It's revealed the reason Amy mistook Shadow (and later Silver) for Sonic is that she is legally blind and needs glasses. This was said by Sega themselves I believe. Probably there way of explaining this.

Okay so legit attempt to answer why, the reason everyone mistook Shadow for Sonic, is because Shadows original design when he was called Tarios, looked ALOT more like Sonic. But his design was changed late into the development, and they just didn't really change the mistaken identity plot. Just look up Sonic Tarios or somethin like that.

IIRC, Big the cat is in all versions of the game, but in the old versions he would only start appearing in cutscenes and levels after you did something specific, don't remember what tho, when the game was getting ported to newer systems they decided to just have him show up from the get-go

The reason for the Sonic-Shadow confusion is simply because Amy goes to Prison Island, a human military facility, to rescue Sonic, and when she's there she sees a hedgehog with a similar shape to him, and just makes the connection before taking a proper look. When she does take a proper look, she sees it's clearly not him. Eggman makes a similar mistake, seeing Shadow from afar in a dark room and expecting if any hedgehog is gonna show up while he's doing an evil, it'd be Sonic. The public just see a fast-moving hedgehog who only stops way up high at night, and again, his shape is similar to Sonic's so eye-witnesses just assume it was him. The media only reports what the eye-witnesses said, they don't actually call Shadow Sonic themselves. GUN's higher-ups used Sonic as a scapegoat at first to cover-up Shadow's escape from Prison Island to any soldiers that saw him breaking out, which is why they're after Sonic even before the Emerald heist, and then after Prison Island blows up they double down on the cover-up to try to save face.
