@drimtiazhussain Dr. Sb, with due respect many people sale out fear , means , plz avoid surgeries bcz number of cases What I heard surgeries has been done in the sense greed , so plz try to avoid surgeries, surgery must be the last option not the first one , research what was done before surgery for this issue. Thanks a lot ❤and respect

We appreciate your feedback. Perianal fistulas typically require surgical intervention for complete healing, as they involve an abnormal connection between the anal canal and the skin near the anus. While medications alone are generally not sufficient to treat a fistula, they can help manage symptoms and prevent infections. Here's a breakdown of the treatment options and why surgery is often recommended: ### Why Surgery is Often Necessary 1. **Complex Structure:** - A fistula is a tunnel-like structure that usually cannot heal on its own because the internal opening continues to drain and may become infected. 2. **Recurrence Risk:** - Without surgical intervention, the fistula may persist or recur, leading to ongoing discomfort and the risk of complications. 3. **Infection Control:** - Surgery helps remove the infected tissue and close the abnormal connection, reducing the risk of recurrent infections or abscess formation. ### Role of Medication - **Antibiotics:** - Used to treat or prevent infection but cannot close the fistula. - **Pain Management:** - Pain relievers can help manage discomfort but do not address the underlying problem. - **Anti-inflammatory Drugs:** - These can reduce inflammation and swelling but are not a cure. ### Non-Surgical Approaches in History In the past, surgery may not have been as commonly performed due to limitations in medical knowledge, techniques, and anesthesia. Historical treatments may have included: - **Poultices and Herbal Remedies:** - Used to manage symptoms but not to cure the fistula. - **Natural Healing:** - Some fistulas may have healed naturally if they were simple or drained effectively, but this is rare. ### Modern Surgery for Fistulas - **Fistulotomy:** - A common procedure where the fistula tract is opened and allowed to heal from the inside out. - **Seton Placement:** - A surgical thread is placed in the fistula to help it drain and heal. - **Advanced Techniques:** - Options like the LIFT procedure or fibrin glue are less invasive but require surgical expertise. ### Why Surgery is Recommended Today - **Higher Success Rates:** - Modern surgical techniques have high success rates and low recurrence rates. - **Safety and Precision:** - With advances in anesthesia and surgical techniques, surgery is safer and more precise. - **Patient Quality of Life:** - Surgery provides a long-term solution, improving patient comfort and quality of life. ### Conclusion While it may seem that surgery is promoted more today, this is because of advances in medical science that offer effective and reliable solutions to a problem that historically had few options. If you are considering treatment for a perianal fistula, it is essential to discuss all available options with a qualified healthcare provider to determine the best approach for your specific situation.

It means , Dr. Sb, there is no permanent solution, what is the purpose of surgical process necessary? simply, to care about the abscess & use some medicine and mustard oil on perianal area after cleaning with warm water of neem leaves/ sitz bath ...

@drimtiazhussain 2 ай бұрын
I'm sorry to hear about your struggle with fecal incontinence for so long. Fecal incontinence can significantly impact your quality of life, but there are several treatment options available, including surgery. Here’s a guide to help you consider your options: ### Evaluation Before considering surgery, it's essential to have a thorough evaluation by a specialist, such as a gastroenterologist or a colorectal surgeon. They will assess the severity of your condition and the underlying causes, which can include muscle or nerve damage, pelvic floor dysfunction, or prior surgeries. ### Non-Surgical Treatments 1. **Dietary Changes:** - Increase fiber intake to help firm up stools and regulate bowel movements. - Stay hydrated but monitor fluid intake to prevent diarrhea. 2. **Medications:** - Anti-diarrheal medications like loperamide can help manage symptoms by slowing down bowel movements. 3. **Pelvic Floor Exercises:** - Kegel exercises can strengthen the pelvic floor muscles and improve control over bowel movements. - Biofeedback therapy might be suggested to enhance muscle coordination. 4. **Bowel Training:** - Establishing a regular bathroom schedule can help manage symptoms. 5. **Absorbent Products:** - Use pads or adult diapers for protection, especially when leaving home. ### Surgical Options If non-surgical treatments are not effective, surgery may be considered. Here are some common surgical options: 1. **Sphincteroplasty:** - Repairs damaged or weakened anal sphincter muscles, often due to childbirth or injury. 2. **Sacral Nerve Stimulation:** - A small device is implanted to stimulate the nerves that control bowel function, which can improve muscle coordination and strength. 3. **Artificial Bowel Sphincter:** - A device that mimics the function of a healthy sphincter, implanted surgically to help control bowel movements. 4. **Colostomy:** - In severe cases, a colostomy may be performed, where the colon is diverted to an opening in the abdomen to collect stool in a bag. This is usually a last resort when other treatments have failed. ### Considerations - **Consultation:** Discuss with a specialist to understand the risks and benefits of each treatment option. - **Lifestyle Impact:** Consider how each option might affect your daily life and which aligns best with your preferences and goals. - **Support:** Seek support from groups or counselors who understand the challenges of living with fecal incontinence. ### Final Advice It’s crucial to consult with a healthcare professional who can tailor advice to your specific situation. They can help determine the best course of action based on the cause and severity of your fecal incontinence and any other health conditions you may have.

Fistula in ano surgery ke baad kuch logon ko stool control mein temporary issues ho sakte hain, khaaskar agar surgery ke dauraan anal sphincter muscles ko bhi touch kiya gaya ho. Yeh condition aksar temporary hoti hai aur waqt ke sath theek ho sakti hai jab healing hoti hai aur muscles apne normal function par wapas aate hain. Lekin, agar aapko lagta hai ke yeh masla zyada waqt tak rahe raha hai, toh yeh zaroori hai ke aap apne surgeon ya doctor se mashwara karein. Woh aapki condition ko assess karke aapko behtar guidance de sakte hain. Kabhi kabar, pelvic floor exercises ya physiotherapy bhi madadgar sabit ho sakti hai stool control ko behtar karne mein. Agar aapko koi aur symptoms ho rahe hain, jaise ke severe pain, infection, ya bleeding, toh foran apny surgeon doctor se rabta karein.

Coconut oil laganay se aam tor par koi nuksan ya side effect nahi hota, lekin har kisi ki skin ka reaction alag ho sakta hai. Yeh achha moisturizer hai aur isse infection ka khatra bhi kam ho sakta hai. Lekin agar aapko koi allergy ya irritation mehsoos hoti hai, toh istemal band kar dein aur kisi doctor se mashwara karein.

@drimtiazhussain
@drimtiazhussain 2 ай бұрын

Agar aapko perianal fistula ka operation hue ek saal ek mahina ho gaya hai aur abhi bhi thoda bohot white ya halka discharge aa raha hai, toh yeh kisi bhi nafradi infection ya incomplete healing ki nishani ho sakta hai. Yeh cheezein madadgar ho sakti hain: 1. **Follow-up Appointment:** Doctor se consult karein. Woh aapko batayenge ke ye discharge normal hai ya nahi, aur koi additional treatment zaroori hai ya nahi. 2. **Hygiene:** Affected area ko saaf aur dry rakhein. Isse kisi bhi nafradi infection ka chance kam ho sakta hai. 3. **Watch for Other Symptoms:** Agar discharge ke saath dard, redness, swelling, ya fever ho, toh ye infection ki nishani ho sakti hai aur aapko foran doctor se milna chahiye. 4. **Diet and Lifestyle:** High-fiber diet lein aur hydration maintain karein taake aapki bowel movements smooth rahein aur area pe unnecessary pressure na pade. Agar discharge ki wajah se koi dikkat nahi hai lekin aap phir bhi pareshaan hain, toh ye behtar hoga ke aap apne doctor se discuss karein taake wo koi bhi zaroori test ya examination kar sakein.

Perianal abscess ke baad discharge ya raisha ana kabhi kabhi normal ho sakta hai, lekin agar yeh lambi muddat tak chalta rahe, to iski janch zaroori hai. Kuch tadabeer aur options jo aapko consider karne chahiye: 1. **Second Opinion:** - Agar aapko shak hai ki fistula ho sakta hai, to kisi doosre specialist se second opinion lein. Fistula ke case mein raisha ana aam baat hai, lekin agar aapka doctor kehta hai ki fistula nahi hai, to shayad dusri medical janch ki zarurat ho. 2. **Imaging Studies:** - MRI ya ultrasound jaisi imaging studies karayi ja sakti hain taake fistula ya kisi aur underlying issue ko rule out kiya ja sake. 3. **Follow-up Appointments:** - Apne doctor se follow-up appointments zaroor rakhein. Unhe aapke symptoms ke bare mein batate rahein taake wo proper management kar sakein. 4. **Wound Care:** - Jahan se discharge ho raha hai us area ki achi tarah se safaai aur proper dressing zaroori hai taake infection na ho. 5. **Lifestyle Changes:** - Apni diet aur lifestyle mein aise changes karein jo aapke immune system ko support karein, jaise ke hydrating, healthy eating, aur stress management. Agar aapko koi aur severe symptoms jaise bukhar, badboo dar discharge, ya intense pain ho, to foran medical attention lein. Yeh kisi infection ka ishara bhi ho sakte hain jiska foran ilaj ki zaroorat ho.

Seton removal ke baad agar ab bhi fistula ka hole maujood hai aur wahan se air pass ho rahi hai, toh yeh indicate kar sakta hai ke fistula abhi poori tarah se heal nahi hua hai ya koi chhota tract abhi bhi baaki hai. Iska treatment ke liye aap ko kuch steps lene ki zarurat ho sakti hai: 1. **Doctor Se Consult Karein**: Sabse pehle, apne doctor ya surgeon se consult karein. Woh aapki condition ko evaluate karke pata laga sakte hain ke kya koi further intervention ki zarurat hai. 2. **Imaging Studies**: Kabhi kabhar, doctor MRI ya ultrasound jese imaging studies recommend karte hain taake dekh sakein ke fistula tract kitna bada hai aur kya woh poori tarah se heal ho gaya hai ya nahi. 3. **Repeat Surgery**: Agar fistula tract ab bhi maujood hai, toh doctor aapko repeat surgery ya koi aur procedure jese advancement flap ya fistulotomy suggest kar sakte hain. 4. **Wound Care**: Achhi wound care maintain karein. Cleanliness aur proper dressing zaroori hai taake infection se bacha ja sake aur healing achhi ho. 5. **Regular Follow-ups**: Apne doctor ke saath regular follow-up visits zaroor rakhein taake woh aapki recovery ko monitor kar sakein aur kisi bhi complication ko timely address kar sakein. Agar aapko kisi bhi waqt koi aur symptoms mehsoos hote hain jese ke pain, bleeding, ya discharge, toh foran apne doctor se rabta karein. Proper diagnosis aur treatment plan ke liye medical assessment zaroori hota hai.
