absolutely. I am currently combatting sibo. If I were to follow this advice and just take more fiber, it puts my stomach into a world of pain. I've tried it before. It's bad advice.

Dr. Pam Popper is one doctor in the WFPB medical community who doesn't preach 'broccoli broccoli vegan rice' (as she calls it :) for every physical ailment. More fiber is most definitely not the answer. I say this from experience. Getting the SIBO under control is the answer. After trying everything, I had literally convinced myself my problem was in my head. Adopting the dogmatic approach of so many in the WFPB community has not served me well.

Im not sure if my exact supplements would work for anyone … but she put me on boswelia, orthospore, and meriva. Those took away my pain and ‘pregnant belly’ after a few days. I just finished a round of rifaximin but will likely do another because I still have some bloating and discomfort. But the pain is virtually gone. I know it’s important to fix the underlying issue so the sibo doesn’t come back. So once I am finished with the antibiotics … i will work with gojiman on what to take. Hope that helps 🤗

This is very interesting but unfortunately does nothing to help the people already on high fiber diets who have SIBO symptoms and many of whom seem to helped by SIBO protocols (which Gojiman has been documenting on his channel). There needs to be more support for plant-based dieters who have digestive issues rather than "it's all in your head" or "go back to eating meat/switch to carnivore diet" dismissal.

I agree. I am so grateful for this content yet as a vegan for a decade I would love to see more info on how those of us living the wfpb lifestyle can thrive and overcome things like SIBO/IMO. Fibre is killer until the overgrowth of archea and/or intestinal permeability is sorted and them one can load up again and get back that healthy microbiome. I had a stage where I could only eat tofu, white rice, apple sauce, overboiled veg or Iiterally I thought I was going to die and the malnutrition even when diet got healthy again is no joke. Now after almost a year I am making up for lost time with fruit and veg galore and so relieved and happy. This video is an oversimplification of a complex and mostly individual basis issue.

If you’re like me, fiber isn’t enough. I wish the answer were that simple. Elimination diets suck and are especially hard when you’re an athlete but i strongly encourage them when you’re doing everything else right and symptoms still present themselves. Background: I’ve been essentially WFPB for 4 years and have had digestive symptoms for over a decade. As of the last couple months I’ve figured out corn, gluten, and yeast all trigger episodes. I also finally cut out artificial sweeteners (a tricky thing to do when you’ve been hooked on pre workout since 2008).

I wish there were a good way to do blind elimination diets. With digestive issues, there's so much potential for psychological causation, which means elimination diets that you design for yourself are ripe for placebo effects. For example, I'd hate to "find out" that I'm gluten-intolerant and lose out on eating wheat forever, if the issue were actually just that I *believed* gluten would hurt me, and then the belief caused psychogenic symptoms whenever I knowingly ate gluten, that are then relieved by knowingly not eating gluten. These kind of things really can be all in your head (and that's the diagnosis I would prefer!). I'd take needing therapy any day over having to avoid a ubiquitous, nutritious plant.

Dr Greger please talk more about methane producing (archaea) SIBO. Main symptoms are constipation and bloating but fiber doesn’t seem to help. Atrantil is the thing that has helped me, though I don’t know how it works.

I've needed to work on mine for 1 year. I started with low fibre vegan diet to manage symptoms as everything hurt so ate tofu, white rice, soy yogurt, apple sauce etc then got a plan from gojiman and after my first round herbals I was able to add in lots of food again. I did a few more rounds of herbals unt il it no longer worked and then did 1 round rifaximin which I had to get gastroenterologist to help me get and that brought me from 60 to 80% better. Then I stagnated again so I started healthy gut course by happy pear as they work with dietician and gastroenterologist. I did not follow fodmap really but ate lots and lots of their super delish recipes all while following second protocol from gojiman of fermented food and probiotics which I avoided during kill phase and now a couple of months later I am eating so much fruit and veg and over 90% better which is also due to adding in daily exercise, working on my stress levels always, avoiding my root cause - painkillers for period as much as possible. I hope you find a professional to help you heal. You will get better! Xo

Atrantil has prokinetic herbs and it also attacks methanogens like archaeabacteria. Another good choice is Iberogast, it's a prokinetic. It's a German homeopathic remedy that alot of people have used successfully. It's important to get the right kind of probiotics and prebiotics. Acacia and guar gum are better, whereas inulin is terrible and should be avoided if you have SIBO, because it ferments too quickly. Unless you restore your gut microbiome, SIBO will never get better on its own. That means avoiding eating too much of triggering foods (typically wheat and onions being the worst) and feeding healthy bacteria in the intestines. Lactobacillus reuteri has also been good for my IBS-C symptoms. You can find it in kefir or a supplement. You only need to drink about a quarter cup of kefir every day to see benefits from it.

N=1 so take my advice with a grain of salt. Had a lot of gas symptoms a while ago. Thought it was fiber. I started avoiding it like the plague. Well, apparently it WASN'T the fiber. I tried a lot of stuff. Probiotics. Digestive Enzymes. Trimebutine (which regulated motility and other stuff). Tried changing the food. I didn't notice a correlation @ type of food, but mostly the quantity of food - i.e. if I ate over 2,000kcal I'd get gas symptoms. Nope. Once I started taking 10-20g psyllium per day? My gas symptoms subsided. Worth trying in my opinion. I take mine with orange juice as that stuff is disgusting with water. And I take simethicone as well - doesn't reduce the gas per se, but it reduces the surface tension and makes the smaller gas bubbles coalesce thus it favors ...expulsion and reduces bloating.

I used to have debilitating bloating where I would just lay in bed in pain for hours. I discovered gluten was causing it and I would encourage you to keep a food diary and see if there’s any correlation between what you eat and when you have episodes

I'll give my own personal anecdote but I'll have to go into detail: I used to have severe gluten intolerance my whole life without even realizing it. Suffered with Eczema and being very underweight. Since I was a kid I always wasn't able to eat white pasta without very quickly getting an uncomfortable "full" feeling so I stopped eating it, also learned from my grandpa at the time that he could only eat only whole wheat. So I swapped white breads and pasta and only eat whole wheat grains and I could eat more without quickly feeling uncomfortably full. (I think it's cause the fiber releases the hard to digest protein gliadin in the wheat slower, giving the stomach more time to break it down.) It did help, but many years went by and I eventually started developing more intolerance and it got to the point where it was 1 bite would cause sharp stabbing pain in the stomach and I would be in agony and lose all appetite, I hadn't really looked at meals as invidiual ingredients before but I became very health conscious and started paying attention to what I was eating, I noticed that specific foods triggered it, had extreme fatigue and brain fog at times I was couldn't tell anymore if it was real or placebo, after countless times accidentally eating something getting pain and finding out it had gluten, I thought it maybe is celiac and got an endoscopy and it wasn't celiac, eventually after a lot of thought and accumulated research, I followed celiac lifestyle and eat no gluten containing foods, even avoiding anything with potential cross contamination, if I wasn't sure if it was gluten free I wouldn't touch it. At the same time as avoiding all gluten, I started taking everyday 2 Tbsp apple cider vinegar (with the mother), 10-15 minutes before every meal to aid digestion. I also took specific supplements: 1. Betaine HCL, to support stomach acid production. 2. L-Glutamate Micronized (easy to absorb and assimilate proteins will aid repair to the gut lining, important to start from a very low dose as possible so your body slowly gets used to it. 3. And Digestive Enzymes. (Make sure you can trust the brand because most supplements aren't regulated and be useless, or even harmful. I used Pure Encapsulations because it's actually regulated and safe. After feeling so much better avoiding all gluten and repairing the gut for 3-6 months, I decided I would try eating a nibble of whole wheat bread after dinner each day, and gradually increase the whole wheat bread each day very slowly by less than a tsp. I did this and within a month I could eat half a slice of bread just fine I was so happy everything payed off. today I can eat 2 plates of white or whole wheat pasta at a time with ZERO noticable symptoms! My health is slowly improving my eczema 95% gone, weight is going up, etc. I'm eating mostly whole food plant based now with some processed or gluten containing foods with no noticeable symptoms and pain free.

Hmm, I still got symptoms on about 80g of fibre/vegan diet after longer multi antibiotics therapy after an accident and the wounds got infected with MRSA Bacteria... only a genetic Microbiom analysis got the key... now restore a healthy Microbiom and the symptoms stop... and it wasn't only gas/bloating and pain... I had it all, headache, joint pains, fatigue, concentration issues, mental health issues...

To repair I got a overall 12 Weeks of two probiotics preperates, the first fixed the bifido's , the second all the others ... the 'meds' have been the bacteria and prebiotics combinded... The first days have been worden, but then it got better

In my case, I had 2 or 3 months of excessive gas, bloating and intermittent diarrhea episodes when I was eating an omnivore (Mediterranean) diet. After changing to a mostly WFPB diet, it went away very quickly. Sadly, 2 months ago, without changing anything, got all the symptoms back. I actually eat more WFPB and no longer use oil for cooking but I guess the change of diet only hid the symptoms temporarily. After being ignored by my doctor after he said I was describing "very generic and ambiguous symptoms", I will have a stools test and a special analysis to discard an intolerance to gluten (my sister is celiac). It's frustrating how little doctors know about nutrition and what little help they gave. If nothing comes from these tests, I'll maybe do the SIBO one.... Any advice is appreciated! PS: in my case I completely swapped diets overnight which caused bloating for 2 weeks but then it went away for more than 6 months... So I doubt it could be the reason? Cheers.

*AN IMPORTANT MESSAGE TO ALL:* Dr. Greger isn't an expert in this complex topic you should take some of what he says with a grain of salt, no doubt there's absolutely a ton of bad information out there and SIBO is thrown around like a buzzword. Now most studies have a very narrow and simple understanding of SIBO, to complicate matters further SIBO is generally too simply defined and understood at this point. And yes it can be misdiagnosed or treated incorrectly by those who know very little about SIBO. Also don't self diagnose, talk to a Doctor first who also specializes in SIBO, it has to be a case by case basis and customized treatment plan with an expert, in some cases it can take up to 2-5 trials of treatment before SIBO is fully resolved. What we should realize with SIBO it should be looked as one component of understanding in the bigger picture as so far as it pertains to the overall health of the intestinal tract. Couldn't agree more. I've had SIBO about 1 year and over 90% better and it took a personalised plan and hard work to get here but gojiman literally saved my life. We also need to determine root cause. I believe mine was taking opioid painkillers causiing dysmotility and then chronic stress and poor diet during covid ser it off. SIBO is definitely the cause of many peoples misdiagnosed IBS and they spend a lifetime suffering when there are protocols out there to help. A vegan diet can be a healthy part of this and I made it work for me. Thank you for sharing this message for others.

Also Dr. Alan Desmond who is a plant based gastroenterologist from Ireland who has a good book out plus the book fibre fueled I have heard good things and was recommended by doctor. I have/had methane dominant myself but its almost gone. Try determine root cause and get in a prokinetic for motility. Good things for constipation: magnesium citrate, triphala, green banana resistant starch.

This is absurd; I was whole food plant-based for five years, still whole food now after treating my sibo; I was symptom-free for the first two years and had some improvements, and then it all got awful. I tried the increased fibre approach, and it made things worse. I tried elimination diets. By the end of it, I could eat nothing, so don't tell me that increasing my fibre was the answer to my gut issues. You and your team need to address this horrible cherry-picked miss-representation of the evidence! After working with a professional, I have been symptom-free for close to a year. I can eat as much as I want!

I can second Gavin's personal experience. Coming from someone who ate a strict SOS free diet for 7 years as my health steadily declined... increasing my fiber only made things worse. People who haven't experienced the amount of pain and suffering that something like this causes won't ever know the gravity of it. I too got to the point where I could only eat about 5 foods. Even tried an extended water fast. The only thing that helped was working with 2 practitioners who specialize in this (one being Gojiman... I don't understand why people say he is a fraud) and taking supplements and antibiotics. I wanted to avoid antibiotics... but after 7 years of intense pain that completely dominated my life... forgive me for wanting an actual medical intervention. Thanks for sharing Gavin :)

Don't rule out a psychological cause. Stress, anxiety, depression, etc. are all common struggles and they can severely interfere with subconscious physiological processes like digestion. Obviously there are real digestive conditions that are independent of mental health, but if your problems are more general and hard to pin down, they very well could improve with improved mental health.

There have been multiple possible solutions, many of which are questionable. There's no shortage of SIBO test labs that will "custom make" a probiotic to "cure" your SIBO/dysbiosis after doing some mail tests. Then there's the western medicine grande approach as you said, to use a broad spectrum antibiotic to wipe most of the bugs, good and bad, and then begin building up the good ones and that will hopefully minimize the bad ones. It all seems highly speculative to me. I am disappointed in this video.

I have SIBO and adding Acacia or Guar Gum has helpe while avoiding the bad FODMAPS like onions and wheat. Plant gums also help feed beneficial bacteria without fermenting too quickly.

On a very low calorie WFPB diet I'm getting about 60 grams or more per day of fiber & I feel fine. My movements have never been better. Is there gas? Some days, but not painful & there has been no bloating. The movements on days with more than 1600 calories are epic. If my calorie intake is under 1600, then not so large.

FODMAPs are still relevant. I can eat fibre rich fruits, grains and green veggies for days but I still can’t tolerate beans, artichokes, inulin etc. I would love an explanation of that.

So what happens there is you lack the gut bacteria to be able to digest those foods in the amount needed for normal portions of those foods. In order to be able to get those bacteria levels up, you need to ingest small amounts of those foods. You need to go small enough where you're feeding those bacteria and helping their population grow, but not big enough for you to experience symptoms. You might have to start with very tiny amounts, like a tablespoon or less, depending on your symptoms. Once you start those tiny amounts, increase portions of thosr foods gradually over time, taking it as slow as you need to.

Yes, eat fiber (in the form of raw veggies and low sugar fruits) along with lots of high nutrient dense eggs, organ meats, fish, sea food, chicken, beef and avoid all other food to create a winning combination of good health, strength, endurance and long life

@@VeganLinked Why would you say that? So far no evidence of such. Plus, eating meat and eating excess are clearly two very different things. No scientific evidence or factual based finds that animal fat causes disease. Correct Balanced eating is key. Not too much and not too little of each items all in the correct amount based on the person, age, existing health, goals, activity levels,.... and you use the word "Chronic" disease. We have not seen this with anyone following a correct balance diet (meats, eggs, veggies, fruits) with balanced activity. Sure excess of anything can be bad, I treat a lot of patients following stupid "keto" diets with lack of balance, also treat a lot of men with low T levels from vegan eating without correct balance of protein and cholesterol. All things have to be correct balance. No one size fits all. Sure, I am not a fan of 100% vegan eating as it has cause many people issues, same I am not a fan of 100% meat eaters, again too far extreme other direction. However, that said even though both extremes cause health issues, none of these issues can be considered a "chronic disease". That would be like saying Scurvy is a disease. No, it is not a disease. Personally hate artificial sweeteners but again, have to be honest with my patients and tell them it is not good to eat but no scientific fact that it will cause harm in humans however anecdotally, would rather they avoid it. 2+2 = 4 because 1+1+1+1 = 4 because 3+1 = 4 because 4 - 2 = 2 and so on. Scientific facts we can demonstrate and prove. Nutritional science still way behind and we have to be honest about the facts and the unknowns.

@@DrAJ_LatinAmerica Apparently you don't understand, eating animals comes with excess sulfur containing amino acids and branch chain amino acids and trans fats and arachidonic acids and palmedic acid and so on. If you compare the nutrient distribution, I have, you will see how you get access of certain nutrients from animal flesh, you will ALWAYS get excess protein and fats from animals because they don't have carbohydrates. Therefore all of your energy is coming from protein and fat so those two macros will always be in excess. Animals are not a good distribution source of macro nutrients or micro nutrients.

@@k.h.6991 Doesn’t work that way. If you can’t eat your FODMAPs you never will. I think it’s to do with the physical reality of your gut. Some of us have longer bowels, some have a naturally slow transit. Give some type of carbs that much time to ferment and they produce a lot of gas which slows transit even more, causes pain and co stop action and the other thing..

SIBO or microbial dysbiosis- it has several names- doesnt mean it's not a specific condition and doesnt just show results from breath tests. You can biopsy for several strains with deep EGD and even spot yeast based strains in bloodwork. Are we also discounting the thousands of testimonials available (including mine) that low fiber, SC diets and AIP have behind them showing success. I can tell you personally, until my gut was fully healed, thr smallest bit of fiber would have me bedridden for days. Thank you for the video. I humbly believe he is so wrong stating "to know what kind of bacteria." We have hunders of different bacterias in the small intesting, much more in the large intestine. Any test is not able to tell what bacteria is causing IBS/SIBO, GI maps, (all of them) only test few of them. I am sorry, but his statement doesn't make sense.

Obviously this doesn’t speak for everyone: my diet is constantly pretty high in fiber (constantly use Cronometer). I still suffer from heavy SIBO type reactions on seemingly random but normally consistent days. If this is the only battle against SIBO then unfortunately many people who are already vegan may continue to suffer.

@@krzysztofh6308 There's lots of alternatives to giving up being vegan if you have IBS. A carnivorous diet to fix IBS is like fixing a broken toe by amputating a foot. My point is that certain kinds of fibers do sometimes bother people with IBS, but there's no reason you have to eat a high fiber diet to be a vegan. There are plenty of other vegan diets out there besides WFPB.

I also have it. Fiber does help so much. Some of the problem is regularity of fiber intake. If you only have fiber once in a while then it is an acute problem. The key is fiber every day from a diversity of plants.

It depends on the type of fibre. Gregor’s model is too simplistic. Some can tolerate a lot of fibre but one type of carbohydrate can cause disruption to the whole system. I found the FODMAP chart the most helpful thing to use to manage IBS. It allows you to understand what type of carbs give you problems. Not every sufferer reacts to every FODMAP category. I just have to avoid three classes of carbs and I’m good. Not a big deal unless you are vegan and need to eat them to get enough nutrients. I have to eat some fish and eggs because I can’t eat the beans that you need for the protein.

I suffered from ibs and chronic gastric reflux and bloating and abdominal pain for half a century. My entire adult life. I went carnivore diet 10months ago, and every one of these conditions disappeared completely in week 2. Gone. Completely. My diet contains zero sugar, zero carbs, zero plants, zero fibre. Zero fibre. My health, mental and physical, is astonishing.

You mention prevotella in the gut. One possible way of safely increasing prevotella in the gut, is to throughly chew your foods rather than gulping down whatever it is you stuff in to your pie hole. Saliva is amazing. Get you some.

Either you can believe that nothing created everything or someone outside of space, time, matter created everything out of nothing. Since nothing can't have an effect, it makes much more sense that the other is true. Both world views need the biggest miracle ever--but only one allows miracles. To me, it sounds pretty straight-forward.
