Fibromyalgia is characterized by "centralized pain," which differs from nociceptive or neuropathic pain. This means that conventional pain management techniques like opioids, NSAIDs, and injections are less effective. In patients with Fibromyalgia, the sensory processing "volume" is turned up, leading to heightened sensitivity to pain, pressure, and even auditory perception. However, it is crucial to understand that sensitivity is not synonymous with fragility.
We will explore the concept of impaired descending analgesia and its role in Fibromyalgia, as well as the effects of the condition on patients under anesthesia. The use of Nabilone, a medication shown to improve sleep disturbances and pressure allodynia, will be discussed, along with the potential mechanisms that lead to central nervous system sensitization, such as serotonin, dopamine, and glutamate pathways.
Additionally, we'll touch upon the link between Fibromyalgia and autonomic dysregulation, including conditions like Postural Orthostatic Tachycardia Syndrome (POTS), and how sleep disturbances (e.g., α-spindles in non-REM sleep) and insomnia may contribute to the development of the condition.
The video will also cover the benefits of Nabilone in managing Fibromyalgia-related pain and offer practical anesthetic management tips for patients with this condition. Some recommendations include using magnesium and lidocaine, ensuring vitamin B12 is replete pre-operatively, and optimizing nutrition due to the common coexistence of leaky gut or IBS. As someone who has fibromyalgia, I love going under anesthesia bc I wake up feeling like my muscles had the chance to relax and if it's a longer surgery with deeper anesthesia I feel like I got the best rest. It's a brief pause that I wish I could feel like that more often.

I am a retired psychiatrist (MD) and I appreciate this physician. His explanations I know to the extent my knowledge go, is correct. He has also added information to my databases. I am glad he has this channel and I hope more tune into it. Yes, I really like his compassion.

My medical care provider, I was her first Fibro patient. Yeah, I've spent so much time online weeding out the garbage from the facts. Trust me, there's a lot of garbage out there. Sadly, many people believe everything they see online.

Less than 3 minutes in and I’m sobbing. Unrelenting pain for over 10 years. I was a ballet dancer, then 2 hospitalizations, (pneumonia and hemorrhaging with transfusions) and 3 months later my body was shot, I could not dance again. I was 56, and had more endurance and strength than someone half my age, No cellulite, solid muscle, then I was a wreck. I can’t even walk up stairs, I hobble on a rollater. Your compassion is what broke my heart. My husband and children are narcissists. I am alone in this shell. May God bless you for your sensitivity and compassion. People like me, just are so ALONE.

I know what you mean about being in a shell. My husband still after 20 years will kick me in the but when I walk by his chair and then he gets mad because he thinks it is all about him when I say it hurts.

I'm a Fibro Warrior. The nervous system being turned up is such a good analogy. It's like super powers, but not the ones you want. Super hearing really is unpleasant in large crowds. I really needed to hear about the difference between Sensitive and fragility. I've been really depressed about the limitations Fibro & Chronic Fatigue has brought to my life. I've been struggling because I slipped out of good habits I had before the pandemic. This is another good reminder to keep taking the steps to get back into the groove. The descending part, yes, top down body scan and meditation, things definitely go better when I practice those meditations. I've had medical trauma in the past (PTSD because of it) and honestly just listening to you is healing. Emotional pain and physical pain are on the same neuro-pathways. An integrated between natural, life style, and modern medicine is definitely key. Now I'm focusing on getting back into good habits without judgement. Knowing that there are compassionate and knowledgeable folks out there in the healthcare world advocating for people is super hopeful. Thank you for this video!

I have Lyme disease and Fibromyalgia. And I have zero support. My entire family has ‘estranged me’ because I’m ‘crazy’. I have a frickin’ Ivy League education and a Masters in Medical Anthropology. Was that enough? Nope! My parents stopped talking to me, died, and I have no one! My thesis was about Black people being treated differently in hospitals - and there are Far fewer Black organ donors, because they think they won’t be given the same life-saving measures as other people. 😢 Thank you SO much for being So caring, and for believing us…..and for educating others! ❤

I am so sorry that life has been so difficult for you. I have fibromyalgia as well and don’t know who I’d cope without family support. I hope you can find some type of support system. Years ago fibromyalgia was not well recognized. The medical system is beginning to understand that fibromyalgia exists and how it effects people. Hope you find some support ❤

Harriet, I am sad to hear of your frustrations with your family. Initially, I suffered from this with my family too. My mum has since gained more understanding. I chose to stop going to my brother's family holiday events because he was aggravated with my chronic lateness to his get-togethers. I chose to remove myself from the added stress of trying to be at those events where I sat uncomfortably for hours and him taking zero effort to understand how it impacted me to be there. As an adopted person, I can say, family is not always blood-related but whom you choose to have within your circle of trust. Maybe you can create new family members online and offline to be able to unpack the challenges that you face when you have low days. Much love

And the age old battle of the sexes. I kept saying…..I had cardiac issues. Only after, I went into severe left sided heart failure did I get treatment. I had trouble breathing and was treated for COPD. The inhalers were useless. I need diuretics. I was filling up with fluid. I showed up at my doctors office in severe failure. I was tired of going to ER with no help. Men get help so much more than women. Also if you are a nurse…..omg! We are seen as nuts. My little old lady patients had severe heart disease. A 39 yr old man comes in and is given a very expensive cardiac cath. Little old ladies die. I had to threaten to sue the doctor. He finally cathed my mother. She had severe blockages and coded. They got her back with the help of anesthesia. I fired the cardiologist. I’ve seen this over the years. Trying to get my autoimmune diagnosis was a nightmare. They sent to to the psychiatrist for two years. He told me I was physically ill. Finally, able to get diagnosed with SLE. I could go on. Years of mistreatment or no treatment. Because….I am a woman and a nurse. I’ve had enough of the medical system. I left nursing and very delusional.

@Rosebud Adkins Did they not do chest xray? You can see pulmonary edema and cardiomegaly on one. You can also hear pulmonary edema (wet lungs). Jugular vein distension can show pulmonary edema, which is hard to see on some people related neck anatomy.

There are many prejudices in the medical field. I am now in my seventies and have noticed that many doctors don’t want to be bothered with me. Ageism is a really big prejudice. No fun when you are a senior with health issues.

I’m a nurse and you had me at “our broken medical system” I also have Fibro, Chronic Fatigue, C-PTSD, lifelong treatment resistant depression & anxiety, and about 20 other medical conditions. Having invisible illnesses is very difficult. I’d love for you to talk about MTHFR and all of the things that affects. How do we get other physicians on the same wave length as you? ❤️

The medical professionals I've encountered with regard to fibromyalgia, were absolutely unprofessional even downright stupid . Jerks. Made me doubt my sanity. Not ALL medical professionals are like this. But biases in medicine absolutely needs to be addressed especially with the older male m.ds. I had an endoscopy and colonoscopy today. Yes, I cried. I told them I wanted my mother who was dead and didn't care what anyone thought. I woke up and asked, " I wasn't an asshole was I?"

Yes I agree too as a sufferer of both Fibromyalgia and Severe ME/CFS and now CPTSD. I find medics in the States are far more knowledgeable and have total understanding. We need them to come over here to teach our NHS staff.

I don’t have fibromyalgia but I do have mTBI, post concussion syndrome, ADHD, chronic migraine and neuralgias as well as two painful nodules post MVA that started the “party” described above.👆🏻I underwent surgery last December to excise those red hot pain lumps and I woke up in a deep state of calm and restfulness that I hadn’t felt in many years. My overall allostatic load was ramped into a constant overload state from the pain they were contributing…I had some post anesthesia sympathetic arousal but overall, I woke up with greater calm…and much less pain, much as you’re describing. Really interesting and insightful post. 🙏🏻

I'm so happy that you made this video. Doctors don't understand fibromyalgia for the most part. Everytime I see my GP and she asks me about my fibromyalgia, after I tell her about what I've been going through, my subconscious ends up blurting out of my mouth, I have a broken central nervous system! I never know I'm going to say that but, something in me knows this for the last 15 years. It blows my mind that it can actually screw up my body temperature. I've had a couple of flares recently and I was reminded of just how many different systems go haywire. Thank you for being understanding. Too many women have been told that they need a psychiatrist and were not taking seriously about fibromyalgia. The pain can sometimes be so debilitating. I can't stand it when my skin feels like it's been burned. I thought the anesthesiologist was not aware of the patient after they woke up. I didn't know that they checked on the patient. We need you to become a teacher and teach patience and empathy and the power of one person's heart toward another. 🤗💙

My anesthesiologist, was with me from lights out until he took the vent tube out, 2 hours after surgery. Dr. Gregoretti. UAB hospital, Bham Al. He's now a Professor 😊

I call the temp flux "fibro flu" I sometimes wear a heavy sweater in 80° heat. I can't adjust. And sometimes I wear a halter top when it's 20° out.

@@southernbellebornnbred7811 Isn't that really something? I didn't think about other people feeling the same thing. I went to someone's house and they didn't have air-conditioning on and it was 80° in the house. I couldn't tell the difference. I remember two winters ago being very cold. But, I couldn't wear warm sweaters. It's not like menopause when you have a hot flash. It's more of an ebbing and flowing thing. You could be freezing and go to bed with nice pajamas on but, five minutes later you're burning up. I just listen to what my body says. I couldn't imagine this when I was younger.

@@alchemybyangela I've only ever had one rheumatologist who would treat my fibro. Most won't anymore, either because they think it's akin to mental illness, or because they think it's a play for pain meds. I quit taking pain meds years ago because of the stigma, not because I was even the slightest bit addicted.

When people ask me about Fibromyalgia. I tell them that when I get cold, every 'goosebump' has its own hair follicle. Every hair follicle has its own tiny muscles, this makes your hair stand on end to trap the warm air. Every muscle group HURTS, every individual hair HURTS. On top of that I have noticed that I no longer feel the cold, I DO NOT shiver to warm up. When I am hot I cannot cool down either. Combine that with smells, sounds, extreme pain, my life is awful. This is real folks.

I couldn't agree more. I was dx at 19 then had to move shortly after. After the 1st 2 Drs telling me Fibro didn't exist and it was all in my head. Or a couple of others who treated me like I was just after pain pills. So I quit going. I did as much research as I could on it so I'd know what I was dealing with. That was rough then. So much easier with the internet. I have finally found a good Dr I appreciate it dearly 😊 I'm 58 now.

One thing that has helped me describe the "sensitivity" is instead use hyper aware. Fibromyalgia is horrible. Thank you for addressing it in a respectful manner. It's not all in my head as often perceived.

They went over me and finally suggested I see a psychiatrist. I told him my mind is part of my body. I went to the psych doctor, and he said he thinks I have fibromyalgia. He sent me to a rhumitoid doctor, and they said I have fibromyalgia and Epstein barr syndrome. Later I found the celiac disease, rhumitoid arthritis, psoriasis, and ankilospondidyosis. With many other things. I had been to so many doctors as a child and had all the signs of all except for rhumitoid. It took them (I had to basically my own doctor, btw) till I was in my 40's to find all these diseases, and I figured out two of them myself by studying my own stomach and nervous system that I found two of the autoimmune diseases myself. I'm sick of the medical system in this country. It's all about the Benjamin's in this country. Thank you. I look at surgery as a chance to sleep. I'll hop right on their and tell the doctor to put me to sleep for 8 hrs. ✌️❤

I don't recall having an issue with anesthesia as a fibromyalgia patient. But I do remember the pain hitting about 2-3 days later. That's usually how it happens with me. I'm fine while dealing with the stressor as it is present but then my body flips out a day or two after the stressor is gone. My doc was kind enough to prescribe me a bit more pain meds than other docs at the hospital usually prescribe. I thanked him and told him had I not had fibromyalgia I would not have needed all the pain meds but once the surgery pain ended, the fibromyalgia pain began full force.

My husband was a surgeon. He always told me that pain is the worst on the 3rd day post-op. I have fibromyalgia and have found this to usually be true.

Geez you understand more about Fibromyalgia than years of different doctors and specialists I've seen over 21 yrs!! I've had 8 out patient back surgeries with anesthesia and 3 without for 4 bulging discs, a pinched nerve and arthritis. The ones without was like being literally tortured and very traumatising. I've been diagnosed with 5 autoimmune disorders main one being fibromyalgia. Yes I had an extreme amount of childhood trauma. Great video!! Thank you for your explanations, knowledge, understanding and empathy.

I watched a documentary on Netflix “Change your mind” and decided to try micro dosing on psilocybin capsules and it helped me with my fibromyalgia ,anxiety,depression and rheumatoid arthritis. I haven't had a bad day in months, I'm feeling productive like before.

I've had multiple surgeries and no one has EVER talked to me about fibro! I wish you could train every doctor to treat their patients as you do. Last time I had surgery the anesthesiologist told my sister that I was "all over the place". I don't even know what that means...but it made me feel guilty for some reason. I just wish they cared more about their individual patients rather than just treating us like an assembly line. 😕 Thank you so much for your videos!

I was diagnosed with Fibromyalgia 40 years ago. I don’t take meds for the pain. I was severely abused as a child and had to find a place in my mind to survive the abuse while it was happening. I use the same approach to manage the pain of the Fibro. Some days (especially during bad weather) it doesn’t keep the pain in order but helps somewhat. I found that not one anesthesiologist believes that I have a high tolerance for painkillers (I can’t take anything with codeine due to a bad reaction to the drug). It takes quite a bit to knock me out and keep me there. I started waking up on the operating table during a gall bladder/appendix removal just as they were stapling me up. I couldn’t move or speak but my mind was wide awake…kind of like sleep paralysis. I had the same problem with cataract surgery when they were supposed to have me in a kind of twilight sleep. I felt the whole thing. I also come out of the anesthesia almost immediately after I get to the recovery room. My sleep is normally extremely light and I don’t sleep well. How do you handle people who have this problem?

I have a high tolerance too, always have!! After my hysterectomy I was in extreme pain and the nurse said I should be dead with the fentanyl she gave me. I was speechless! Ahhhh, I'm awake and in pain!

I share a lot with you. I wake up over and over under general. I stood up and started doing stuff under general for a colonostophy (sp?) recently. I apparently do IT for the surgical staff and they say I am very talkative. I can feel any procedure, but not pain from it. It's like pressure.

I've woken up with having my gallbladder out and a hysterectomy! Luckily with the hysterectomy I also had a saddle block so there was no pain. The nurse flipped out and was like I'm my God are you ok. I kinda laughed and said it sure is cold in here lol.

What a relief it is to hear you validate that this is a very real and very painful condition. When my RA is under control thanks to my biologic infusions my fibromyalgia doesn’t seem to improve that dramatically . I try to tell people the story of the Princess and the Pea to describe how I can feel a cracker crumb in my bed ( yes, I eat in bed a lot) that makes me have to get up and brush off my sheets and smooth out any and wrinkles before getting back in bed and having the pain slowly die away. I also was calm and comfortable after my hip surgery but I attributed it to the pain meds ….so maybe it was a combination of both. Your videos are so relatable and reassuring, thank you for taking the time to help us understand the inter workings of anesthesia.❤

I can't stand the seams inside the toes of my socks. I have to wear my socks inside out. Even then, if the toe seam bump is too big, I can feel it irritating my toes, and I have to take the socks off. I simply throw them away.

I have never heard someone mention CSS and Fibromyalgia….in this context. I’m weeping…..I’ve not heard someone care enough to know what it’s like and address it. I have nearly every thing that you spoke of in terms of sensitivity. I freak out with needles because I can feel them ripping my skin and tearing open my vein…..sounds exaggerated, but it’s real. When you are poor, they don’t care. You’re accused of being dramatic or whatever….ungrateful. If everyone was like you, it would be easier to deal with. Thank you Doctor.

As someone with fibromyalgia, I wish I would have had you as my doctor! 2 surgeries in past 6 months... it was a nightmare... the most painful event started as soon as the Bood pressure cuff went on... the pain of it was soooo intense that it shot my BP sky high so the couldn't put me under.... I kept asking the nurses do please move cuff further down my arm... they would not... the tears would not stop because of the pain and got progressively worse as the nurses were harsher and harsher... I was bullied by them... "stop exagerating" "your wasting our time", etc, etc!

I’ve had fibro since 2002 and I’ve had surgery three times, and I always wake up during surgeries. Anesthesia never keeps me unconscious, but they never believe me when I warn them ahead of time. When I wake up on the table, everyone in the room always freaks out but I’m the calm one.

My son had FM he had cancer and was on oxygen, and he said the oxygen helped the pain. I've actually heard that before..that oxygen could be something that was affecting the muscles..not enough..

As someone who has severe fibromyalgia, this made me cry to hear a medical professional speak about something that I have been dealing with for 20 years but my local doctors are behind on this information. They treat me like I am exaggerating my symptoms at times. They have also put me on contrasting medications because they didn't believe me about the symptoms I have. Now all I can take for the pain is acetaminophen and ibuprofen. Edit: I live in a state where weed isn't legal. My current doctor, who FINALLY gets it, has said that the second it becomes legal I will get a prescription if that's what needs to be done. No relief from the pain and tenderness for the last 5 years has been difficult. 20 years total of near nonstop pain but these last 5 years have been the hardest.

It does help the muscles.relax.ao much! It is illegal.where I live also. My problem is that I take vicoden for the osteoarthritis and I cannot have any other substances in my syatem. Right now the bone pain trumps the fibro pain. So I am.sunk! The cannibis doesn't help the bone pain.

I've just watched a video by Dr Ken Berry and he was saying get off ov that medication it causes kidney disease...I'm taking anti inflammation tablet i should mention it to my dr..ibuprofen. is the one he mentioned..I'm on a different one but makes no difference..i don't know why I'm taking it..dr said it would get worse, it's already worse..

So sorry for how you've been through and the way the "professionals" treated you. I have severe fibro, ME, and a permanent back injury. Of the 3, the fibro is the absolute worst. Went through a long period of trying to find a doc who wasn't abusive. Sadly, seems like in these times, there are more who don't have any compassion for us. Am somglad to hear your doc is open to giving you a rx for alternative meds. You deserve no less. God bless you and pray that your pain will be less and tolerable.

I was a hospital RN, diagnosed with Fibro in 2007. My medical/surgical hx is quite extensive. In 2012 I broke my ankle and it required surgery. I had my first 'block', and I can't tell you how great that experience was. I've always had a tough time with anesthesia and I couldn't believe how great I felt post op (and not in a 'high' sense!). I was put on permanent disability in 2015. I just came across this video, and want to thank you so very much. I do have a problem with focus now (can't read articles, or novels any more).....but I've watched 1/2 of this tonight. Tomorrow I'll watch the rest! 😉

I had the exact opposite reaction with a block. I was diagnosed w/ Fibro about a year after being in a head on collision on the interstate. I was bedridden and non-weight bearing for 3 mo. while all my injuries healed. Having been a very active person before I became quite depressed and couldn't go back to teaching due to the physical constraints I now had. The following year I began aching so badly deep in my bones that I thought I had bone cancer. Eventually I was diagnosed w/ fibromyalgia. Years later I had to have both knees replaced and the first knee w/ general anesthesia went great! I was able to take my medication for pain, from the replacements, and got off of it with no problems, right on schedule. By the time I had the 2nd one they talked me into their 'new' anesthesia technique of a spinal and nerve block. Once it wore off, I was totally unable to take any of the oxycodone as it set up a hive/itch reaction. We tried several different manufacturers as the Dr. thought perhaps it might be caused by a filler one or the other was using, but nothing helped except not taking pain medication. Let me tell you it's quite difficult to use only Tylenol after a knee replacement. My body has reacted quite differently to many medications, vaccines and treatments ever since the accident, and I'm unable to take them.

When I was hit by a drunk driver, I had a 3 level spinal fusion under general, but I was able to be weight bearing after an acceptable period of time. I had 2 months non weight bearing before my ankle surgery, and then 3 months after - and I know it's tough. It's certainly understandable how one might fall into a depression with everything that goes on with everything we've been through. When I had to leave work - it was tough. So sorry you're not able to work either. It's a massive adjustment isn't it!? I had several teachers in my family. My Dad @ a Chicago University, and my sister was a Principal. Thanks for what you have contributed to the education system!! 👍 Correct me if I am wrong.....are you relating your sensitivity to your pain meds to the Nerve Block you received vs the general anesthesia? I only ask that, because It is extremely common for people with Fibromyalgia to develop sensitivities to medications (and other things)....because of the Fibro. I understand having a sensitivity to the nerve block components, but I didn't know that reaction would cause a reaction to other things. I used to be able to take meds, that I now have reactions with - and I have been told that is not uncommon as Fibro progresses. I now am very limited in what I can take too. Even the components used to make the capsules that hold medication is a problem for me. FYI: I have had the most luck with plant based products. I don't get relief of pain with Hemp CBD, but it helps me a lot with getting a better nights sleep. I also have a stinky Hemp CBD topical gel that gives me some relief, and unlike many people.....I find some relief with Diclofenac Sodium Topical Gel. Have you tried that by chance? I have an Over the Counter allowance from my medicare allowance (I have medicare due to my disability - not age), and it covers the cost of the Dicofenac. Do you mind me asking if you have a sensitivity to laundry detergent, & if you do...have you found a specific product that works best for you? Thanks so much! 😉

I have fibromyalgia. We can trace it back to my younger years. It just didn’t completely rear it’s ugly head until I went through a major cervical spine surgery. I have problems with anyone who even playfully pokes my arm or leg. It’s like a knife to me. I loved your analogy of fibro.

Thank you SO MUCH for speaking to the needs of patients with fibromyalgia in such a complete, compassionate way. We are so often disbelieved and under-believed. The link between fibro and childhood trauma cannot be overstated -- top-down regulating of pain (and emotions) becomes so compromised when we're not taught a healthy relationship between our body and mind. -- It's so simple: if a developing creature is damaged, and especially if not treated properly after, they can be crippled for the rest of their lives -- the EXACT SAME THING happens to human beings, and it's true of physical -AND- emotional damage.

I am glad to hear someone, a surgeon of all people, to be aware of how to treat and accept fibromyalgia through prevention and treatment within a broken medical system. Too bad you weren't my doctor. The emotional damage brought on by mistreatment, compounded through years of neglect by those who are supposed to help, are more damaging and scaring than just the physical aspect of not treating a physical wound.

You are one in a million. First surgery, woke up on the table, I’ll never get over that. Second surgery, emergency - appendix ruptured and I had a wonderful anesthesiologist like you who spoke with me all the way into surgery. I told him what happened, he said he would make sure I was taken care of, and he was true to his word. Six hours on the table, woke up with tubes in my nose and elsewhere with hardly any pain.

Finally acknowledgement of my Fibromyalgia and all it’s hellish symptoms. I woke up once after anesthesia singing. I was told I have a beautiful voice. I am considering accupuncture since I cannot take pain medication for the reasons you explained. I am constantly battling pain, and multisymptoms. I’m learning so much. I have had Fibromyalgia for over 25 years, and although my GP is great, most other medical personnel roll their eyes, and suggest counseling. This video answers so many questions. In June 2021 I broke my right leg and ankle very badly. It took 3 surgeries (in 3 weeks) to rebuild my lower leg. I had the worst reaction. It took me forever to begin to feel “normal” again, then I lost most of my hair. Sometimes it’s overwhelming. Thank you for caring, and for not treating those of us who suffer with this as “HEAD CASES.”

I just stumbled across this and I think it was meant to be. I’m an RN who has been dealing with neuro and neuromuscular problems for two years now leaving me unable to work.. I made it through nursing school and worked only in hospitals (ICU then ER) despite having been told at 20 my anxiety was too bad to make it through. 🙄I had PTSD and survivors guilt, which no one really knew what those were back then. So I suppose that’s why my therapist said I’d never make it. I mainly worked ER and I guess now I’m medically retired after 23yrs. I was so lost and depressed because no one knew what was wrong with me. I see specialist 5hrs away at UCSF in the Bay Area because I live in a small Northern California town. My pcp recently diagnosed me with fibromyalgia as well as pots. Just waiting on a second cardiology consult so I can get a tilt table test. I can’t believe in all my years of nursing I hadn’t heard much about pots. Anywho I’m going back the end of June for an EMG and some biopsies, but I can’t thank you enough for talking about this subject. I'm a retired physician who had to partially retire at age 53 because of fibro. I'm fascinated by this doctor even considering fibro and anesthesia. I have never had an anesthesiologist even seem to acknowledge it in my history. I apparently have been blessed with lack of post-op pain, even 2 hip replacements! My osteoarthritis is what I need pain meds for. I have had kidney injury from Tylenol so cannot longer take anti-inflammatories, which helped me so much.

This is incredibly sensitive. I have had a number of surgeries, and fibromyalgia is just one of a number of conditions with which I try to cope. We need more people in the medical fields who are aware of patients' needs, particularly as the patient is preparing to be anesthetized. I am very grateful for addressing so many topics for the layperson.

This is so interesting. For 62 years, I was a lazy, hysterical hypochondriac. THEN, I was diagnosed with fibromyalgia. Over those 62 years, I had 17 surgical procedures and 8 general anaesthetics and included a number of abdominal operations. Every time, the effects of the anaesthetic took months to get over, and scar tissue/ adhesions complicated recovery. This video helps me understand what I have lived. Thank you.

How did you find strength? I've been diagnosed 7 years ago and every year the pains are stronger, the exhaustion is behind belive, the dizziness, the nausea, the fact I can't do lots of things anymore, I don't to go or do nothing, I'm a prisoner of my body and mind. ❤

Try and find something you can still do to distract you....knit, color, games, puzzles, reading, writing, pet therapy, aquatherapy, etc. Be kind to yourself! Give up trying to get others to understand. 😐 Ppl don't get it, unless they've got it. That said, very surprising this dr understands somewhat! 🥺🤧 Maybe there's hope (for medical ppl & subsequently, us). 🙂

I'm American and now live in the UK for the last 5 years. My last operation was here, two years ago for a bunionectomy. My anaesthetist talked to me, in pre-op, about my fibromyalgia, sjogrens, Lupus SLE and RA. She said i would most likely be in more pain than usual but would help as much as she could. I never had post-op pain like i did in America for my last operation 10 years ago. It was a spinal cord stimulator and it was horrific. I only needed antiinflammatories after my bunionectomy here in the UK. I woke up so calm. I'm about to have a complete hip replacement soon and I am terrified. I was in tears before I went to sleep in my bunionectomy. I've never been like that. Even after at least 10 operations. I want to wake up calm again so I think I'm going to ask for a spinal anesthetic instead of a general. Thanks so much for all your care Dr Kaveh!

I had a spinal cord stimulator put in 15 months ago. I woke up screaming in pain and I was begging for them to knock me back out. I had pains that felt like someone was stabbing me over and over again in my abdomen. I went through all of my post op pain medication in the recovery room. My Dr, Dr Christine Mina, was the worst Dr. EVER! The nurse told her what was happening and the Dr.s response was, “yeah, it happens, it’s rare but it happens.” And then she LEFT! I have fibromyalgia too. Plus, I have the redhead 🧬 gene. 😢

That is great Heather; you can also ask how many people will be in the room for your surgery and people that just bringing the device don't have to be in the room until you are completely draped if that makes you comfortable. You can also make an appointment with the anesthesiologist before the surgery which makes you feel comfortable. I wish the best my mom had RA.

Oh my! I’ve never found anyone with all my diagnoses. I have same as you with one small change mine is OA instead of RA. Everything else is the same. I know just how your days are. This sucks! I know there are new technologies coming soon. I am hopeful because it’s been years of pain. 🙏 for hip surgery. Hug for you! ❤️ I had surgery August 2022. I had some issues in recovery due to my conditions. SLE has cause diastolic heart failure. Dx May 2022.

I really enjoy your videos. They always think I won't need much to put me out because Im only 113 lbs, but I have a high tolerance, so I alway tell them to make sure I'm completely out. I went to an in and out clinic once for a colonoscopy and they were only allowed (by law) to give me just a tiny bit of medication....I was awake the entire time and the nurses were holding my legs down because I was in so much pain. I never went to a place like that ever again. That was horrific. I have fibromyalgia and always get so excited when I see the anesthesiologist. I know I'm about to be pain-free for at least a few minutes (time depending on what's being done) and a few hours afterward.

I learned more about fibromyalgia from this video than from any doctor or any other material that I’ve come across. Mine came after a head injury from getting rear ended at high speed. I also have PTSD since the accident and have great difficulty regulating my nervous system. I do believe though that learning to control my brain is the best way to find a cure. There are some great online programs to help down regulate the Vagus nerve and the autonomic nervous system.

I have ME/CFS and Fibro. I've been under once for a D&C, and sedated another time for a gastroscopy to check for celiac. The former went fine, the latter - I was actually fully awake rather than sedated, and felt the tube going down my throat and was wide awake during recovery. In fact, when I got home I moved furniture around to redecorate, I felt like superwoman. I wish I knew what that last stuff was, I could sure use it most days! 😆 Thank you for describing the difference between sensitivity and fragility. Many people don't understand. I can't take most medications including pain meds (even cbd oil), I react quite badly but ultra low dose naltrexone has been of great benefit, for both pain and sleep. What a wonderful, caring and knowledgeable doctor you are! Newly subscribed 😊

For my surgery last year, I spoke with an anesthesia nurse and spoke about my fibromyalgia and other symptoms. She was very attentive to me. I was given my relaxing medicine before I was put under in the OR. The pain after surgery was so painful. I wish postop was kinder to my needs.

You know more about fibromyalgia than the average doctor! You should teach them. The fact that the word sensitive triggers many is because they are called out overly sensitive in a bad way by many doctors before they even get a serious diagnose. Thank you doctor for being so considerate and compassionate! I have fibromyalgia which I was diagnosed with in 2002. My body literally shut down after getting pneumonia and a couple of fluke accident that you wouldn't think would do that much damage. Later, in 2011, I started having flashbacks of abuse as an infant. It scared me because how in the world would I remember that right? I knew my dad was abusive but to remember that I said when I need counseling so I did and of course was diagnosed with cptsd. I would shake literally. I had a neurologist examine me for migraines who did not know about any of the trauma in my childhood, and he told me I had shaken baby syndrome. I was told and 2003 that I would be in a wheelchair for the rest of my life due to fibromyalgia. I cried I was in the wheelchair at the time, but I told the rheumatologist that I would not stay in that wheelchair and I have not. I had to go through the pain and literally walk out of it. In 2014, a neurologist named dr. Sultan who had a internship in your muscular disorders diagnosed me with hemiplegic migraines. Once the migraines were under control, I could work full-time again though I have to understand those pain sensitivities. Now I take care of others as a caregiver for an agency and a music teacher. I recently had acdf surgery which helped tremendously in the way I walk. It was for disc C6 and c7. I can't think my anesthesiologist and dr. Lewis who performed the neurosurgery enough. Walking through this pain has made me more compassion, and I can immediately tell when someone's in pain even though they're trying to hide it. Your job is awesome and I'm glad you are there for the people that you care for.

I was diagnosed at age 12 (early 1970s) with what is now called fibromyalgia. I've had rough experiences coming out of general anesthesia and dentists/dermatologists have found that I require more/stronger local anesthetic for procedures. My condition is much better since I have improved my nutrition and I'm glad to hear that this may help if I ever need surgery again. This video has also let me know that I can/should talk to the doctor/anesthesiologist about these things. So, thanks! P.S. I love that you use the term woo woo! You have such a sweet countenance!

Excellent help here for fibromites and others. Sadly I had none of this consideration prior to major surgery 5 years ago. I was too sick to advocate for myself and the doctor's office was mismanaging me from the start. I have fibro secondary to Lyme plus I'm bipolar. Oh joy. I had complications and procedures afterwards that scared me to death and it made my cptsd worse. Dang. I've been a self help kind of person since 1981 when I kicked hard drugs (got the Lyme Fibro in 1994) but since this surgery I admit to giving up. Done. I now have near constant tremors and anxiety which I never had a problem with. Thank you for what you are doing to educate people. I think you are terrific!

Thank you for speaking of Celiac disease and fibromyalgia and low B12 I suffer with all these among many others. I also have lung and heart diseases. Many autoimmune diseases. I appreciate your words of encouragement. Many Drs are in it for the money and not bc they care. It’s nice to finally meet someone who cares. I wish you worked at a hospital near me. Though the last surgery I had to put my pacemaker in I had great care at Toronto Western Hospital and the team that worked with Dr Dean Elterman. 💝🥰🤗

I was diagnosed with Fibromyalgia in 1989. I don't know exactly when I got it, it's suspected that I got it after having chickenpox as a young adult. I always take longer to wake up after anesthesia and I am sensitive to all the chemicals such as perfumes, detergents, cleaners, and just about everything. Each surgery I've had has marked me more sensitive. No surgeon or anesthesiologist has ever told me that they would change things to work with my body, but one surgeon told me he wouldn't fix my gapping hiatus muscle for fear of losing me on the table because of my sensitivities/allergies. I won't have another surgery if I can help it. It doesn't come out well for me. And believe it or not, I was - for many years- told that this is all in my head, all made up. Blessings.

I’ve never had issues with anesthesia as a fibro sufferer. I would always look forward to anesthesia so I’d be out of pain for a bit. Goll it’s so debilitating. Have had Fibromyalgia for 14 yrs now. Multiple surgeries and other things, but no one's ever explained it like this! Makes sense when I 'wake up' every single time in far more pain than expected.

Thanks for this discussion of Fibromyalgia and the relationship to POTS. Non-POTS Dysautonomia (Orthostatic hypotension/syncope) also complicates medical procedures with it’s labile BP and HR. I just had to convince the nursing staff that a post cardiac procedure low salt diet was contributing to my BP tanking. They listened, gave me salt and watched my BP slowly climb into normal range. 😊

The hypotension thing happened to me 3 days ago. I'm in a recovery hospital after bilateral amputation of legs after a thrombosis. They put me on a normal diet right after I got here. Blood tests were ordered jic it wasn't the fibro. It was. Lol Hang in there fibro friend. 👵🤗❤

My vitals started crashing once outa anesthesia & i immediately sensed & asked what went wrong while “under” & should my brain be on fire… Anesthesiologist then pushed IV Dilaudid for that…my POTS kicked in & I went down fast… Had good “team” who talked me through the “intense breathing” it took to raise me back up…several hours later I finally stabilized. I’ve been traumatized ever since,months now…until your informative videos! Many thanks & Blessings to you! 🙏

I'm a critical care nurse, and I educate people constantly about POTS (I have first symptom ~4th grade, fibro & more) and the sodium intake needs being doubled than regular recommended amount. I carry Gatorade powder with extra sodium in it, never drink plain water because my sodium tanks. I have pacer so I'm cardiac and they always try low sodium until my EP (who said to drink Gatorade/powerade daily) fixes it. You are trained on common conditions, then specialized after schooling. If you don't see a population you tend not to know much.

I have fibro since about 1986. Took a long time for doctors to believe that was what I had. Since then, I have had at least 50 surgeries, some serious, some easy and a lot in between. I was fortunate that one anesthesiologist told me something to tell all future anesthesiologists that I would encounter in order to prepare them for possible problems that they may not always encounter. I pop out of anesthesia, several times and it is horrible if they are cutting into you at that time... anyway, if they are prepared they will make sure nothing changes as they go. One of them however, told me, so nothing is different. I told him i had fibro and it is important to be prepared. That particular time I was having the first of 2 cataract surgeries. He was so arrogant I could not believe it. Well somewhere in the surgery, I popped out. The doctor started to yell at him, and of course he was not prepared as he did not believe me. The second surgery a few months later, same doctors, I again reminded him I have fibro and may pop out. He said ok, did not get nasty to me, and was prepared this time for when I did. My surgeries have lasted anywhere from 7 hours for a rod up the back of my neck to 2 hrs for opening every fingers tunnel so I could use my hand again as I had trigger finger. It is hard enough to have to undergo surgery after surgery and I have at least 3 coming within next 2 years, knees and wrist and had 3 last years breast cancer and 2 surgeries for a stim implant, and the person I look forward to speaking to, is your profession is the only one that will relieve my pain, at least for the duration of the surgery, you will be at peace. Sad but true. I only worry when I have the horrible firestorms before surgery , just how much that will interfere with it. But the pain is unending and they have tried everything, scanned everything and have left me barely able to walk, lay in bed with heat on my knees so I will not fall when I stand and that is where I stay. Sorry for making this so long, but it is so important and I get so much ignorance on this subject. Thank you for putting this information out there, and no, it is not in the patients mind, after the large number of surgeries I have had, I can attest to that.

I applaud what you are saying(speaking for us) ❤ I had sinus surgery and woke up with blood all over my neck and johnnie. Apparently, when they tried to wake me, i became extremely combative and it took 5 nurses to keep me down..they realized that i had to be woken up slower. I was definitely calm once i did wake up but Now, whenever i face any surgery that they need to be careful with me..i do have fibro, diabetes and hypothyroidism....i honesty believe that anesthesiologists are far more educated in people with chronic pain than most GPs..my GP told me the pain was in my head..months later he sent me to a neurologist who diagnosed my diabetes..i now go to a pain center and my doctor is an anesthesiologist 😊

Before I was officially diagnosed almost 20 yrs ago w/Fibro, my upper arm felt like I’d had a blood pressure cuff on WAY too tight for days. No other way to describe it. The pain lasted for 6 weeks going down to a massive ache in my forearm. Couldn’t even let my arm hang at my side. That symptom alone started the serious journey for a diagnosis. (Although I’d had different symptoms since late teens, leg aches, IBS etc. I can’t let my bp be taken on my left arm ever!

I was diagnosed with fibromyalgia in my 40s or 50s. I am 64 now. I have suffered Hypermobile joint issues since I was 2 and would have dislocated elbows from being picked up by my arms. I always told my Rheumatologist that each of my sore areas seemed more connected to my loose joints than anything. I wanted to be assessed for Ehlers-Danlos Syndrome, Hypermobile Type. My doc told me he did not diagnose this disorder and I would have to go to the University of Michigan to be assessed. Eventually, I had to stop working and had the time to be able to travel there for an appointment. I received a diagnosis of EDS Hypermobile Type. In past years, American doctors who specialized in EDS agreed that it often went along with Irritable Bowel Syndrome, POTS, and Mast Cell Activation Syndrome. We were also sometimes less sensitive to anesthetics, especially seen in dentistry with several shots of novocaine needed to numb the mouth. See Brad Tinkle’s writings from the early 2000s. In 2017, when EDS docs from around the world met to better define the symptoms and treatments for EDS, they drastically narrowed the criteria for EDS Hypermobile Type. Ours is the only EDS (of 12 or 13 types) that does not have identified genes as a cause. They took out the co-occurring disorders I listed above. Too many non-specific symptoms make it harder to identify the genes. the search goes on. Listening to you today, I hear you saying about Fibromyalgia the very same things that used to be said about EDS. Again, I find myself wondering if these patients were ever assessed for hypermobility. I cannot help but think that Hypermobile EDS and Fibromyalgia are the very same thing! I need a lot of novocaine to have dental work done. I have woken during surgery (just awareness, not able to feel pain). The most recent was during eye lens replacement and dental surgery. Have you heard of Ted’s Pain Cream? Two pain specialists at the University of Texas have put out a cream that uses Resveratrol to deaden pain impulses. I use it for my knees and thumbs and it helps my bone-on-bone knees be mostly fine. tedsbrainscience.com/pages/the-science

20+ years and not one medical professional has ever considered my fibro diagnosis or told me anything of you what you speak here. I just had the worst coming out of anesthesia from a carpal tunnel surgery. They have done nothing but gaslight my questions, new symptoms and pain from this since March of this year. Thank you for a valid reason what of possible portion of my experience.

I am nearly 60 and have allodynia so bad a tag in my clothing feels like it's sawing into my skin. I have fibromyalgia, adult-onset diabetes, IBS, widespread osteoarthritis, and rheumatoid arthritis. The worst damage to my body is bone-on-bone in both knees. After reading all the posts by people who were conscious during the "hammering" of the knees, I'm terrified. I'm used to pain; live with it daily; but they're going to have to knock me the heck OUT.

I am a kidney cancer survivor since 2020, and have been suffering through fibromyalgia, chronic pain, depression, anxiety, etc. since I was 10 yrs old, and it feels like so many healthcare professionals either 1. Don’t believe I have fibro, 2. Believe I’m just sensitive, or 3. Just a drug-seeker. Fibro and pain patients all get compared to their everyday patients and what’s standard for the ‘normal patient’ (which in my opinion, there is no such thing as a normal patient. If they were normal, they wouldn’t be a patient in the first place). I also used to work in inpatient and outpatient pharmacy stocking OR medications. The best career choice I ever made until I went on disability. ANYWAY, I almost immediately started to cry listening to your explanations about how everything is so different for people like me, and how ridiculous our bodies can react to pretty much every type of stimuli, even in the OR. I greatly appreciate you taking your valuable time in educating with facts and compassion about different scenarios and what you and hopefully other anesthesiologists do in order to cater to the patient in front of you. I have never heard such an accurate description of the what I feel 24/7 . I am really amazed at your acknowledgement and understanding of a devastating illness. I have been left housebound by it. Due for an op next month. Am 67 now had it 25 years. Thank you for your understanding and kindness.❤❤❤❤

I’ve had Fibromyalgia since I was 14, now I’m 60 years old. It seems no one understands how to help us. You have said more in the first two minutes of this video! Thank you, where have you been! I have fibromyalgia but after anesthesia I feel great and it lasted for about a whole week of feeling good. It almost felt like I was reset no pain. I thought I was loosing my mind but I'm glad it does the same for others.

You have such a good and comforting and compassionate and kind manner that good anesthesiologists should have. In every surgery I have been thru as I suffer from fibromyalgia and chrinuc spinal and arthritis pain along with atrial fibrillation condition and risk for blood clots and cpstd from alot if physical traumas in my life. And the effects of being the main care giver to four family members who I had to care for until they passed away which took a great toll on me having to go thru that many family deaths if loved ones really took a toll on my emotions and fear of dying. Everytime I went into the operating room for my surgery when the anesthesiologist came to talk to me and they were always like you very kind and comforting the thing I always asked them right when they were about to put me under and they asked me if i had any questions I always said yes, just please dont let me die. I was more worried about the anesthesia than the Dr doing the surgery because I knew that my life was literally in the hands of the anesthesiologist

I have fibromyalgia, EDS, POTS, OA, RA, a brain tumour and most recently cerebral aneurysm. I have had so many surgeries. In the space of 2 years I had 17 surgeries. But every surgery I have to have a GA because LA and nerve blocks don't actually work on me. They believe that it's due to my EDS... In one weeks time I will be having a total knee replacement on my right leg. Once recovered I'm due to get my left hip replaced. Then my left knee and my right hip. They also need to change the shoulder replacement on my left side. I'm tired, scared and extremely lonely due to my husband not understanding my condition or even excepting it. I have needed to leave my home, my husband and my children (ages 10 a because 11) because I am unable to live in our 3 storey house anymore. I am 40 years old and living in a body that's no longer fit for purpose.

Don't know if it was my request that prompted you to talk about this topic but I'm profoundly grateful, there is so much need to raise awareness about fibro/central sensitisation. My fibro looks was triggered by endometriosis and a series of surgical ops...no doc would believe that I could wake up in such agony. I wish someone would have looked into that right after the op and managed my pain control and come up with the right diagnosis. Thank you so much.

Diagnosed 23 yrs ago with fibromyalgia. I have endured several physical painful situations, I found it may have increased my pain level, however it increased my determination to heal.

I have fibromyalgia. It is great to see a video like this. I get sensory overload very easily. When I'm in the pre-op area, all the noise and movement puts me in a panic. And post-op too. That is the worst part of it all for me.

I have fibromyalgia and Hemiplegic migraines and other chronic migraines. One time under anesthesia I awoke to the worst HM ever. Complete numbness from toe to shoulder. Locked in unable to,speak. I was rushed to er and treated as stroke. About 5 hours later voice and eventually limbs came back. It is so hard to know that you can hear but they don’t know you can hear. They rushed my husband in and he was able to explain HM to the anesthesiologist and the nurses and was able to ask me questions that I could answer by squeezing with my moveable right hand. I feared. On my next procedure a year later and let the surgeon and anesthesiologist of the previous response of HM and the anesthesiologists was very tender in his approach. I recall I came out talking to my best friend up in Heaven and she said it wasn’t my time yet, I had to go back. I kept telling anyone and everyone…”I saw Alison”. And was at such happy peace.

I’m almost 65 with PTSD (60 years), diabetes (35 years) and Fibromyalgia (20 years). I’m finally seeing something else besides your crazy or lazy! I’ve been very very sensitive my whole life, so much so that I figured I was an Empath, and still believe I am. I love all this new information!! I’ve actually cut gluten out of diet, and am looking at allergies and my diet. Thank you!!

Thank you for being an advocate for people with fibro. Still there are MDs that consider fibro a "garbage diagnosis." With osteoarthritis and psoriatic arthritis I often feel fibromyalgia is a distant third place when medical personnel see me. I hope that you represent the future of medicine.

You just really helped me out a lot!! I have fibromyalgia and struggle with anesthesia. What you said makes so much sense. I struggle with my pain response in recovery room. I am going to talk to my anesthesiologist more before my next surgery. Last time I was given vercet before going to OR and that helped a lot! Thank you for all the information!!
