Only 10x 180s were kill with this method. You're able to gain some XP, and still remain level 3. That XP is used to poison the rest of the 180s. It isn't enough to realistically poison all 34 of them, so 10 were recoiled. The entire cave from start to finish was ~40 hours.

The ring isn't worn. Few years ago there was an arms race between Rendi and Xzact for lowest level fire cape. At certain point the fact that they couldn't store all the recoils needed(as the accounts had so little exp to spare that recoils were becoming mandatory)became problematic. But someone realized that adding a ring of recoil to suffering resets your ring of recoil counter. And this can be done with NOTED rings of recoil! So as long as you add noted ring of recoil before the physical one on you breaks, you can keep doing this indefinitely. So that's why he has ring of suffering on him(and few thousand noted recoils)

Tick eating. Many non-melee attacks calculate their damage in the game tick before the damage is dealt. As a result, if you heal in the same tick that the damage is dealt, the game will have already decided how much damage you will take from the attack. If you only had 1 hp when the damage was calculated, the game will deal that much damage in the next tick, so if you eat you will take 1 and heal at the same time, leaving you above zero. This method is also used for several high-end strats in places like the Theatre of Blood, where players will tick-eat Sotetseg's giant ball attack while on low health in order to preserve supplies, and also on Verzik's final phase when she uses a similar attack. However, you can't do this with melee attacks, as they calculate and deal the damage in the same tick. You have to figure out a way to stall the damage using some other animation (which is done with the kitten in this example), and also to heal in the ticks between the damage being calculated and dealt. Since you can't eat during the kitten animation, that leaves only natural regeneration as a method of healing.
