most basic advice for all pilots to prioritize their attention -- aviate, navigate, communicate -- would have saved the day.

No excuse. His responsibility is to the souls on board. An ability to fly the plane in the face of extreme adversity is the prerequisite for having the job. He had two working engines and a mechanically functioning plane. Sully had no engines and managed to preserve life. This guy stopped flying the plane and 60 people died as a result, as confirmed by the accident report. I know what you're saying and I feel for him, but nowhere near as much as I feel for the people who had no control over their destiny and lost their lives to his negligence.

Indi Newall What you’re missing is the fact that there was a fault with the plane that was not fixed. Sully knew he had no engines, this guy did not know that his plane would randomly throttle back at the worst possible moment. Then Airbus waits another 6 months to fix it and the fix doesn’t even become mandatory for another year? Pretty obvious where the blame lies. Money over safety. Not much different than the 737 Max situation. You blame those pilots, too?

HeavyBrocks hi mate, I didn't miss that, as with all air crashes it's usually a case of a perfect storm of several things that all needed to happen to bring the bird down. The fault should have been fixed, but the evidence that the fault was occurring was plain to see for anyone properly focused on flying the plane. The plane was aerodynamically sound so the situation was totally, and easily recoverable. I don't agree with your equating of this with the MAX incidents. In those instances the aeroplane actually resisted the pilots attempts to right the craft. In this case the attempt simply wasn't made- he dropped the ball. All compassion for him, but this is what occurred.

My sister was an air hostess, she saved the Captain's life when he suffered an heart attack on flight. The first officer took control and landed safely. The crew very happy to land safely and the captain was alive. Of course, the airline did not care that my sister and the first officer were heroes.

Seems this is the never-ending legacy of commercial aviation safety: a known and potentially disastrous technical problem isn't thoroughly dealt with until dozens or hundreds die in a completely avoidable accident. :-/ Thanks for another well-crafted and informative simulation, Allec.

Actually the problem is with air bus because they did not design a fix until after the crash. Remember they issued a Mandatory service bulletin after the crash.

His job, SOLE job, was controlling the aircraft. Help for the pilot MUST wait until the aircraft is at a safe altitude in a safe configuration. He's hand-flying in clouds and let his attention lapse. ERROR!

Hello Allec. Nice video, as always!. Actualy, there is another side story of this accident, which began in late december 1989, 28th if i remember right. One of the F/A of this flight was Camelia Jucovan who lost her life in this crash. Her husbant was also pilot for TAROM (national airline from RO). On december 28th, right after the revolution, airspace was closed for commercial flights. But due to crissis and need for blood in hospitals, an international flight bound for Belgrad was authorized, with crew of voluntiers. Pilot Walter Jucovan offered as voluntier for this flight (he was previous banned from international routes because of his brother fleed in west), among with other 6 crew members and one foreign reporter. Their flight was brought down by mistake (or not) by flake near Visina village, DB county. After Camelia and Walter remained a boy who was raised by his grants. Around the age of 14, boy climbed on a carriage of an electrified railway when an electric discharge took place, killing him instantly.

A310 is known for its very powerful engines. With only one engine on climb power and aircraft is not so heavy, that bank caused by Asymmetric trust probably happened very Fast. Fo tried to put AP but because aircraft wasnt trimmed properly, autopilot couldnt handle the situation. And he was also looked if captain is Okay, but once aircraft is in such bank at a very low altitude, he couldnt save the plane. Yes pilots responsibility is always fly the airplane but in this situation, noone can hardly blame first officer, this accident was almost inevaidable. RIP all who passed away.

The first rule my first flight instructor taught me at the very beginning of my aviation career, “Fly The Plane”. That is especially true in emergencies when you are likely to be excited and distracted. This is a horrific example of why.

"Since then, there had been no reported cases of thrust asymmetry on the Airbus A310." Not true. There was another case of thrust asymmetry on the A310 when on July 9, 2006, S7 Airlines Flight 778 overshot the runway during landing at Irkutsk Airport, killing 125 of the 203 people on board. Unlike flight 371, flight 778 did not experience a mechanical failure and the cause of the crash was pilot error; the captain accidentally caused the left engine to go into takeoff mode. The reverse on the left engine was not activated, as it was disabled. The reverse on the right engine was applied as required, but with the left engine generating power, a thrust asymmetry occurred and the aircraft overshot the runway. It's a precursor to TAM flight 3054.

Guys: aviate, navigate, comunicate. Always make sure the aircraft is under control, speed, attitude... Use automation but always verify autopilot and autothrottle or autothrust are doing what expected. Only then take care of other issues. Or it might get ugly. Read FMA!

Here in Romania there is quite a controversy about this crash in people's mind... This is the reason that much information about this accident is hidden from public eye.... Many say it was in fact a sabotage due to some high profile passengers that were uncomfortable to the regime , and many whitnesses say they saw the plane explode in a fire ball before falling to pieces down to ground, and that it was really a bomb and the pilot's death was a "convenient" explanation for its crash in the official inquiery. In a private reportage made with anonimate cover, Some workers claimed they had found some parts of the plane's interior furniture on the landing strip, and for those parts to get that far away from the place where the plane actually crashed, it would support the theory about the plane exploding in the air prior to the crash... We may never know the truth... I was really curious how this event is officially seen abroad, because here in Romania we never heard an actual credible solid explanation for it. Even seeing it mentioned here is a big thing for me at least 😁

I flew with Tarom in 1990. It was the only flight that has ever truly scared me. The craft was literally shaking and felt as if it was about to break up. The Romanian people had endured 25 long years of savage austerity and deplorable, inhumane totalitarianism. It was effectively a third world country. They would face a further five years of the same, despite a courageous uprising.

In aviation we say: Aviate Navigate Communicate. Never stop flying the airplane regardless what happens. That is always no1. A pilot with that experience should easily be able to fly straight and level, disconnect the auto throttles if they are acting up and set a trust setting that result in a safe speed for either slats extended or retracted. Once satisfied its all safe even if manual flight, with 1 simple push button contact the flight attendant to enter the cockpit to attend to the other pilot. Its not that difficult. We do it regularly in simulators.

That's why you don't just put a card in the cockpit when a flight control system is not functioning properly - especially something as critical as throttle management during climbout. You ground the birds and fix them. Having to manually overcome a faulty flight system as an SOP is no bueno.

I have these unanswerable questions: 1. Was the F.O even aware of the autothrottle issue? -*why would he engage autopilot if he was?* 2. Does/did this F.O. know the common rule that "flying the plane comes first"? -*If he did, then, why didnt he summon a flight attendent or a steward/ess to the cockpit?* 3.How good was this captain's health predating this accident? -*I personally think that this 'faint' he probably did was his first & last 'faint*

Three rules created by WWII fighter pilots seems to have been forgotten in this accident. AVIATE, NAVIGATE and then COMMUNICATE! The CP should have kept to HIS job appointed at the time... fly the damn plane. Trying to help the captain was NOT the job of the co-pilot. The co-pilot's mistake caused the deaths of all onboard. Aviate, navigate then... and only then... communicate. This aircraft had at least five other crew members working the back that he SHOULD HAVE called to the cockpit to check on the pilot while he continued to fly manually. WHAT WAS THE CO-PILOT THINKING!?!

Please don't kill the messenger, but it is appropriate to say that the rule of "No matter what happens, fly the plane" fits this scenario. While I completely appreciate the likelihood that the co-pilot may have been concerned with the captains condition, he needed to focus on the flying and perhaps call another crew member to attend to the captain. And, by the way, I am a pilot.

Two unrelated issues (auto throttle and the captain's heart attack), neither of which would have necessarily crashed the plane, conspired together to cause this tragedy. So many crashes are because of two or three issues that happen at the same time.

Rule #1... Fly the plane... failure to do so can result in catastrophe the CoPilot quit flying!!! On takeoff if the door opens leave it fly the plane the door can wait ask me how I know...etc etc...

In the late 1990s there was a book published about airline safety called "The Tombstone Imperative". The author proposed that (a) a human life has a monetary value, and (b) regulators, manufacturers and airlines only make improvements to safety when there are enough deaths (and lawsuits) to justify the expense. I don't remember if this very sad accident was in the book, but it seems like a classic example.

Such a horrible situation. Instead of a reminder card fix the issue people's lives are at stake. A helpless feeling seeing that plane going down and nothing passengers can do about it. It's heartbreaking for passengers and crew

First Officer totally forgot what he was there for. Fly the airplane, analyze the situation, take corrective action. He was poorly trained.

The co-pilot had plenty of time to recover the aircraft AFTER he became aware of the dangerous bank angle and all the bells and whistles started going off in the cockpit. What he did immediately next was to push the stick LEFT instead of RIGHT, resulting in the airplane quickly turning upside down and going in a 80 degree negative pitch angle. Basically he couldn't tell left from right and one theory is that he misinterpreted the reading of the artificial horizon. He had most of his hours on IL-18 and IL-62 where the artificial horizon works in reverse. This is also written in the official report. It was pilot error and the rest are contributing factors at most.

What was on the CVR after the captain fell ill, or passed away? Surely the F/O would not have been silent? Why is it conjecture that he was trying to find out what happened to the captain?

Maybe it just me, but it seems that the more automated an aircraft is the more difficult it is for the pilots to respond to an emergency, but then again, I have only flown light aircraft with no automation.

I agree Kent..automation and greed and hubris that fuels it has caused alot of needless deaths...less is more alot of the time...Flown light aircraft... only soloed a few times but....been a student of aviation more or less for 35 years...mostly armchair but....this is sad one...Tarom must have been sued extensively for the auto-throttle idiocy...very disturbing and difficult recovery scene of course...My deepest condolences to the families of these souls.....recovery teams...etc..may these souls rest in Peace . ... ...

This is a little like the King Air incident where the pilot died and a passenger had to land the plane. Obviously, it was a much smaller aircraft and didn’t have the mechanical issue, but the passenger had virtually no flying experience. As one other comment stated, fly the plane first and worry about the captain second.

Problem with a lot of these overseas Airlines is cheap maintenance workers that basically cut corners to save the Airline as much money as possible. Proper Airline maintenance and upkeep requires an ongoing investment from the company. However it would appear that once the Airline acquires the aircraft from Airbus or Boeing, they don't want to invest any more time and money into proper maintenance and upkeep. Quite sad really!!

Why did they not fix the technical problem instead of leaving a card warning the pilots about it? The FO was probably in shock about the Captain, but there are several instances where a Captain suffered a fatal heart attack in the air and the FO got himself together and brought the plane to the ground. However as with most crashes, there are multiple reasons why the tragedy occurred. RIP to all on board.
